A 1.5-yr-old captive male Gambian giant pouched rat (Cricetomys gambianus) died after suffering from anorexia,
weakness
, and dyspnea for 3 wk.
Thoracic
radiographs of thorax and abdomen and computed tomography showed a severe biventricular enlargement of the heart and a moderate hepatomegaly. Necropsy revealed a severe, bilateral hypertrophic cardiomyopathy and dilation of the right ventricle due to multifocal bilateral, valvular endocardiosis of all atrioventricular valves and acute hepatic congestion. Histologically, the atrioventricular valves were multifocally thickened by a marked endocardiosis with stromal accumulation of Periodic acid-Schiff (PAS)-positive, mucinous material. Although common in dogs, endocardiosis has not been described in Nesomyidaes. As in other affected species, the pathogenesis of endocardiosis in this species remains unclear.

We report a case of sudden onset of paraplegia shortly after thoracic epidural catheterization for postoperative analgesia and discuss the possible causes of this event. A 38-year-old woman was scheduled to receive right lobectomy of liver because of hepatocellular carcinoma.
Thoracic
epidural catheterization for postoperative analgesia was performed before the induction of anesthesia. After skin disinfection and local anesthetic skin infiltration with lidocaine, epidural catheterization through T(10-11) interspace was performed. Dural puncture without any neurological symptoms was noticed in the attempt and the epidural space was successfully identified through T(9-10) interspace in the second attempt. However, acute motor
weakness
and sensory impairment were met as the epidural catheter was being threaded into the epidural space. Magnetic resonance imaging (MRI) revealed no abnormal findings and the neurological deficits resolved spontaneously within 2h without any sequela. Finally, it was supposed that the transient neurological deficits were resultant from accidental subarachnoid injection of the local anesthetics used for skin infiltration. Preoperative image studies of the spine revealed a relatively short skin-to-dura distance either from median or paramedian approach, which might be the cause of the inadvertent intrathecal injection of local anesthetic during skin infiltration.

An 11-year-old, spayed female, teacup poodle was evaluated for a chronic cough, lethargy, hindlimb
weakness
, and reluctance to exercise.
Thoracic
radiographs revealed megaesophagus and aspiration pneumonia. Serum antibodies against acetylcholine receptors confirmed the diagnosis of myasthenia gravis. The unusual clinical history and case outcome are discussed.

Nontuberculous mycobacterium (NTM) species are mycobacterial species other than those belonging to the Mycobacterium tuberculosis complex and M. leprae. NTM are generally free-living organisms that are ubiquitous in the environment. Pulmonary disease, especially in older persons with and without underlying lung disease, is caused primarily by M. avium complex (MAC) and M. kansasii. The symptoms and signs of MAC lung disease are variable and not specific, but include cough, malaise,
weakness
, dyspnoea, chest discomfort and occasionally hemoptoe. Two major clinical presentations include disease in those with underlying lung disease, primarily white, middle-aged or elderly men - often alcoholics and/or smokers with underlying chronic obstructive lung disease, patients in whom MAC develops in areas of prior bronchiectasis, and patients with cystic fibrosis; and those without known underlying lung disease, including non-smoking women over age 50 who have interstitial patterns on chest radiography. M. kansasii infections are endemic in cities with infected tap water. Symptoms of the M. kansasii lung disease resemble to tuberculosis. M. abszessus is the most pathogenic rapid growing Mycobacterium which causes pulmonary infection. The American
Thoracic
Society and Infectious Disease Society of America's diagnostic criteria for nontuberculous mycobacterial pulmonary infections include both imaging studies consistent with pulmonary disease and recurrent isolation of mycobacteria from sputum or isolated from at least one bronchial wash in a symptomatic patient. For treatment of MAC lung disease we recommend depending on severity and susceptibility testing a three to four drug treatment with a macrolide, rifampicin and ethambutol and for M. kansasii a treatment with Isoniazid, rifampicin and ethambutol. Surgical management only plays a role in rare and special cases. Treatment should be continued until sputum cultures are consecutively negative for at least one year.

A seven-year-old male Jack Russell terrier was presented with a history of coughing,
generalised weakness
and lethargy 10 days after an abdominal coeliotomy to repair a large diaphragmatic rupture.
Thoracic
radiographs demonstrated a soft tissue mass in the midcaudal right thoracic cavity. Ultrasonographic studies, bronchoscopy and subsequent exploratory thoracotomy confirmed a diagnosis of a right cranial lung lobe torsion (LLT), with an anomalous caudodorsal displacement of the affected lobe. LLT should be considered as a differential diagnosis for respiratory tract disease following diaphragmatic rupture repair.

A 6 yr old domestic longhair cat was evaluated for progressive weight loss,
weakness
, and dyspnea. Results of a physical examination and electrocardiogram were suggestive of cardiac disease.
Thoracic
radiographs revealed pleural effusion, which thoracocentesis revealed was consistent with chyle. An echocardiogram was performed, and aortic valve endocarditis with secondary aortic insufficiency was presumptively diagnosed. The cat was treated with broad-spectrum oral antibiotics and palliative cardiac medications. Two days after discharge, the cat's dyspnea returned, and it died suddenly. Histopathology and culture confirmed Pseudomonas bacterial endocarditis of the aortic valve. Bacterial endocarditis in the cat has rarely been reported in the literature. This case described heart failure and chylothorax resulting from bacterial endocarditis.

All pulmonologists, including those recently completing training, should be competent in critically evaluating and interpreting pulmonary function tests (PFTs). In addition, some authorities recommend that respiratory therapists learn to provide preliminary PFT interpretations for the medical directors of PFT labs. The 2005 American
Thoracic
Society/European Respiratory Society guidelines for interpreting PFTs lack recommendations for the best reference equations for lung volumes and diffusing capacity of the lung for carbon monoxide (D(LCO)), and lack reference equations for non-whites. The pre-test probability of lung disease should be determined using a short questionnaire. The "nonspecific pattern" occurs in about 15% of patients referred to a PFT lab, but it has many clinical correlates and the course is usually benign. Less common PFT patterns and those resulting from comorbid conditions (such as obesity, respiratory muscle
weakness
, or heart failure) are not discussed by the guidelines. More than half of patients with interstitial lung disease have a normal ratio of D(LCO)/V(A) (alveolar volume), and many have a normal total lung capacity.

ossification of the posterior longitudinal ligament is a rare but debilitating condition. It is most commonly diagnosed in the Japanese population. If left untreated, significant myelopathy and
weakness
can ensue. Anterior decompression and stabilization is effective but technically demanding and is associated with high rate of complications. Posterior laminectomy appears to be an acceptable approach with lower risks. This work reviews the published literature on this topic and highlights the salient points.

An eight-month-old male child was admitted with
weakness
and swelling in the feet. Paraparesis and bilateral lower extremity edema were present in the neurological examination.
Thoracic
MRI showed an intradural intramedullary mass 61 x 11 mm in size in the T5-T10 levels. Laminotomy between the T5-T10 vertebrae was performed. A mass with smooth borders was separated from most of the neural tissue. In the postoperative MRI, we observed a contrast enhancing area, considered a residual fragment, only 5 x 4 mm in size. Histopathological properties were compatible with the intermixed subtype of ganglioneuroblastoma. Only a limited number of thoracic cord Ganglioneuroblastoma reports have been previously published. Although very rare in children and young adults, ganglioneuroblastoma should be included in the differential diagnosis of thoracic cord tumors. It is difficult to obtain a preoperative diagnosis with clinical features and radiological investigations. Diagnosis depends on histopathological examinations. Curative treatment should be in the form of a complete resection of the tumor. In partially resected cases, adjuvant radiotherapy may become necessary, along with close follow-up.
