Society to review and bring up to date guidelines on control measures for tuberculosis. The updated code of practice emphasises that all cases of tuberculosis must be notified. A minority of patients need admission, and those with positive sputum smears should be regarded as infectious until they have received two weeks of chemotherapy. NHS staff at risk should be protected, and evidence of infectious tuberculosis should be sought as routine among certain prospective NHS employees, schoolteachers, and others. Contact tracing should be vigorously pursued, and all entrants to Britain from countries where tuberculosis is common should be screened. BCG vaccination should be offered in selected instances, and local organisation of tuberculosis services should be extended.

Society (BTS) recommends that all new prison staff be screened for tuberculosis (TB) as 'at risk' health workers. This study of prisons in the West Midlands area of England shows that there are considerable variations in the practice of TB control amongst prison staff and that the recommendations of the BTS committee are not routinely implemented. The study highlights the need for a routine and robust system of TB surveillance and prevention amongst prison staff which can be applied nationwide.

Society in 2000 were monitored prospectively from late 2000 until the end of 2003. No cases in contacts that could have been detected by interval X-rays at three and 12 months were found. The data, on 291 cases, support the abandonment of X-ray follow-up in favour of an 'inform and advise' strategy after an initial normal chest X-ray in this category of tuberculosis contact.

Screening of the close contacts of patients with pulmonary tuberculosis remains an important component in the control and prevention of the disease. It is carried out to identify active and latent infection, and those requiring BCG vaccination. Guidelines suggest giving chemoprophylaxis to asymptomatic contacts with a positive Heaf test (grades 2-4) and normal chest radiograph [Control and prevention of tuberculosis in the United Kingdom: code of practice 2000.
