The authors have utilized a computer for a spatial analysis of deformities of the spine using antero-posterior and lateral radiographs. The posterior limits of the sacral plateau and the centre of each vertebral plateau was demarcated on the antero-posterior radiograph. The posterior margin of each vertebra was demarcated on the lateral radiograph. With this information, the computer can make a picture as if the spine were seen from above and vertebral rotation can be assessed. The vertebrae are of different colours according to their level. Thirty cases have been studied and six types of infantile

In 39 cases of narrowed lumbar spinal canal associated with a
scoliosis
, the authors have studied the role played by deviation of the spine and the origin of the narrowing. It seemed that central stenosis with intermittent claudication was more frequent when a
scoliosis
was present. In more than half of the cases, the level of the compression was at the junction of the curves. After failure of conservative treatment, the surgical procedure performed was the same as in straight spines. The results were satisfactory in 90 p. 100 as regards root involvement and intermittent claudication. However 7 patients had increased low back pain after surgery, 3 of them being disabled. In young patients with curves of more than 30 degrees with noticeable rotation, straightening with arthrodesis appeared to be worthwhile rather than simple posterior laminectomy. In other cases a posterior liberation of the cord associated with limited posterolateral fusion is indicated.

PMID:[Lumbar stenosis with scoliosis. Symptomatologic study and surgical treatment of 39 cases]. 624 90
Ten patients suffering from Duchenne muscular dystrophy were followed up for more than 7 years. The diagnosis was determined by biopsy. Progress of the disease was severe and all patients died before the age of 18. The average angle of the
scoliosis
at the end of this development was 73 degrees. The maximum vital capacity was 1198 ml. The authors conclude that neither the use of a brace nor adaptation of a wheelchair are helpful. Surgical treatment is hazardous when the curve is greater than 23 degrees. The indications for such treatment are based on the importance of the degree of pulmonary restriction and on early signs of severe progress of the
scoliosis
.

A follow-up study on spine deformities of 59 consecutive Wilm's tumors treated between 1950 and 1963 was conducted. The selected cases had no metastasis at any time and had reached end of growth (16 years old of bone age). Treatment was surgery, chemotherapy and radiationtherapy mostly was 200 KV and partially with CO 60. This study showed 65p. 100 kyphosis greater than 10 degree between T9 and L3 (normal less than 5 degree) and 40p. 100

both induced by alteration of vertebral growth and by sclerosis of soft tissues. -- Kyphosis even slight seemed to induce more pain in adult age than

. -- This work demonstrates also that growth spurt produces increased deformities. Therefore it is necessary to wait the end of growth before the evaluation of the results. Finally the most important for prevention is to irradiate as symmetrically as possible in the 3 planes of the body. The results of surgical treatment are reported and principles of early bracing for prevention are mentioned.

21 cases have been observed and followed up for 10 years. All were children. Three types of anomaly were noted: Spinal curvature (mostly
scoliosis
), skin abnormalities in the mid line (hypertrichosis, angioma, skin dimples) and problems in the lower limbs (varus deformities of the feet, leg length discrepancy, muscular atrophy, neurological disorders). These signs, even if not all are present, must lead to a study of the vertebral bodies and to gas myelography whcih demonstrates a double spinal cord over a variable length from 2 to 14 segments. 15 children were treated surgically by mobilisation of the spinal cord and arthrodesis of the spine. 4 of them were made worse. In contrast, none of the 6 children treated conservately was made worse. The authors conclude that surgery should be reserved for recent spontaneous neurological deterioration and severe deformities of the spine.

The authors consider that scoliotic curves should be studied in the spatial planes, not only in the frontal plane. Lateral views show that the displaced segments of the spine are always an extension even when there is a kypho-
scoliosis
. This element must be considered when the deformity is about to be reduced or treated by bracing. In the axial plane, the rotational displacement must be measured. In a dorsal curve, there is specific rotation of the upper vertebrae. The measurement of this rotation may make it possible to predict the extent of the curve at the end of growth in cases of spontaneous evolution.

Double-curved
scoliosis
was present in 18.5 of 750 scolioses operated on at the St. Justine Hospital in Montreal. 128 cases were reviewed with a follow up of more than 3 years. Pre-operative curves measured 61 and 58 degrees on average and post-operative curves measured 27 and 23 degrees. At the time of review, the curves were 39 and 36 degrees. The loss of correction varied with the age of the patient and the severity of deformity. The technique of early ambulation improved the post-operative results. However, it was concluded that compression rods did not provide a better correction of the curve or of the thoracic kyphosis.

The technique used by the authors for removal of hemivertebrae is fully described. Two approaches are made, one posterior and one lateral at the same surgical session. This allows satisfactory control of roots, medulla and radicular vessels. The indications are described with reference to the age of the patient, the level of the lesion and the type of anomaly. It is concluded that it is a valuable procedure in progressive severe congenital
scoliosis
. 7 cases are reported without any neurological complication.

With the goal to reduce the number and the volume of homologous blood transfusions during surgical treatment of
scoliosis
and spine deformities, the authors have studied the results of 50 cases treated with immediate acute haemodilutional autotransfusion. In 40 per cent of the case neither per nor post operative homologous blood transfusion was necessary. In addition they have seen a surprising result such as a significant lowering of absolute blood loss during surgery compared to a similar series treated without haemodilutional autotransfusion. Physiopathological principles, precise technique used and limitation of the method are described.

The authors report an observation which is interesting in many points of view. 1. A homocystinuria was found in a 7 year old child. The diagnosis of this disease in the sister of the patient led to make a family schedule and permitted to connect to this homocystinuria the entirety of the pathological signs found in the patient, particularly a serious

. 2. Because of the rarity, the severity of the vital prognosis and the risk of thrombo-embolic complications of the homocystinuria any important surgical intervention was given up until the age of 13 years, but the aggravation of the
scoliosis
at this moment made an operation necessary. After an orthopaedic, dietetic and anti-aggregative preparation, a posterior vertebral arthrodesis was done without any incidence. 3. One year after this intervention, the orthopaedic as well as the medical evolution have been favourable, and there have been no accident to deplore, especially no thrombo-embolism.
