An analysis of the local and systemic perioperative complications is conducted to explore the risk of resection of
colon cancer
. In a retrospective study we analyzed 231 consecutive patients operated on between 1984 and 1988. The mean age was 70 (37-91) years. The operations consisted in 3 ileocecal resections, 144 right hemicolectomies, 10 resections of the transverse colon, 22 left hemicolectomies, 77 resections of the sigmoid colon and 5 subtotal colonic resections. 2 patients (0.9%) had a clinical leakage of the anastomosis. 3 patients were reoperated: one because of anastomotic leakage and two because of
ileus
due to small bowel adhesions. 2 patients with uncomplicated local healing died within 30 days after the operation from systemic complications (mortality 0.9%). It is concluded that with standardized preoperative bowel preparation, prophylactic perioperative antibiotics and modern anesthesia, the resection of
colon cancer
is today possible with minimal perioperative risk.

The authors analyse 62 patients suffering from
carcinoma of the colon
with metastases in the lungs. Six patients were operated on not only for primary tumor of the colon but also in solitary metastatic affection of the lungs. The interval during which a solitary metastasis in the lung was detected lasted 18.1 months on average. Palliative resection of the colon when distal metastases were found in the lungs was conducted in 21 patients; in 7 of these patients, metastases in the lungs were recognized before the operation while in 2 the diagnosed solitary metastasis was mistaken for carcinoma of the lung and only during operation for
ileus
was the tumor of the colon discovered. The postoperative mortality was 23.8%, in 17.7%, death was caused by incompetence of anastomosis sutures.

Mean age was 62.0 years (range, 36 to 73); four were male and two were female. Case 1 and 2 were admitted with anterior chest pain, Case 3 with lumbago and abdominal pain, Case 4 and 5 with an abnormal shadow on chest x-ray film and Case 6 with abdominal pain. , one was operated on with right upper lobectomy during about 5 weeks after right hemi-colectomy. Case 3 with abdominal aortic aneurysm operated on with left upper lobectomy during 4 weeks after replacement of abdominal aorta. Case 4 with cholecystitis was operated on with left pneumonectomy during about 3 weeks after cholecystectomy. The postoperative course of 4 cases and the post-chemotherapy condition of 2 cases were uneventful.

Reported is the case of a 66-year-old woman who complained of a pain in the upper abdomen. A barium enema revealed a stenosis in the transverse and sigmoid colon and since her
ileus
worsened, an emergency operation was performed, which revealed an unresectable transverse
colon cancer
with a diffuse peritoneal metastases. After closing the wound, the patient was treated with local thermotherapy of the abdomen using an RF wave in combination with chemotherapy and immunotherapy. Later, since the tumor could not be palpated and the tumor markers dissipated, a reoperation was performed, and it was found that diffuse metastases had completely disappeared from the peritoneum. Further, a histopathological study did not disclose any tumor cells. Therefore, as the cancer was remarkably reduced, a partial transverse and descending colon colectomy was performed.

A 51-year-old man with congenital diaphragmatic hernia and enterothorax was found to have persisting leucocytosis (25,000/microliters), diarrhoea and weight loss (20 kg). Computed tomography (CT) revealed intrahepatic space-occupying lesions. CT-directed needle biopsy demonstrated adenocarcinoma metastases. Colon contrast enema was ambiguous. Since no primary tumour had been found, ambulatory treatment with 5-fluorouracil was started. After initial improvement diarrhoea and obstipation alternated so that the patient finally gave permission for coloscopy to which he had not consented at first. It revealed a
carcinoma of the colon
located in the thorax about 10 cm oral to the left colonic flexure. Progressive
ileus
necessitated an ileodescendostomy for palliation. The patient died three months later while on symptomatic treatment.

were analyzed. Operations for acute

were performed in 147 patients, for perforation of the bowel and peritonitis in 45 patients, for profuse intestinal bleedings in 23 patients. The lethality after the urgent operations was 32.5%. Operations with the removal of the primary tumor give better direct and remote results.

Over the period 1963 to July 30, 1978 a total of 312 patients with clinical picture of acute or subacute mechanical

were admitted to the surgical department of the district hospital in Targovishte, and 227 of them were operated on as emergency cases. During the first period covering 1963-1970, the number of operated patients amounted to 78 of which eighteen - a 23.07 per cent lethality. During the second out of 149 patients operated on twenty-three died which makes a 15.63 per cent lethality. Since April 1990, multiple metastatic liver tumors have been treated with intraoperative ultrasound-guided microwave tumor coagulation (MTC) in combination with postoperative intraarterial chemotherapy. Twenty-four patients have been enrolled in this therapeutic modality and two of five patients treated in 1990 achieved long-term survival. In case 1, a 67-year-old woman was diagnosed as bearing gastric leiomyosarcoma with multiple liver metastasis. Total gastrectomy was performed, and six hepatic lesions were treated by MTC along with implantation of an intraarterial reservoir. Postoperative intraarterial chemotherapy was administered in a selective or subselective manner. Her survival time was 4 years and 6 months. In case 2, a 47-year-old man was diagnosed as having liver metastasis from descending
colon cancer
1 year and 5 months after left hemicolectomy. When he was laparotomized for the treatment of adhesive
ileus
, three metastatic liver tumors were treated with MTC. Afterward, he underwent hepatic resection, intraarterial and intraportal chemotherapy. He survived for 5 years and 5 months after being diagnosed with liver metastasis. MTC is one of the useful modalities for the treatment of multiple liver metastasis.
