The authors report a few unusual cases of Crohn's disease of the colon and discuss the diagnostic problems which are raised in such cases. They then review the various colonic diseases liable to be considered in a differential diagnosis, and which may be linked to colonic Crohn's disease, e.g. ulcerative colitis, intestinal tuberculosis, carcinoma of the colon, colonic diverticulosis and sarcoidosis. In a chapter on treatment, they emphasize the favourable effect of acexamic acid and the necessity for very wide colonic resection, and they note a complication which they encountered during surgery for Crohn's disease: lymphatic peritonitis on the 8th day after right colectomy.

Extrapulmonary tuberculosis is more frequent in hemodialysis patients than in the general population but intestinal localization is an unusual presentation of this infectious disease. We report a 60 year old patient on regular hemodialysis with intestinal tuberculosis masquerading as colon cancer. The patient presented with rectal bleeding, abdominal pain and fever and the radiological findings were compatible with ileocecal carcinoma. After surgery histological examination showed non-caseating granulomas but mycobacterial culture was not available. We performed a colonoscopy and obtained a biopsy of colonic mucosa for culture and other analyses. We identified acid-fast bacilli with Ziehl-Neelsen staining of formaldehyde preserved, paraffin-embedded tissue from the hemicolectomy and the colonic mucosal biopsy. Treatment with isoniazid, rifampicin and pyrazinamide for nine months was successful and well tolerated. Intestinal tuberculosis is a rare entity that we must keep in mind in a patient with abdominal pain, unexplained fever, digestive bleeding and particularly with a positive tuberculin reaction. When culture is not possible we can obtain intestinal samples by colonoscopy and use appropriate staining of paraffin-embedded tissues.

In recent years there has been an increase in the number of cases of intestinal tuberculosis mainly due to the HIV epidemic. Its clinical manifestations and endoscopic findings are nonspecific, making diagnosis difficult, requiring high degree of suspicion. The authors present the case of a man, 55 years old, immunocompetent, who goes to the doctor due to constitutional symptoms after two months of evolution. Given the family history of colon cancer, colonoscopy is sought, showing an ulcerated lesion at the blind. Histology showed non-caseating granulomas and for AFB was negative. The following study carried out with TC chest, sputum and bronchoscopy, has lead to the diagnosis of pulmonary tuberculosis with achievement of the digestive tract. The purpose of this case history is to emphasize the importance of differential diagnosis with other pathologies with similar clinical symptoms and endoscopic changes, and the role of bronchoscopy in diagnosis of pulmonary tuberculosis in patients with negative smear.

Tuberculosis of the sigmoid colon is a rare disorder. An 80-year-old man visited Bongseng Memorial Hospital for medical examination. A colonoscopy was performed, and a lesion in the sigmoid colon that was suspected to be colon cancer was found. A biopsy was performed, and tuberculous enteritis with chronic granulomatous inflammation was diagnosed. Intestinal tuberculosis is most frequent in the ileocecal area, followed by the ascending colon, transverse colon, duodenum, stomach, and sigmoid colon, in descending order. Hence, we report a case of intestinal tuberculosis in the sigmoid colon, which is rare and almost indistinguishable from colon cancer.

Intestinal tuberculosis is a rare diagnosis, which may often be mistaken for mb. Crohn or cancer. We present a case of a 57-year-old man, who was diagnosed with intestinal tuberculosis. Due to increased abdominal pain, a computed tomography was performed, revealing a growing sigmoidal tumour, and the biopsies taken showed an adenocarcinoma. Further histological tests revealed no spread of cancer, and the patient could receive curative surgery. While intestinal tuberculosis can appear similar to colon cancer, it may also as in this case be an intercurrent disease.
