The cause of adenocarcinoma of the colon has not been proven and a viral association has not been reported with this disease. A sensitive biochemical technique for the detection of viral nucleic acid was used to determine whether one type of herpesvirus, namely a human strain of cytomegalovirus, was uniquely present in tumour tissue. 24 specimens of colon, obtained at surgery from 14 patients, were analysed by membrane CR.N.A.-D.N.A. hybridisation. 4 of 7 tumours of the colon were definitely positive for C.M.V. D.N.A. (2greater than or equal to 2 genome-equivalents/cell), or repeatedly showed more than 1 genome-equivalent/cell. Macroscopically normal colon 5 cm from the tumour, as well as histologically normal and abnormal colon from a control population with
Crohn's disease
were uniformly negative for C.M.V. D.N.A., except for 1 specimen which was macroscopically normal tissue from a patient with
carcinoma of the colon

Among patients with conditions that predispose to adenocarcinoma of the colon, 1 of 2 patients with familial polyposis and 1 of 3 with ulcerative colitis harboured in their colon greater than or equal to 2 genome-equivalents of C.M.V. D.N.A. per cell, and another ulcerative colitis patient had 1-2 genome-equivalents/cell.

resected previously for benign disease has not been published. However an increasing number of patients resected for
Crohn's disease
, diverticulitis or trauma may reach nowadays a high lifespan. On the other hand, it is known that the gastroenteral anastomosis is predisposed to cancer development. In this study, the question of whether the large intestine following colotomy or ileotransversostomy is sensitive to carcinogenesis is examined. Male Wistar rats, subjected to colotomy or resection and ileotransversostomy, were treated weekly by subcutaneous injection of 1,2-dimethylhydrazine (12 mg/kg body weight) for seven weeks. The animals were killed 54 weeks after the first injection. At autopsy, 21 out of 29 operated rats had developed adenocarcinomas of the remaining colon. Intact control animals had the same incidence of malignant degeneration of the large bowel. When the anastomosis is chronically irritated by inflammation or by formation of a diverticulum, development, of carcinoma near the stoma was observed. This was the case in three rats of 28 animals. The results demonstrate that the resected colon of the rat is not more sensitive to experimental carcinogenesis than the intact one.

From 1955 through 1974, 336 patients with ulcerative colitis diagnosed before age 21 years were studied. In 93 patients (29%), a blood relative had ulcerative colitis, one case of
Crohn's disease
being found. The total colon was involved in 63% of patients; the entire colon or all but the rectal stump was removed in 35%. Eighteen patients died, nine of
carcinoma of the colon
. Sixty-five percent of patients had symptoms for longer than 6 months before the diagnosis of ulcerative colitis. If the diagnosis was delayed more than 24 months, there was a statistically significant correlation with increased rate of operations and complications and less good quality of life. When the 20-year study period was divided into two 10-year periods, the operative and complication rates were significantly different. Early diagnosis and treatment appear to improve the long-term prognosis of young patients with ulcerative colitis.

, we feel it to be of interest to stress the possible urinary origin of vesico intestinal fistulae (4 of our cases, and 4.5% of the total number collectedin the literature). From a diagnostic standpoint, we would emphasise that these fistulae present almost solely with urinary symptoms and signs and stress the clinical importance of what is now known, since J. Cibert, as "pre-fistulous cystitis" preceding the development of pathognomic signs--faecaluria and pneumatria. Radiological opacification of the colon and rectum is essential in patients with a syndrome of chronic urinary infection unexplained by a urological cause. From a therapeutic standpoint, the surgical treatment of vesico-intestinal fistulae is associated with a high operative mortality (2 deaths amongst our 14 patients) and which, independently of the aetiological factor (predominance of carcinomas) is explained above all by the septic nature of the lesions.

We have reported long-term results in the cases of 42 patients following total colectomy and ileorectal anastomosis for inflammatory bowel disease. In this group, 35 patients had
Crohn's disease
and seven had ulcerative colitis. Five of those seven patients with ulcerative colitis had
carcinoma of the colon
at the time of colectomy. A diverting loop ileostomy was constructed in 14 of the 35 patients who had Crohn's colitis at the time of operation, and none of these patients had any anastomotic leakage either before or after the ileostomy was closed. However, there patients with Crohn's colitis in whom anastomotic leaks developed postoperatively; all three patients died. In the group with ulcerative colitis, one patient had an anastomotic leak but there was no operative nortality. Of the 29 patients with
Crohn's disease
followed for one to 18 years, 12 (41 per cent) developed recurrences in the ileum and/or rectum, and seven of these patients had to have their anastomoses taken down.

The course was complicated by recurrent erythema nodosum and wandering arthritis as well as sclerosing cholangitis and multifocal adenocarcinoma of the large bowel. (1,8) whereas pericholangitis in liver biopsy specimen was diagnosed in prospective studies in 8 (7) to 30 (2) per cent. The same is true for carcinomas complicating granulomatous enterocolitis. In a recent review 36 cases of small bowel cancer (4) and 20 cases of
colon cancer
(5) were reported. Weedon et al. (9) calculated the risk of developing carcinoma to be 20 times greater in
Crohn's
enterocolitis than in a control population. Using life-table methods the 10-year probability of remaining cancer free was 99.7 per cent, the 20-year probability 97.2 per cent. In our patient the long-lasting history of Crohn's colitis with several minor complications and the multiplicity of colonic carcinomas speak in favor of a causal relationship between the chronic inflammation and the development of malignancy.

anorectal fistulas and involvement of the terminal ileum are frequently found. In cases with ulcerative colitis anorectal fistulas are more rare but in this disease a
carcinoma of the colon
often develops, especially when the course is severe and the involvement of the colon is extended. Clinical symptoms, courses and complications of both diseases are described. In differential diagnoses endoscopic procedures especially coloscopy play an important role. In cases of ulcerative colitis about 90% the rectum is involved. Spontaneous haemorrhage of the colonic mucosa with edema and pseudopolyps are typical endoscopic findings. Most cases with
Crohn's disease
show flat and irregular ulcers within a normal mucosa. In later stage a "cobble stone" relief is demonstrable. Endoscopy and guided biopsy are not only important for differential diagnosis but also for treatment and prognosis.

occurs at a younger age, on the average, and in areas similar to the distribution of

rather than the usual distribution of the cancer. Preoperative diagnosis is difficult, but changes in the nature of chronic fistulas should be investigated.
