We studied the clinical usefulness of nasal continuous positive airway pressure (CPAP) treatment of obstructive sleep apnea syndrome (OSAS). The subject were 56 men with OSAS. Their mean age, apnea index, and SaO2 were 51 +/- 8 years, 48 +/- 18, and SaO2 were 51 +/- 8 years, 48 +/- 18, and 81 +/- 7%, respectively. After titration of CPAP, they underwent nasal CPAP treatment at home every night. Symptoms, blood pressure, pulmonary function, compliance with the prescription and complications of treatment were evaluated every one or two months in an outpatient clinic. Blood pressure decreased significantly from 135 +/- 15 mmHg (systolic) and 88 +/- 14 mmHg (diastolic) to 126 +/- mmHg (systolic) and 78 +/- 6 mmHg (diastolic) in 51 cases after two weeks of treatment. No significant change in pulmonary function except for blood gases was observed after longterm treatment.
Excessive daytime sleepiness
disappeared and sleep quality improved after treatment in most cases. Thirty-six of 43 patients continued to use nasal CPAP for 34 +/- 16 months and 33% complied with the prescription. Although nasal mask

and dryness of the nose and mouth occurred in several cases, there were no serious complications. These findings suggest that nasal CPAP is useful for treatment of patients with OSAS. However, the problem remains that the cost of nasal CPAP is not reimbursed by the public health insurance system in Japan.

Pharmacological therapies are presented for two typical neurological sleep-wake disorders, restless legs syndrome (RLS) and narcolepsy. The individual
discomfort
caused by RLS and the accompanying problems with initiating and maintaining sleep often require a therapy with L-dopa and dopamine agonists. Positive treatment effects on sensory and motor symptoms have been shown in open trials and controlled studies. Development of time shift and/or augmentation of symptoms is a problem of L-dopa therapy. Further efficient drugs are opioids and benzodiazepines. The therapy of narcolepsy depends on its severity and the pattern of the symptoms.
Excessive daytime sleepiness
and sleep attacks are the most impairing symptoms, which are difficult to treat. These symptoms require an optimal combination of CNS stimulants with regular napping. Cataplexy and other REM sleep associated symptoms are effectively treated with REM sleep suppressing antidepressants.

Parkinson's disease (PD) is an alpha-synucleinopathy that leads to prominent motor symptoms including tremor, bradykinesia, and postural instability. Nonmotor symptoms including autonomic, neurocognitive, psychiatric symptoms, and sleep disturbances are also seen frequently in PD. The impact of PD on sleep is related to motor and nonmotor symptoms, in addition to the disruption of the pathways regulating sleep by central nervous system pathology. Rapid eye movement sleep behavior disorder is a parasomnia that can lead to self-injury and/or injury to partners at night. Restless legs syndrome is a subjective sensation of
discomfort
and urge to move the legs prior to falling asleep and can lead to insomnia and reduced sleep quality.
Excessive daytime sleepiness
is common in PD and exerts a negative impact on quality of life in addition to increasing the risk of falls. Obstructive sleep apnea is a breathing disorder during sleep that can cause frequent awakenings and excessive daytime sleepiness. Circadian rhythm dysfunction can lead to an advanced or delayed onset of sleep in patients and create disruption of normal sleep and wake times. All of these disorders are common in PD and can significantly reduce sleep quantity, sleep quality, or quality of life for patients and caretakers. Treatment approaches for each of these disorders are distinct and should be individualized to the patient. We review the literature regarding these common sleep issues encountered in PD and their treatment options.
