Multiple sites of atherosclerotic occlusion in high risk patients may be treated by angioplasty of the iliac obstruction and distal reconstruction. We report 18 male patients with symptomatic
peripheral vascular disease
in whom proximal iliac percutaneous transluminal angioplasty was combined with femoropopliteal bypass (11), femorotibial bypass (2), or femorofemoral bypass (5). There were no operative deaths. The pretreatment ankle brachial index of 0.40 +/- 0.04 was increased to 0.64 +/- 0.04 by discharge (p = 0.0001), and remained significantly increased through 27 months (0.65 +/- 0.07) (p = 0.0001). During the follow-up period of 2-57 (mean 27 months) one dilated iliac artery required repeated percutaneous transluminal angioplasty and revision of the femoropopliteal bypass at three months. Two late amputations of study limbs occurred at two years and three years due to progression of distal disease in the infrapopliteal segment. (1). Life table analysis shows a 76% success rate for the combined procedures at two years. In selected, high risk patients, proximal iliac dilatation and distal bypass is an acceptable alternative reconstruction for multilevel occlusion.

Lung cancer, a disease of epidemic proportions, is to some extent preventable. Elimination of smoking would certainly reduce its incidence. Conventional therapy has not produced any major advance in this field, the 5-year survival of all patients with lung cancer still being very low. The disease is best managed when detected at an early stage, but on the basis of a study carried out at the Mayo Clinic, screening of the general population cannot be advocated. A few high-risk groups of patients have been identified, such as those treated surgically for laryngeal or pharyngeal tumours and those previously operated on for
lung carcinoma

, lung cancer was found in 3.25%. The distinction is made between those whose lung cancer was detected at the time of vascular surgery (synchronous) and those whose cancer was found later (metachronous). Every patient scheduled to undergo surgery for

should be screened by chest roentgenography and cytologic examination of the sputum. These patients should be followed up closely in the postoperative period.

Arsenic has been well documented as the major risk factor for blackfoot disease (BFD), a unique
peripheral vascular disease
(
PVD
) that was endemic in the Southwestern coast of Taiwan, where residents had used high-arsenic artesian well water for more than 50 years. Long-term arsenic exposure has also been reported to associated with lung cancer mortality in a dose-response relationship. A tap-water supply system was implemented in the early 1960s in the BFD-endemic areas. Artesian well water was no longer used for drinking and cooking after the mid-1970s. The objective of this study is to examine whether lung cancer mortality decreased after the improvement of drinking water supply system through elimination of arsenic exposure from artesian well water. Standardized mortality ratios (SMRs) for lung cancer were calculated for the BFD-endemic area for the years 1971-2000. The study results show that mortality from lung cancer declined gradually after the improvement of drinking water supply system to eliminate arsenic exposure from artesian well water. Based on the reversibility criterion, the association between arsenic exposure and lung cancer mortality is likely to be causal.
