The level of serum alpha-fetoprotein (AFP) was estimated by radioimmunoassay in 153 normal healthy Malysians of different ethnic groups. The mean level was 7.5 In1/ml (SD 2.28InU/ml). Among 330 patients with malignant tumors, 11 had increased levels of AFP. The only patient who had hepatoma had a very high level of serum AFP. High levels were also found in three of four patients with dysgerminoma of the ovary, in the only two patients with carcinoma of the testis, and in one patient with secondary carcinoma of the humerus of unknown origin. Lower, but significantly increased levels were observed in one patient (of 48) with breast carcinoma, one patient (of 8) with
basal cell carcinoma
of the nose, one patient (0f 27) with
carcinoma of the lung

Among 172 patients with hairy cell leukemia (HCL) seen at the University of Chicago over a 10-year period, 15 were found to have a second malignancy. Neoplasia of the skin was noted most frequently; there were three cases of
basal cell carcinoma
, one case of anaplastic squamous cell carcinoma, and one case of malignant melanoma. This was followed in frequency by three cases of
carcinoma of the lung
. The clinical characteristics of these 15 patients did not differ from those of the general population of patients with HCL. A variety of second hematologic malignant disorders and solid tumors were identified. In one case, the second neoplasm occurred before the diagnosis of HCL; six were diagnosed concurrently; and eight followed the diagnosis of HCL. Since HCL is a well-defined clinicopathologic entity, patients with HCL who exhibit unusual features of the disease should be investigated further for the presence of second malignancies.

Photofrin is a light-activated compound used for photodynamic therapy (PDT) of malignant tumors. Although PDT with this drug has been approved for clinical use in the United States, Canada, Japan, and the Netherlands there are few published reports on the biodistribution of Photofrin in humans. In this study we report measurable amounts of Photofrin in human serum up to approximately 1 year following injection of two different Photofrin doses. Concentration-time data were collected from 3, 12, 19, and 10 patients after 0.75, 0.875, 1, and 2 mg Photofrin/kg body weight. basal cell carcinoma
; 1 mg Photofrin/kg is now a standard dose for PDT of cutaneous malignancies at this institute. For the 1 mg Photofrin/kg dose, a triexponential 3-compartment pharmacokinetic model was fitted to 30 data points pooled from the 19 patients, as if we had one "superpatient." The alpha, beta, and gamma halflives were approximately 16 h, 7.53 days, and 155.56 days, respectively. The mean (+/- SEM) serum concentrations 48 after injection (when most tumors are exposed to drug-activating light) of 0.875, 1, or 2 mg Photofrin/kg were 2.70 +/- 0.47, 4.00 +/- 0.66, and 3.47 +/- 0.97 micrograms Photofrin/ml, respectively. No porphyrin fluorescence could be detected in serum collected from patients 560 to 1335 days after Photofrin injection.

Endobronchial metastases (EBM) secondaries to extrapulmonary solid malignant tumors are rare. Breast, colon and renal adenocarcinomas are the most frequent tumors associated with EBM. Since 1990 we have treated eight patients with EBM secondary to renal adenocarcinoma (three cases), colon adenocarcinoma (two cases), gastric adenocarcinoma (one case), bladder carcinoma (one case) and
basal cell carcinoma
(one case). Endobronchial lesions were detected by bronchoscopy and their metastatic nature was confirmed histopathologically in all eight cases. We also conducted a review of EBM reporting studies published in English language. The median interval from the diagnosis of the primary tumour was 41 months. Symptoms and radiological findings were indistinguishable from those of primary lung cancer. Five patients were treated with external radiotherapy with symptomatic improvement while two patients had chemotherapy and one patient underwent surgical resection of the metastasis. Systemic treatment was used in six cases with no significant effect on EBM. Median survival after EBM diagnosis was 9 months with one patient surviving 3.5 years and two patients still alive at 1 year. In conclusion, EBM usually represent a late manifestation requiring differential diagnosis from a primary lung cancer. Local treatment may result in symptomatic improvement but prognosis is generally poor averaging 1-2 years in most series.

Basaloid carcinoma of the lung is a rarely occurring form of lung cancer morphologically resembling the
basal cell carcinoma of the skin
. Although various histologic types of lung cancers have metastasized to the skin, basaloid
carcinoma of the lung
that shows cutaneous metastasis has yet to be reported. A 48-year-old man was admitted to our pulmonology department with mild dyspnea and a solitary cutaneous mass on the left portion of his chin. Chest radiography and computed tomography of the thorax revealed an irregular infiltrative mass confined to the central portion of the left lower lobe bronchus, which is a compatible finding of primary lung cancer. Microscopically, both a cutaneous mass and fine-needle aspirate material in the lung revealed the identical findings of basaloid morphologic patterns.

basal cell carcinoma
in a patient with undiagnosed lung cancer. Immunohistochemistry was invaluable in confirming both the metastatic nature of the secondary skin lesion and its site of origin.

Inhibitors of the hedgehog pathway are effective in patients with
basal cell carcinoma
and a subgroup of patients with medulloblastoma with active hedgehog signaling. Despite preclinical work suggesting otherwise, clinical trials in solid tumors of epithelial origin have not shown added benefit with these drugs. Here, we review the preclinical and clinical data of hedgehog pathway inhibition in the most common histologic types of lung cancer. We focus on highlighting areas of uncertainty, where further research might define a niche for hedgehog pathway inhibition in patients with lung cancer.
