A 63-year-old female, with a chief complaint of right chest pain was referred to our hospital because of an abnormal right chest wall shadow on chest X-ray. A rib tumor was suspected based on her chest CT scan. Percutaneous needle biopsy yielded a diagnosis of well-differentiated adenocarcinoma. As metastatic rib tumor was suspected, the
primary tumor
was sought for. A thyroid gland nodule was recognized by ultrasonography, and the cytological diagnosis was
class V
. Immunohistological demonstration of the thyroglobulin studies of the rib tumor tissue revealed papillary and trabecular patterns of cell arrangement and the presence of thyroglobulin existence. Definitive diagnosis of metastatic rib tumor from thyroid cancer was established. Immunohistological examination is useful to confirm the diagnosis of metastatic cancer of thyroid origin.

We treated a case of unresectable gastric cancer in which peritoneal lavage cytology and the
primary tumor
responded to combined chemotherapy by intravenous, intraarterial and intraperitoneal injection. The patient was a 61-year-old male with loss of appetite. He underwent laparotomy for gastric cancer in November, 1997. An intraperitoneal catheter was set without gastrectomy because of the unresectability due to extensive peritoneal dissemination and local invasion to pancreas. The patient was given combined chemotherapy using 5-FU 6,000 mg i.v., CDDP 360 mg i.p. and 5-FU 10,500 mg ia by the catheters in the supra vena cava, right gastroepiploic artery and peritoneal cavity. One month after laparotomy, peritoneal lavage cytology had changed to class I from
class V
, and the
primary tumor
had been reduced to 10 mm in size from a diffusely infiltrating tumor. His quality of life in appetite and activity was improved for 13 months.

Our patient is a 28 year old female with end-stage renal disease (ESRD) secondary to membranous lupus nephritis who received a living related transplant from her mother. At 26 months post-transplant, she presented with proteinuria and a rise in creatinine (Cr). Allograft biopsy was consistent with recurrent membranous nephropathy. Five weeks later, she was found to have a high-grade leiomyosarcoma originating within the allograft. We reviewed the literature on recurrent post-transplant membranous nephropathy and the possible role of the Epstein-Barr virus (EBV) infection in smooth muscle tumors occurring in organ transplant recipients. We also considered the association of membranous nephropathy and malignancy.

A 46-year-old woman was referred to our hospital because of nausea. Endoscopy revealed scirrhous gastric cancer, and abdominalcomputed tomography revealed peritonealdissemination.
