A 9-year-old Thoroughbred mare was examined because of
pollakiuria
, hematuria, and weight loss of 3 weeks' duration. Physical examination revealed a regular cardiac rhythm with occasional premature contractions, and a soft tissue mass in the pelvic canal palpable per rectum. Microscopic examination of urine sediment revealed numerous RBC and a large population of lymphocytes and lymphoblasts with characteristics of neoplasia. Similar cells were found in peritoneal fluid obtained by abdominocentesis. The horse was euthanatized without treatment. Necropsy revealed a soft tissue mass infiltrating the bladder,
vagina
, and uterus. Additional masses were found in the sublumbar muscles and myocardium. The histologic diagnosis was lymphosarcoma.

The cause of the female urethral syndrome has previously been obscure, as it has been associated by definition with a lack of objective findings but a plethora of subjective complaints of retropubic pressure, dyspareunia,

, and dysuria. There is now strong evidence that the microscopic paraurethral glands connected to the distal third of the urethra in the prevaginal space are homologous to the prostate. They stain histologically for prostate-specific antigen and, like the prostate, are subject to both infection and cancer. The most important aspect of recognizing this microscopic "female prostate" as an anatomic feature is that its infections may completely explain many cases of the urethral syndrome. Further, the diagnosis is not elusive if trained clinicians palpate for localized and objective paraurethral tenderness through the anterior
vagina
wall to one or both sides of the urethra. Treatment parallel to that for male prostatitis is usually rewarded by the elimination of symptoms and the objective finding of the loss of tenderness of the paraurethral glands. As with prostatitis, the localized problem often recurs. It is time to alert primary care physicians to this disorder and to eliminate the widespread practice of treating affected women with either invasive urethral dilation or tranquilizers.

Introduction. Chronic inflammation of the urinary bladder (CIB) is a disease entity which mostly concerns young women and which is connected with their short urether and vicinity of

sexual activity. Frequent recidication, in spite of use of new generation of antibiotics - often of considerable cost - have a significant influence upon life of the patients. Side-effects connected with frequent use of antibiotics are also an important factor to consider.<br /> Aim. The life discomfort of the patients, frequently low effectiveness and side-effects of antibiotherapy, numerous cases of recidivation were the reasons of application of medical ozone in chronic inflammation of the urinary bladder.<br /> Material and method. During 18 months oxygen-ozone mixture was applied directly to the blader in case of 72 patients, to 65 women, that is 90% of patients, and to 7 men, that is 10% of patients. The age group was 19 to 67, with the average age of 36. In case of men, the age group was from 38 to 51, with the average age of 44. The number of procedures, depending both on bacteriological results and subjective symptoms, veried from 10 to 18. Ecery patient, prior to the ozonotherapy underwent general urine analysis and bacteriological tests; also subjective symptoms as pollysis and bacteriological tests; also subjective symptoms as

, pain and burning during miction, and pressing tenesmus were estimated, according to QL life comfort scale.<br /> Those tests were repeated three and six months after the ozonotherapy procedures.<br /> Results. In the women group (in all cases) statistically significant improvement of life comfort was noted. Results of bacteriological tests after one month and three months were germ-free, after six months bacteriological results of 4 patients, that is 6% showed initial results with relapse of subjective symptoms. Which are statistically significant improvement in the case of remaining 47 patients, that is 72% germ-free culture were gained and lack of subjective symptoms. In the 6 patients (that is 86%) men group, after six months, in all cases germ-free-culture and lack of subjective symptoms were gained.<br /> Conclusion. Application of ozonotherapy in treatment of chronic inflammation of the vladder is a of significant statistical effectiveness method, and what is more cheap and with low risk in performance.

A 55-year-old woman had
urinary frequency
and a constant urge to urinate. Computed tomography confirmed a urethral tumor, and transurethral biopsy confirmed adenocarcinoma. She visited our hospital to undergo treatment, and we performed an anterior pelvic excenteration. On histology, the tumor had spread to the bladder, urethra, and
vagina
. However, the majority of the tumor was located in the bladder and urethra, a duct with intestinal metaplasia was present around the urethra, and carcinoma in situ was seen in the urethral mucosa. Based on the above findings, the patient was diagnosed as having primary urethral adenocarcinoma. No tumor cells were seen in the resection stump. Six months after surgery, the patient developed bone metastasis, followed by peritoneal and pleural dissemination, as well as multiple lung metastases. The patient died nine months after surgery. In the present patient, the carbohydrate antigen (CA) 19-9 level changed with the clinical course, and it was a useful marker. Urethral tumor is relatively rare. A urethral tumor accompanied by vaginal wall infiltration is likely to be mistaken for a primary vaginal tumor. It was very difficult to identify the primary organ in our case. To the best of our knowledge, the present patient is the sixth reported case of primary urethral carcinoma accompanied by vaginal wall infiltration in Japan. The six reported cases are compared and analyzed.

A randomized double blind controlled trial of intravaginal cream ovestin (estriol) was conducted in 102 postmenopausal females with chronic cystitis. The patients were divided into two groups. Group 1 (n = 52) received standard therapy, group 2 (n = 50) received standard therapy plus ovestin (daily intravaginal administration for 2 weeks, then twice a week). Final results were evaluated 1 year after therapy. Before the treatment 94% patients had subnormal levels of blood estradiol. One year after the treatment positive results were achieved in 91.3% patients of group 2 and 65.8% of group 1 (p < 0.001). In group 2 patients ovestin significantly reduced

, nocturia, episodes of urgent urinary incontinence, painful voiding. Percentage of patients with leukocyturia diminished from 100 to 8%, bacteriuria--from 74 to 4%, with vaginal lactobacilli increased from 0 to 56%, enterobacterial contamination of the

fell from 66 to 12%. Vaginal pH decreased from 6.0 to 3.6. Addition of ovestin to standard therapy reduced the number of cystitis recurrences 11-fold, days of antibacterial therapy--12.4-fold. Side effects of ovestin (vaginal pruritus) were seen in 4% patients.

CASE DESCRIPTION-4 dogs were evaluated because of recurrent urinary tract infections. CLINICAL FINDINGS-All dogs had recurrent urinary tract infections and similar clinical signs; 3 dogs had urinary incontinence. Digital vaginal examination revealed vestibulovaginal stenosis in all dogs, which was confirmed by results of contrast vaginourethrography. From image measurements, the vestibulovaginal ratio (ratio of the height of the vestibulovaginal junction to the maximum height of the
vagina
on a lateral vaginourethrogram) was calculated for each dog. Three dogs had severe stenosis (vestibulovaginal ratio, < 0.20; severe stenosis is defined as a vestibulovaginal ratio < 0.20), whereas the fourth dog had moderate stenosis (vestibulovaginal ratio, 0.24; ratio range for moderate stenosis is 0.20 to 0.25). TREATMENT AND OUTCOME-All dogs were anesthetized for surgical correction of the vestibulovaginal stenosis. Vaginal resection and anastomosis of the stenosis was performed in all 4 dogs, with 1 dog also undergoing episioplasty. Complete resolution of clinical signs was apparent in 3 dogs; 1 dog had postoperative complications including
pollakiuria
and stranguria, which resulted in rectal and vaginal prolapse. This dog underwent ovariohysterectomy, after which clinical signs resolved. All dogs had resolution of urinary tract infections at the time of follow-up (6 to 8 months after surgery). CLINICAL RELEVANCE-Resection and anastomosis may resolve recurrent urinary tract infections in dogs with severe or moderate vestibulovaginal stenosis. Episiotomy was not necessary for success of surgical treatment, and overall, that procedure increased morbidity, the severity of intraoperative hemorrhage, and duration of surgery.
