Leiomyosarcomas
are rare malignant tumors of the kidney. They may arise from the renal capsule, renal vein, renal pelvic musculature or renal parenchyma. Renal pelvis is an uncommon site of occurrence, with around 10 cases reported in the literature so far. Here we present a 60-year-old male who presented with increased
urinary frequency
, lower limb weakness, anorexia and weight loss. Imaging showed a right renal mass. A renal cell carcinoma was suspected clinically. A right nephrectomy was performed, which showed a large circumscribed mass in the hilar region. Histology revealed a tumor mass arising from the renal pelvis. The tumor was composed of spindle cells arranged in fascicles. Immunohistochemistry showed tumor cells to be positive for smooth muscle actin (SMA) and desmin (Des) and negative for cytokeratin (CK), HMB 45, CD117 (C-kit), and CD34. That confirmed the diagnosis of
leiomyosarcoma
.

The objective of this case report is to present a rare bladder tumour in a young patient 25 years after the treatment with cyclophosphamide because of a neuroblastoma of the right eye. The first symptom of the tumour was macroscopic haematuria with dysuria and
pollakiuria
. The final diagnosis was dictaminated by the pathologist and the best treatment option was radical cystoprostatectomy.
Leiomyosarcoma
was presented in a 26-year-old patient like the third different tumour, the second was an esphenoidal meningioma. At the 8th year of follow-up after the radical surgery, the patient is free of bladder disease. Bladder
leiomyosarcoma
is a rare tumour of the bladder and its early diagnosis and treatment are mandatory for best prognosis.

Bladder
leiomyosarcoma
is a rare malignancy, it's nonurothelial tumors and account less than 5% of all bladder malignancies. Because of the very low incidence less than 200 cases reported in the literature, etiology is still little know, also there is lack of consensus regarding definitive treatment for this rare tumors. The most common clinical presentation is gross hematuria, which is reported in 81% of cases, followed by
pollakiuria
(28%) and dysuria (19%), respectively. Herein, we present the case of a patient diagnosed with a bladder
leiomyosarcoma
and treated with laparoscopic radical cystoprostatectomy and totally intracorporal orthotopic ileal diversion.
