A series of 92
thyroid cancer
patients treated in collaboration between the Nuclear Medicine and the Oncological Radiotherapy Departments of the Turin Mauriziano Hospital is reported. The age, sex, histotype and stage of the various groups of patients subdivided by therapeutic indications were analysed. In 17 patients, treatment was carried out for pain killing purposes on particularly painful secondary bone localisations with good results symptomatologically and from the point of view of
functional recovery
. In 21 patients with unfavourable histology who did not take up radioiodine, external radiation treatment was carried out for palliative reasons with, in some case, a lasting effect. In the other 44 patients although generally speaking, at advanced stages of the disease and often in the presence of invasive and anaplastic secondary manifestations, total survival with the two treatments combined was about 55% (24 cases) of whom 75% (18 cases) are alive 5 years later.

Vocal cord paralysis occurs with and without infiltration of the recurrent laryngeal nerve. Patients with no paralysis may have recurrent laryngeal nerve infiltration. We studied 262 patients with invasive thyroid carcinoma and an additional 480 patients that we operated on, and we offer an approach to the management of the recurrent laryngeal nerve during surgery for suspected or proven
thyroid cancer
. Because complete excision of papillary carcinoma with resection of the recurrent laryngeal nerve did not improve survival over incomplete excision, we recommend incomplete excision of papillary carcinoma infiltrating a functioning recurrent laryngeal nerve with postoperative I-131 and thyroid-stimulating hormone suppression. We observed (1) lymphoma and Reidel's thyroiditis causing paralysis with infiltration of the recurrent laryngeal nerve with
recovery of function
, (2) benign and malignant nodules causing paralysis without infiltration of the recurrent laryngeal nerve with
recovery of function
, and (3) infiltration of the recurrent laryngeal nerve without paralysis in lymphoma, Graves' disease, and thyroiditis. In surgery for a suspicious nodule with paralysis, identify the recurrent laryngeal nerve, resect it if it is infiltrated by carcinoma, and preserve it if not infiltrated or if infiltrated by any pathology besides carcinoma.

In
thyroid cancer
surgery since 1978 we have made parathyroidal autotransplantation which has been to resect the entire parathyroid, cut fine to mud and autotransplant into greater pectoral muscle. To evaluate our procedure and consider the mechanism of hypocalcemia and tetanus following surgery, we examined the intact-PTH recovered to about 80% of its preoperative value on 14-postoperative-days and thereafter remained almost constant. It remained below the sensitivity of measurement from immediately to 3-postoperative-days, and with our procedure, parathyroid was considered to be totally resected. Comparison between 2 (6 cases) and more autotransplanted glands (11 cases) revealed that the former showed slightly later
functional recovery
, but recovered to the almost some extent as the other 21-postoperative-days. Comparison between the group requiring calcium supplement therapy (12 cases) and the otherwise (5 cases) revealed almost the same course of recovery. Thus, our procedure seems to enable us to make satisfactory functional preservation and expect
functional recovery
by 2-gland autotransplantation at least. The supplement therapy was considered not to be detrimental to the take of autotransplanted glands and
functional recovery
, and no correlation was noted between the onset of tetanus and intact-PTH.

Autotransplantation of the parathyroid glands by mincing method has been performed to maintain parathyroidal function in patients with
thyroid cancer
, following total thyroidectomy and parathyroidectomy. The
functional recovery
of grafted parathyroid glands was studied in matured mongrel dogs histologically and by determining the PTH level and serum P, Ca, and compared three different forms of implant; whole gland, minced parathyroid tissue, and thick slices. The minced parathyroid tissue and thick slices were found to be far superior histologically, and endocrinologically, to the whole gland type of implant, with successful takes. And between two types of implants minced tissue and thick slices, no difference was observed histologically and endocrinologically. On the other hand, it is necessary for shortening the time to prepare implant tissue to get successful reimplantation. The time to prepare minced tissue is very short in comparison with thick slices. Thus our method (minced tissue) is very useful for successful reimplantation of the parathyroids.

Development of vocal fold paralysis in the presence of thyroid disease is strongly indicative of
thyroid cancer
, and requires surgical exploration. At the same time, vocal fold paralysis does not relieve the surgeon of his obligation to identify and preserve the recurrent laryngeal nerves, since the cause of the paralysis may be a benign disease, with a fair chance of
functional recovery
after surgery. We hereby report a case of recurrent laryngeal nerve palsy secondary to a multinodular goitre.
