are reported in a 29-year-old female with recurrent carcinoma of the uterine cervix. Following a Wertheim-Meigs procedure and resection of local tumor recurrence eight months later, a second recurrence was detected on CT as a purely cystic lesion in the left pelvic wall. Although the size of the cyst subsequently increased, the patient remained asymptomatic until slight
pain
developed in the left leg 28 months after initial therapy. One month later, limping was noticed; plain radiography showed a large osteolytic lesion of the left hip and on I.V.U., a dilatation of the left renal collecting system was observed. Pelvic CT showed only minimal solid tissue components within a large cyst. Multiple needle aspirations of the cyst and cytological examinations of the fluid obtained showed no evidence of tumor cells. A definitive histological diagnosis of metastasizing squamous cell carcinoma was only reached after surgery.

Cecal carcinoma has been associated with a poorer prognosis than other colon carcinomas because of the presumed longstanding obscure symptoms. In a combined study of three Dutch hospitals, a total of 166 patients with cecal carcinoma were evaluated after right hemicolectomy. Special emphasis was placed on clinical symptoms related to advanced

, anemia, and palpable mass. These factors and clinicopathological staging were evaluated with aid of the Cox regression model. Ninety percent of the resected specimens contained a Dukes' B or C carcinoma. Only 5% were found to have widespread metastatic disease. Overall 5 year survival rate was found to be 0.57. No statistically significant relation to

or palpable mass was found. Anemia, however, was related to a better survival, especially in patients with a Dukes' B carcinoma. Clinicopathological staging according to Dukes' is closely related to survival. It is concluded that carcinoma of the cecum behaves similarly to other colon malignancies.

When response rates to antineoplastic therapy are good and the associated nutritional morbidity is high, TPN is recommended when the gastrointestinal tract is unavailable for use. Patients should not be denied adequate cancer treatment because of the fear of complications secondary to malnutrition. On the other hand, prolongation of
pain
and suffering for the terminally ill cancer patient is obviously not an indication for TPN. Appropriate clinical judgment is required in proper selection of the TPN candidate. Malnutrition is almost always a prerequisite, as is failure of the gastrointestinal tract. Therapies designed to direct nutritional support to preferentially benefit the host and "starve" the tumor have had only limited short-term success in animal models. The use of diets that are glutamine- and arginine-supplemented are only now being examined in animal experiments. Since TPN allows total and complete control of exogenous nutrients, it should be used to explore alternative therapies in the form of nutrient composition. For example, if tumors are preferential glutamine utilizers, then TPN-enriched solutions might be employed to increase the number of cells in a phase of mitosis that makes them more susceptible to attack by cell-cycle specific chemotherapeutic agents. Cystosarcoma phylloides is a breast neoplasm that has a frequently unpredictable clinical course. We made a retrospective study of 25 patients with this disease in an attempt to evaluate the indicators of aggressive behavior. In our series, older patient age, nulliparity, rapid

, and large size of tumors increased the suspicion of malignancy but were not always reliable indicators of malignancy. Skin ulceration, tumor necrosis, and infiltrating tumor margins were the most ominous characteristics. High-grade tumors, that is, those with increased cellularity, vascularity, mitotic figure, and pleomorphism, often indicated aggressive behavior. Mixed mesenchymal components were sometimes related to a malignant course. We found a 24 percent incidence of associated breast cancer. Carcinoma of the ipsilateral breast was found in four patients and later in the contralateral breast in two patients. Of our 25 patients, 10 (40 percent) had recurrence and 4 (16 percent) died from disease. Recurrences after treatment usually occurred within 3 years. Patients must be followed carefully for local recurrence or metastases, since the clinical course is not predictable. Forty percent of the lesions were diagnosed as being malignant. Local excision was associated with recurrence in six of eight patients and was clearly inadequate treatment. Quadrantectomy was effective for benign peripheral lesions when a generous margin could be obtained. From these data, we believe that mastectomy is indicated in all patients with malignant lesions and in those with large benign lesions.
