A 46-year-old woman with a history of breast carcinoma and no known metastatic disease presented with
iridocyclitis
and secondary glaucoma. Intraocular inflammation and pressure elevation persisted despite standard medical therapy, and paracentesis was performed. Cytological examination of the aspirate revealed adenocarcinoma. Subsequent studies disclosed no evidence of extraocular metastasis. Two courses of radiation therapy to the involved eye resulted in a dramatic reduction in intraocular inflammation and allowed temporary control of the
intraocular pressure
. Ultimately, however, progressive glaucoma necessitated enucleation. This case confirms previous statements that
iridocyclitis
may be the initial clinical manifestation of metastatic malignancy. In addition, this report emphasises the importance of paracentesis in the diagnostic evaluation of selected cases of anterior uveitis of unknown aetiology.

We studied 105 patients (105 eyes) operated on for glaucoma with adjunctive fluorouracil, who were followed up for 12 to 48 months (mean, 34.6 +/- 12.9 months). Fifty eyes that had previous glaucoma or cataract extraction or had inflammatory glaucoma had a high risk for postoperative scarring (high-risk group), and 55 eyes had glaucoma surgery as an initial procedure (initial surgery group). One patient is seventy-one years old, diagnosed four years ago with latero-cervical ganglionic tuberculosis, presents approximately one year and half, progressive loss of the vision in both eyes, predominant in the left-eye; for several months periocular pains referred to the left hemicranial field are presented. Ocular exam: visual acuity was 1/100 in the right eye and perception movements of the hand in the left eye:

was 28 mmHg in the right eye and 8 mmHg in the left eye: perikeratic congestion, chemosis, pigmented precipitates on the posterior face of the corneo, iris in "umbrella", seclusion and occlusion of the pupillary at the left eye, posterior synechiae at the right eye were noted. The eye ground exam is not possible due the pupillary partial seclusion at the right eye and the pupillary seclusion-occlusion at the left eye. The general exam releaves several latero-cervical keloid scars and pronounced dysphonia. Laboratory exam: IDR of the tuberculin was high positive, chest radiography shows hilar bilateral calcifications, ORL exam shows a tumor at the foot of the epiglottis and anatomopathological exam shows low differentiated epidermoid carcinoma.
