Correlations between defective cell-mediated immunity (CMI) and infections following surgery for
esophageal cancer
were evaluated. Peripheral lymphocytes, T cells, B cells, PHA transformation, and
PPD
skin test were measured in 81 patients with
esophageal cancer
, 58 with gastric cancer, and 50 healthy controls. The depression of CMI was predominant to a similar extent in patients with
esophageal cancer
and in those with gastric cancer. The average level of PHA transformation immediately before surgery was significantly lower in the
esophageal cancer
patients with fatal septic complications than in those without such problems. Although preoperative radiation therapy markedly depressed the levels of the four parameters, this association was also noted in 28 patients not given radiation. It thus appears that PHA transformation may be valuable in the prediction of fatal septic complications after major surgery in patients with
esophageal cancer

We studied on 66 patients with
esophageal cancer
with preoperative enteral hyperalimentation by elemental diet, comparing with 64 patients without it, and the following results were obtained; Items such as TP, Alb, etc. in surviving patients, as well as those who died within 3 months, were worse immediately before operation than those at the time of admission when neither TPN nor ED was yet in use. In 35 of 66 patients, there were significant differences between the patients with or without postoperative complications, and who were died after surgery, in arm circumference (AC), triceps skinfold (TSF), arm muscle circumference (AMC), albumin (Alb), prealbumin (PA), retinol-binding protein (RBP) and
PPD
skin test. From the studies of about 60 items with the computer, the index as follow were obtained. Nutritional Assessment Index (NAI) = 2.64 AC + 0.6 PA + 3.76 RBP + 0.017

- 53.8 Nutritional status of the patients was divided retrospectively broadly to three groups, good (NAI greater than or equal to 60), intermediate (60 greater than NAI greater than or equal to 40), and poor (40 greater than NAI) in preoperative period. The incidence of postoperative complications and mortality rates were reflected significantly in NAI. NAI would be useful to know prospectively the probability of all kinds of postoperative complications as well as estimating the nutritional assessment.

The mixed lymphocyte tumor cell culture reaction (MLTR) using autochthonous peripheral and lymph node lymphocytes was performed on 21 patients with
esophageal cancer
. Ten of 16 cases (62%) in peripheral lymphocytes and 9 of 16 cases (56%) in the lymph node lymphocytes showed positive reaction. There was no correlation between stimulation index (SI) and the degree of lymph node metastasis which is one of the determinants of clinical stage (Guide Lines for the Clinical and Pathologic Studies on Carcinoma of the Esophagus, 1976). Many cases showed positive reaction in MLTR even if their tumors were at advanced stage. SI of MLTR on the patients who died of a relapse in a short time within one year after operation was found to be low levels. There was no remarkable correlation between MLTR and
PPD
skin reaction. The results of MLTR were compared with histologic differentiation grades of tumor tissues; intraepithelial spread, vascular invasion, sinus histiocytosis of the regional lymph nodes, and follicular hyperplasia of the regional lymph nodes. There was a minor correlation between the results of MLTR and sinus histiocytosis of the regional lymph nodes, and follicular hyperplasia of the regional lymph nodes. There was a minor correlation between the results of MLTR and sinus histioction.
