From January 1981 to December 1987, 346 children with urinary tract infections, proved by urine culture, were admitted to the Department of Pediatrics, Mackay Memorial Hospital. The ratio of male to female was 3.0 in children below 2 years, and 0.8 in children above 2 years, of age. The urine specimens were collected from suprapubic punctures in 281 cases (81.2%). Fever was the most common clinical manifestation. In children below two years old, other common symptoms and signs were body weight loss or poor gain, feeding problems, diarrhea, irritability, jaundice, and abdominal distension. In older children, urinary frequency, dysuria, enuresis, loin and abdominal pain were frequently found. Hematuria and edema were occasionally noted in all age groups. Microscopic examination of 329 centrifuged urine specimens revealed: 256 cases (77.8%) had more than 5 leukocytes per high power field, 233 cases (70.8%) had more than 10 leukocytes. Three hundred and seventy positive urine cultures were obtained from these patients. E. coli was isolated in 273 cases (73.6%), followed by Klebsiella spp., 34 cases (9.2%); Proteus spp., 27 cases (7.3%); Enterococcus, 21 cases (5.7%); Enterobacter spp., 9 cases (2.4%); Pseudomonas aeruginosa, 8 cases (2.2%); Citrobacter spp., 7 cases (1.9%); Morganella morganii, 6 cases (1.6%); Acinetobacter spp., 6 cases (1.6%); etc. Candida albicans was isolated from three patients. A survey of 29 cases of vesico-ureteric reflux treated surgically at the Department of Paediatric Surgery, Singapore General Hospital, between 1974 and 1981 was carried out to assess the Singapore experience with the disease. There was a 2:1 female predominance but proportionately more males presented before 6 months of age (sex ratio 1:1). In contrast to Caucasian children, the peak incidence is below one year of age, and demonstrates the importance of recognising the condition early in infancy. Correspondingly, the most frequently presenting symptom is fever, as a result of urinary tract infection. In contrast, enuresis is uncommon (unlike in Caucasian children, where the peak incidence is at a later age). There is no predilection to unilateral, bilateral or side of involvement. Ectopia lateralis and patulous ureteric orifices were the most commonly identified structural defects. E. Coli and Klebsiella were the most commonly occurring organisms among those with urinary tract infections, and Ampicillin or Bactrim was adequate for most cases.

The incidence of urinary tract infection (UTI) varies depending on the age of the child. In 1984 a review of 1000 school children in the city of Santo Domingo found an incidence of 0.4%. In a prospective study 109 patients were investigated who presented at the outpatient consulting ward of nephrology of the Robert Reid Cabral Hospital, Santo Domingo, Dominican Republic, with fever and/or clinical symptoms suggestive of UTI. Uroculture was taken by means of suprapubic punction in all those cases which had not received antimicrobial therapy at least 72 hours before taking the specimens. The signs of UTI in the 109 patients were dysuria, urinary incontinence, and enuresis. 41 patients had urocultures positive for UTI bacteria, while the remaining 68 children's urocultures did not show bacterial growth, therefore they were judged not to have UTI. In the 41 patients with UTI the etiologic agents were Escherichia coli in 20 (48.78%), Enterobacter in 14 (34.15%), Klebsiella in 4 (9.76%), Staphylococcus in 4 (9.76%), Pseudomonas in 4 (9.76%), and Salmonella in 3 (7.32%). In all cases of positive uroculture more than 10 leukocytes were observed in the urinary sediment. Only 15 of the 41 patients with positive uroculture had leukocytosis. Fever as a clinical symptom was found in 7 of the 109 cases, and in 3 of these bacteria were grown in the uroculture. In 40 patients out of 109 the fever was associated with clinical signs of UTI, finding positive uroculture in 20 of these patients.
