A case of PCO syndrome associated with carcinoma of the endometrium was described at woman of 40 years old. Endometrial carcinoma was superficial and well differentiated.

Serum Epidermal Growth Factor (EGF) concentrations were estimated in 40 women with endometrial carcinoma and in 10 women with proliferative endometrium. Medium EGF level in study group was 0.97 +/- 0.41 ng/ml whereas 0.78 +/- 0.19 ng/ml in control group. The highest values of EGF (1.39 +/- 0.44 ng/ml) were found in moderately differentiated adenocarcinomas (G2) whereas the low values (0.65 +/- 0.18 ng/ml) in poorly differentiated adenocarcinomas were noted. Statistically significant differences were observed among control group and moderately differentiated adenocarcinomas and between all groups of adenocarcinomas. In two patients with sarcoma uteri the lowest EGF (0.49 and 0.58 ng/ml) serum concentrations were found. Our results confirm that low EGF serum concentrations as well as EGF receptor on the cell surface probably may increase the risk of cancerogenesis in human endometrium.

A simple, sensitive method of DNA analysis of nucleotide substitutions, namely, single-strand conformation polymorphism analysis of polymerase chain reaction products (PCR-SSCP analysis) was used for detection of mutated K-ras gene in specimens of human endometrial cancer. The mobility shifts of single-strand DNA fragment was seen in 3 of 8 analysed neoplasms: two in stage I and one in stage II, according to FIGO classification, K-ras gene mutation was found in 3 out of 4 patients over 66 years old.

The treatment of tamoxifen, widely used as adjuvant chemotherapy for breast cancer, increases significantly the risk of developing endometrial cancer. The miscoding properties of tamoxifen-derived DNA adducts, alpha-(N2-deoxyguanosinyl)tamoxifens (dG-N2-tamoxifen), have been explored, using an in vitro experimental system to quantify base substitutions and deletions. Site-specifically modified oligodeoxynucleotides containing an epimer of trans- and cis-forms of dG-N2-tamoxifens were prepared postsynthetically and used as templates in primer extension reactions catalyzed by mammalian DNA polymerases alpha, beta, and delta. Pol alpha catalyzed incorporation of dCMP and dAMP opposite all four stereoisomers of dG-N2-tamoxifen, accompanied by lesser amounts of dGMP. In contrast, pol delta catalyzed preferential incorporation of dCMP, a correct base, opposite the lesions; one of the trans-forms of dG-N2-tamoxifens only promoted incorporation of dTMP. Using pol beta, preferential incorporation of dCMP, along with small amounts of incorporation of dAMP and dGMP, was detected. One- and two base deletions were also observed with pol alpha and pol beta. The miscoding specificities and frequencies of dG-N2-tamoxifens varied depending on the DNA polymerase used. In addition, with pol alpha and pol beta, large amounts of 5-base deletions were preferentially formed at the cis-forms of dG-N2-tamoxifen, but not at the trans-forms of dG-N2-tamoxifen. We conclude that dG-N2-tamoxifen adducts have high miscoding potentials.

The aim of the study was to investigate the incidence and risk factors for endometrial adenocarcinoma which are associated with abnormal bleeding patterns, overweight, a lower number of deliveries and a later menopause, diabetes mellitus and hypertension. A retrospective study (from 1990 to 1993) of 2887 women with pathological gynecological signs and symptoms was performed. Risk factors such as age, weight, height, Brock index, age of menopause, diabetes mellitus and hypertension, stage and cell type were analysed. The patients were between 38 to 84 years old (average 53.9 +/- 10.2). They were divided into two subgroups: 259 patients below 84 years of age with histologically confirmed neoplasm and 2628 women hospitalised due to gynecological signs and symptoms which were not, however, diagnosed as neoplastic disorders. It was shown that overweight woman in sixth life decade with a lower number of deliveries and a later menopause had a higher risk of endometrial cancer.

However, it is necessary to note the relationship between the typical age for these two characteristics and the typical age for endometrial cancer. Diabetes was found to be a strong factor whereas hypertension appears to be a relatively weak one for endometrial carcinoma. High Brock's index is also associated with ab increased risk of endometrial carcinoma.

PMID:[Evaluation of risk factors for endometrial adenocarcinoma in women with diabetes, hypertension and obesity hospitalized for pathologic uterine hemorrhage around the age of postmenopause]. 949 9

Transvaginal sonography (TVS), computed tomography (CT) and magnetic resonance imaging (MRI) were used for preoperative diagnosis of myometrial and pelvic lymph nodes invasion in patients with endometrial cancer. Results of examinations were compared with histopathological findings. Sensitivity, specificity, efficacy, positive and negative prognostic values of these imaging techniques were compared. TVS and MRI had similar efficacy in the evaluation of depth of myometrial invasion (78.8% vs 67.6%). In the evaluation of deep invasion TVS in spite of lower sensitivity had higher specificity and positive prognostic value. Efficacy of all methods for evaluation of lymph nodes metastases was similar (80-82%), but TVS had higher positive prognostic value than MRI. CT in comparison with TVS and MRI was less useful in the diagnosis of myometrial invasion.

PMID:[Preoperative evaluation of the invasiveness of myometrial and pelvic lymph nodes in patients with endometrial cancer]. 969 20

Usefulness of fractionated curettage, histeroscopy, transvaginal sonography and MR-imaging in the preoperative diagnosis of cervical involvement of endometrial cancer was studied. Sensitivity, specificity, efficacy, positive and negative prognostic value of above-mentioned examinations were compared. None of examinations was adequate for diagnosis of presence of cervical invasion because of their low positive prognostic values. Negative prognostic value of all examinations was high. MR-imaging and transvaginal sonography had the highest efficacy and comparatively high sensitivity and specificity. Authors suggest, that transvaginal sonography--considering its simplicity and relative cheapness--should be obligatory in the diagnosis of endometrial cancer advancement.

Authors shows three cases of very rarely occurring ovarian struma. First case--was discovered in a patient with normal physiological hormonal activity. Second case--was discovered in a postmenopausal patient with hyperthyroidism and with endometrial carcinoma. Third case--was discovered in a postmenopausal patient, without any complaints, where an ovarian tumor was found during periodical gynaecological examination. Histopathological evaluation of the material was decisive for the final diagnosis.

In our study we estimated the uses of transvaginal ultrasonography in the diagnostic of endometrial carcinoma. There was noted that the most characteristic symptoms of this neoplasma in USG is the thickening of the endometrium and the absence or distortion of "central echo" (imaging of the endometrial cavity). We confirmed the co-existence of common epidemiologic factors of carcinoma endometrium too.
