Radioimmunoscintigraphy (RIS) was performed in 20 patients with gynecologic tumors, 14 ovarian, 5 cervical, and one
endometrial carcinoma
. One murine monoclonal antibody (mab) against
placental alkaline phosphatase
(H7) was used after radiolabeling with 131I. The labeling procedure yielded antibodies with specific activity varying between 60 and 73 MBq/mg mab. Each patient received 57 to 100 MBq of the preparation. RIS was performed 7 to 35 days later. Patients with ovarian adenocarcinoma had an accumulation of activity on RIS at tumor sites (79%, 11/14) verified by ultrasonography, CT, and clinical examination. A low or absent accumulation of activity was seen in patients with cervical tumors. The patient with an endometrial adenocarcinoma was seen to have an activity accumulation at RIS corresponding to tumor sites determined by ultrasound and/or CT. It is concluded that RIS using monoclonal antibodies against
placental alkaline phosphatase
can provide information which will supplement that gained from other investigations of patients with ovarian adenocarcinomas.

We established seven hybridomas secreting murine IgG monoclonal antibodies (MoAbs) to
placental alkaline phosphatase

). The seven hybridomas were designated (1) 7C6, (2) 6G10, (3) 5B9, (4) 6D5, (5) 6B5, (6) 11G6 and (7) 3E10, respectively. The characteristics of these hybridomas were evaluated by radioimmunoassay (RIA) with 125I-
PLAP
. Their reactivity with the intestinal alkaline phosphatase, one of the alkaline phosphatase isozymes, was (1) 0.04, (2) 0.2, (3) 1.4, (4) 1.8, (5) 0, (6) 4.0 and (7) 6.2(%), respectively. None of them showed signs of cross-reactivity with the liver-type alkaline phosphatase, also one of the alkaline phosphatase isozymes, within a
PLAP
concentration of 2,000 IU/l. The subtype of 5B9 was IgG1, and that of the others was IgG2a. We then used 7C6, to develop a sensitive, specific and convenient enzyme immunoassay (EIA) for the determination of
PLAP
, and assayed sera from patients with various gynecologic diseases. The incidence of increased
PLAP
was 6.4% in patients with benign diseases, 21.5% in cervical cancer, 36.4% in
endometrial carcinoma
, and 39.5% in malignant ovarian tumors. The specificity for malignant diseases seemed to be higher than that of CA125. Among endometrial carcinomas, well-differentiated adenocarcinoma had the highest incidence of an increased concentration. Among malignant ovarian tumors, serous cystadenocarcinoma, endometrioid carcinoma, dysgerminoma and Krukenberg's tumor showed a higher incidence than the other types.

The tumor cells stained for HPL are localized and the SP1-positive cancer cells are almost entirely restricted to an infiltrating front. alpha-AT is shown in both tumor cells with marked cellular atypism and ones in deeply infiltrating foci. is higher than that in endometrial hyperplasia. In

Methotrexate (MTX) was covalently bound to rabbit antibodies (IgG) against hCG and
placental alkaline phosphatase
(PLALP) by two different methods. By carbodiimide method, 16.9 mols of MTX could be coupled to one mol of IgG, but the antibody activity was completely lost. Whereas, 2.6-3.4 mols of MTX were bound to one mol of IgG and a quarter of original antibody activity was preserved by dextran bridge method. Choriocarcinoma cells BeWo and SCH were exposed to the IgG-MTX conjugates prepared by the latter method for 30 minutes, and the cell growth was investigated. To BeWo, which produces much hCG and less PLALP, anti hCG-IgG-MTX showed a considerable growth inhibition compared to the control groups (p less than 0.05). No effect was observed with anti PLALP-IgG-MTX, nonimmunized IgG-MTX, free MTX and anti hCG-IgG. In contrast, to SCH, which produces much PLALP and less hCG, anti PLALP-IgG-MTX exhibited considerable inhibition (p less than 0.05), whereas anti hCG-IgG-MTX showed no inhibition. Neither anti hCG-IgG-MTX nor anti PLALP-IgG-MTX showed any effect to the
endometrial carcinoma
cell HEC50B. We concluded that MTX bound to anti hCG-IgG and anti PLALP-IgG have specific growth inhibition effects to the choriocarcinoma cells in vitro, and suggested the possibility of specific immunochemotherapy of this disease.

In order to examine the production of marker proteins, a reproducible method has been established for culturing purified epithelial cells from normal and malignant endometrium. We have examined the differential expression of secretory proteins using immunohistochemistry in frozen tissue sections, immunocytochemistry in cell cultures derived from the same specimens and protein assays on the culture supernatants. Placental protein 14 (PP14) was produced by normal premenopausal epithelium but not by the post-menopausal or malignant endometrial epithelium. In contrast,
placental alkaline phosphatase
(
PLAP
) was produced by endometrial cancers and the endometrial adenocarcinoma-derived cell line Ishikawa, but not by the normal endometrial epithelium. Other markers such as CA-125, which was produced by both normal and malignant endometrium but not by the cell line, and human chorionic gonadotrophin (beta-hCG), which was produced by Ishikawa cells but not by any of the fresh tissues, were less cancer specific.
