13 of 188 patients with a Papanicolaou smear Type IV of the cervix showed histologically only abnormal epithelium, CIN I or II, a result which does not explain the cytologic result. For several reasons a hysterectomy was performed in 5 cases. In only one of these patients a pathological finding, an endometrial carcinoma stage 0, was detected. In the other 8 cases a routine procedure under regular colposcopic and cytologic control had been performed. According to the results during a median follow-up over 52 months, such routine procedure is justified, if the patient wants children and if respectively a regular colposcopic/cytologic control or a control curettage after 3-6 months is undertaken.

SCC (Squamous Cell Carcinoma) antigen is a fraction of the tumor antigen TA-4, obtained from squamous cell carcinomas of the cervix uteri. In a retrospective study the clinical significance of SCC antigen was investigated in sera of 119 controls, 30 patients with cervical intraepithelial neoplasia (CIN I-III), 170 women with cervical carcinoma, and 82 patients with other malignant gynecological tumors. Radioimmunoassay was performed with a kit manufactured by Abbott Diagnostics. The limit of the normal range was 2.5 ng/ml. Elevated serum concentrations of SCC antigen were measured in 5% of blood donors, 3% of patients with uterus myomatosus, and 13% of women with CIN I-III. Pathologic SCC antigen concentrations were found in 62% of patients with primary and 73% of women with recurrent cervical squamous cell carcinomas. Only one out of eleven patients with a primary or recurrent adenocarcinoma of the cervix had a slightly elevated antigen level. The positivity rates depended on the spread of the cervical squamous cell carcinomas of the cervix and rose from 32% at FIGO stage I to 83% at stages III/IV. Only 2% of the patients with no evidence of recurrent disease after successful primary treatment of a cervical carcinoma had SCC antigen concentrations exceeding 2.5 ng/ml. The positivity rates were 33% in cases of primary vulval and vaginal carcinomas, 8% in primary endometrial carcinoma, and 15% in primary ovarian carcinoma. The matrix metalloproteinases (MMPs) are a family of proteolytic enzymes, which are capable of degradation of the proteins composing the extracellular matrix and basement membrane. Their proteolytic activity depends on their binding to metal Zinc and is controlled by tissue inhibitors of MMP (TIMPs). Degradation of the extracellular matrix and basement membrane is an important component of the process of tumor invasiveness, progression, angiogenesis and metastatic spread. Since MMPs may serve as markers of tumor behavior and as predictors of survival and since synthetic inhibitors of MMP may have a place in the treatment of cancer, researching MMPs and their tissue inhibitors in malignant diseases has attracted growing attention. Studies on MMPs and their tissue inhibitors in malignancies of the female genital tract have shown the following: 1) In ovarian carcinoma and cervical carcinoma, over-expression of MMP-2 and MMP-9 is associated with invasiveness, metastatic spread and poor prognosis; 2) In endometrial carcinoma, MMP-7 (matrilysin) is the main MMP associated with invasiveness, metastatic spread and poor prognosis; 3) In cervical intra-epithelial neoplasia (CIN), measuring MMP-2 can assist in identifying high-risk for progression CIN I and CIN II; 4). In vulvar squamous cell carcinoma, over-expression of MMP-13 is associated with invasiveness, metastatic spread and poor prognosis. It is speculated that using synthetic drugs that inhibit MMPs in combination with conventional chemotherapy may contribute to the improvement of treatment results in cancer patients.
