During a follow-up of 665 patients with carcinoma of the breast age 40 years or older, 46 developed a second primary carcinoma of the opposite breast and 30 developed a nonmammary malignancy (median FU = 6 yrs). Comparing our number of observed cases with the number of expected cases (calculated from the female age-specific incidence rate of the data of the Surveillance, Epidemiology and End Results (SEER) and from the actual cumulative person-years of follow-up), our patients with breast cancer did not have higher probability of developing additional nonmammary malignancy, but did have over five times the chance of developing a second breast cancer as other females to have any such cancer. Comparing the distribution of specific sites of our nonmammary second cancer with that reported by the SEER's study, the proportions of patients with colorectal, lung cancer, and lymphoma were similar. But we did see relatively more carcinomas of the uterine cervix, ovary, vulva, sarcoma, and stomach/duodenum, and fewer carcinoma of the endometrium. These differences could be explained by our small series, patient characteristics, and selected referral pattern.

Information on whether a woman had ever had any children was recorded for all deaths registered to ever-married women in England and Wales between 1938 and 1960. Analysis of the relation between parity and cause of death for 1.2 million women aged 45-74 years revealed that parous women had lower mortality from breast, ovarian, and endometrial cancer than did nulliparous women but a higher mortality from diabetes mellitus, gallbladder disease, cancer of the uterine cervix, nephritis and nephrosis, hypertension, ischaemic and degenerative heart disease, cerebrovascular disease, and all causes of death. There is a parallel between the long term effects of pregnancy and of oral contraceptives on health.

Fifty one endometrial adenocarcinomas were used in this study. For light microscopic observations paraffin section from 43 cases were processed with H and E, PAS, and Grimelius argyophil reaction. Besides, 21 of these endometrial adenocarcinomas (25.6%) showed positive Grimelius' argyrophil reaction. With regard to the degree of histological differentiation, these 11 cases were classified as G1 or G2 and all G3 cases (2 cases) revealed negative for Grimelius argyophil reaction. Under electron microscope, in 3 cases out of 21, many small granules reminiscent of neurosecretory type granules mostly round in shape with high electron dense core so often surrounded by a limiting membrane were detected in some of the endometrial carcinoma cells having argyophil granules under light microscope. Since the distribution of these granules was assumed to be random however, the clusters of granules so often located in the groundplasm of subapical portion and subnuclear area near the epithelial basement membrane of the glandular cells. In high magnification, they consisted of an aggregation of fine granular substances with high electron density. Based on the morphological characteristics we made a comparative study between argyophil cell carcinoma of the endometrium and apudomas of the uterine cervix.

The case of a 23-year-old woman with a primary small cell carcinoma of the endometrium is reported. The light microscopic features were similar to those of small cell carcinoma of the lung and those of the uterine cervix. Electron microscopy revealed cytoplasmic neurosecretory type granules and indistinct cell junctions, features characteristic of neuroendocrine cells. The neoplasm behaved in a very aggressive manner, similar to that of small cell carcinoma of the lung and quite unlike that of the usual endometrial adenocarcinoma. Small cell carcinomas of the endometrium with neurosecretory type granules may have a worse prognosis than better differentiated endometrial adenocarcinoma with argyrophilic granules and thus may require adjuvant radiotherapy or chemotherapy or both.

Therapeutic efficacy of cervical conization was examined on 93 patients with borderline lesions of the uterine cervix who underwent hysterectomy following conization. No lesions or only minimal residual lesions were found at subsequent hysterectomy when the surgical margins of cone specimens were not involved in the malignant or premalignant lesions. From these results, we concluded that cervical conization could be considered as a therapeutic procedure when the following conditions were fulfilled. Cone specimens are sufficient in size. Both endocervical and exocervical margins are negative. No malignancy is found on endocervical curettage. No abnormal cytologic findings are found after conization. Another follow-up study was performed on 69 patients who received therapeutic conization between 1971 and 1981. Among them one developed stage II cervical cancer two years after the last follow up. Two other patients had recurrent CIN in the cervix and another patient was found to have an endometrial cancer. These patients were successfully treated after early detection of the lesions by the cytological examination. From these facts, cytological examination is recommended in the follow-up after therapeutic conization.

Formation of metastases in the ovaries in carcinoma of the uterine cervix is a rare occurrence. 749 ovaries were subjected to histological examination in 980 operated patients with squamous cell carcinoma of the cervix. A metastatic carcinoma in the ovary was identified in four patients (0.5%). In three patients, a tumour had formed simultaneously in the corpus uteri. This might point to a metastatic spread of the tumour in the same manner as in carcinoma of the endometrium. In view of the low incidence of metastases in the ovaries adnexectomy would not be generally justified.

The presence of cytosol estrogen (ER) and progestin (PR) receptors in specimens of normal uterine cervix, endometrium, myometrium, Fallopian tubes and corpora lutei or in samples of neoplastic female reproductive tissues was investigated. The material consisted of PR and ER measurements of tissue samples obtained from fertile and postmenopausal women, receptor assays were performed by a dextran-coated charcoal technique. The radio PR/ER was highest in specimens from the Fallopian tubes (44) and endometrium (4-10),2-3 in specimens of uterine ecto- or endocervical epithelium and about 4 in the myometrium. No ER or PR were found in the four corpora lutei examined. PR or PR/ER ratio in specimens of myoma tissues did not differ from that found in the myometrium specimens. The highest ER and PR values in the endometrium were measured in specimens taken during the late proliferative or intermediate phase of the menstrual cycle. The PR values in specimens from postmenopausal myometrium were lower in comparison with the samples taken from the myometrium of fertile women in contrast to ER values. In the specimens of carcinoma of the vulva and the uterine cervix ER levels were very low, with no measureable values of PR. In the endometrial carcinoma samples the PR/ER ratio was lower than in the normal endometrium. 4 out of 7 specimens taken from ovarian adenocarcinoma had a measureable amount of ER and in 2 out of 7 cases PR.

The trends in mortality from cancer of the uterine cervix, cancer of the endometrium and all uterus in Canada and the ten Canadian Provinces from 1951-53 to 1974-76 at ages 30-64 have been re-evaluated in relation to screening for cancer of the cervix in 1966 and 1971 and hysterectomies for non-malignant conditions from 1969 to 1976. By means of series of mathematical models, the effect of different prior extrapolations of the numbers of hysterectomies performed by age and province has enabled the potential impact of hysterectomies on the trends of mortality from uterine cancer to be evaluated by relating deaths to "uteri at risk" rather than "women at risk" irrespective of the presence of an intact uterus. It has been found that the numbers of hysterectomies performed have little impact on the fall in mortality certified to cancer of the cervix or to all uterine cancer, but in the decade 1964-66 to 1974-66 they convert an apparent fall in mortality from cancer of the endometrium to stability. Re-evaluation of the fall in mortality from 1960-62 to 1970-72 in relation to the intensity of screening shows little impact of the hysterectomies performed on the significant correlation reported previously. However, after extension of the analysis to 1974-76 the correlation disappears. This finding may indicate a more limited potential for the application of screening, as practised in Canada, to reduce mortality from cancer of the cervix, than has been anticipated previously.

We report a histochemical study of alkaline phosphatase (ALP) in normal cells of the female reproductive system, in pre-cancerous and cancerous lesions of the uterine cervix and in endometrial cancer to ascertain the incidence of ALP and its isoenzyme type. For this purpose, serial sections were subjected to heat stability and L-phenylalanine (LP) inhibition tests. The Regan-like isoenzyme, a heat-stable and LP-sensitive ALP, which has been thought to derive only from cancer or the placenta, was found in uterine cervical reserve cells and endometrial luminal surface lining cells. In contrast, ALP activity in endometrial glandular cells was found to be heat and LP sensitive. Of 183 cases of cervical neoplasia, 60 (33%) manifested non-specific ALP activity. One dysplasia and two invasive cancer cases manifested the Regan-like isoenzyme. The other 36 classifiable lesions had small-intestine ALP-like activity (marked heat and LP sensitivity) or a liver ALP-like isoenzyme (marked heat and slight LP sensitivity). Of 42 cases of endometrial cancer, all cases manifested non-specific ALP activity. Seven endometrial cancers exhibited the Regan-like isoenzyme. The other 19 cases manifested either small intestine or liver ALP-like isoenzyme. Our findings indicate that in the course of uterine carinogenesis, the ALP isoenzyme of reserve cell and endometrial glandular cells undergo a change and that enzyme deviation occurs.

The incidence of endometrial carcinoma is particularly high in postmenopausal women. Screening techniques are not yet satisfactory. Postmenopausal anatomic physiologic changes in the genitalia may prevent visualization and exploration of the uterine cervix. A case report is presented of an 87-year-old woman with a 6-month history of genital bleeding. Severe vaginal stenosis prevented diagnostic curettage, even under adequate anesthesia. To perform the indicated hysteroscopy, a panendoscope had to be employed for direct visualization of the cervix and uterine cavity. Selective biopsy specimens revealed endometrial adenocarcinoma. The hysteroscopy technique is easy to learn. However, if the patient's physician has not acquired this experience, a urologist, who must necessarily use endoscopy extensively, can be of great help in the evaluation fo gynecologic patients with uterine problems similar to those described in this report.
