Circulating carcinoembryonic antigen (CEA) and alpha fetoprotein (AFP) levels were measured by radioimmunoassay in 53 patients with

of the ovary, 16 patients with other malignant genital tumors, and 31 women with nonmalignant diseases of the genital tract. The serum CEA concentration was elevated (greater than 5 ng/ml) in 11 patients with ovarian cancer, 2 patients with
endometrial cancer
, 1 patient with carcinoma of the cervix, and 1 patient with a benign embryonal cystic teratoma. Elevated CEA levels were found only in patients with advanced malignant disease, while early stages were associated with normal CEA concentrations. AFP levels were normal in all but 1 patient. Both CEA and AFP levels were markedly raised in a case of advanced genital
carcinoma
arising probably from the ovary. Ascitic fluid of another patient with ovarian cancer contained a high concentration of CEA, giving an identical reaction in immunodiffusion with CEA from colon cancer. The present results indicate that while the increased expression of carcinofetal components takes place in some malignant tumors of the female genital tract, it is usually a late phenomenon.

Two hundred patients at high risk for endometrial adenocarcinoma were screened using the Gravlee Jet Washer. Tissue obtained from the first 100 patients was processed by cell block technic alone; tissue from the second 100 patients was processed by cell block plus direct smear and Nucleopore filtration stained by the Papanicolaou method. All patients had histologic diagnosis by fractional curettage. Six of 12 patients with a tissue diagnosis of
carcinoma
were missed by the jet washer using the cell block technic alone. In the second 100 patients, no cases of cancer were missed using both cell block and cytologic technics. Technic and application of the jet washer in screening for
endometrial carcinoma
are discussed. We conclude that the Gravlee Jet Washer is a safe, effective screening tool which frequently may replace diagnostic curettage; however, if abnormal bleeding persists, curettage must be done.

The treatment regimens are described in 74 patients with endometrial disease among 850 climacteric women receiving oestrogen therapy. Cystic hyperplasia was associated with unopposed oestrogen therapy without progestagen. undergo primary irradiation. The 5-year survival rate amounted to 50.4% of 802 patients in the period from 1950 through 1970. Additional therapy with gestagens is discussed and recommended. The histological alterations in the material obtained by so-called "second-look curettages" are reported. Application of gestagens to other hormonesensitive tumors such as

Carcinoma
of the cervix or endometrium was evaluated in 1,021 patients at the Joint Center for Radiation Therapy, Boston, between July 1968 and December 1977. The patients were retrospectively evaluated for the presence of lung metastases, appearing initially or during their disease course. On chest radiography, 42 patients were found to have metastases. Lung metastases were seen in 5.1% of patients with carcinoma of the cervix and in 3.6% of patients with
carcinoma of the endometrium
. Median time from initial disease staging to detection of lung metastases was 12 months. Once pulmonary spread was discovered, 80% of patients expired within 1 year. Lung nodules varied greatly in size. In 11 patients they were solitary; five patients had pleural effusions; three had mediastinal or hilar adenopathy; and none had excavation.

Numerous investigators have alluded to an association of
cancer of the endometrium
and breast. There is no available information relating the histologic forms of
cancer of the endometrium
to breast cancer. We found ten cases of adenosquamous
carcinoma of the endometrium
in one tumor registry in a ten year period, 1967-1977. Five of these cases also had breast
carcinoma
. In three of these cases the breast
carcinoma
was discovered two to three years after hysterectomy; in one case both tumors were discovered at the same time, and in one case the breast
carcinoma
antedated the diagnosis of endometrial malignancy by approximately one year. These observations, although from a small number of cases, warrant an expansion of this study in order to determine whether patients with adenosquamous carcinoma of the uterus have a higher risk of developing breast cancer.

Comparing cases of
endometrial cancer
(EC) (451) to matched hospital controls (888), the estimated risk of EC associated with estrogen (E) use in this large-scale study was 6, with a 95% confidence interval of 3.7-9.7; this figure jumped to 15 for long-term E users ( 5 years). The case-control study found that excess risk was present for both diethylstilbestrol and conjugated Es. Cancer risk was associated about equally with cyclic and continuous dosage regimens, and all histologic grades of tumors were associated with increased risk with E use. For State I tumors, all 3 histological grades ("well-differentiated"-"poorly differentiated") were found with E use. The relative risk of advanced-stage

was 4 for E users, but the confidence interval was wide, and this question remains unclear. Questions of methodology are addressed which apply to all studies of E use and EC; these include suspicions that women under treatment with E receive swifter diagnoses of

A retrospective analysis of 91 patients with
endometrial carcinoma
is presented with emphasis on important factors in relation to survival. Patients with Stage 1
carcinoma
were treated with a standardized mode of preoperative radiation and extended abdominal hysterectomy. An 87.1% 5-year survival rate was obtained for patients with well differentiated lesions, with only 1 patient developing a vault metastasis.
