Postmenopausal bleeding is associated with a relatively high incidence of
malignancy
. The lowest incidence reported in the literature was observed among Jewish women in Jerusalem, more than 21 years ago (1). A similar survey of Jewish women admitted to our Department for postmenopausal bleeding during 1962-74 is presented. Fifty-five of 397 cases (13.8%) of postmenopausal bleeding were due to
malignancy
. There were 34 women with
endometrial carcinoma
, 11 with cervical carcinoma, five with ovarian carcinoma, four with uterine sarcoma and one with vaginal sarcoma. In 86% of cases, benign pathological states were found, 42.8% being associated with atrophic endometrium. An active endometrium was found in 56 patients (14%), and in two of them the endometrium was secretory. Estrogen therapy was not an important causative factor in these cases. The low incidence of
malignancy
seems to be due to the fact that cervical carcinoma is less common among Jewish women. Nevertheless, 20% of the malignant tumors in this series were invasive epidermoid carcinomas of the cervix. The need for a cytologic screening program in this country must therefore be reevaluated.

The immunobiology of heterotransplanted human tumors was investigated following transplantation into nude mice of human bronchogenic, colon, rectal, ovarian, gastric, endometrial, vaginal, bladder, renal, esophageal, embryonic cell, pancreatic, and breast carcinoma, as well as fibrosarcoma, rhabdomyosarcoma, malignant melanoma, astrocytoma, Wilm's tumor, endometrial hyperplasia, and hydatidiform mole. Several of these tumors were passaged up to 15 generations. During these passages no changes in latency period for tumor development or in histology were noted. There were significant differences between several tumors in the minimum number of cells required for successful transplantation; such differences were independent of the basic biologic aggressiveness of the individual tumors. Nude mice that received transplants of fibrosarcoma and
endometrial carcinoma
had increased serum IgM and numbers of spleen cells and complement receptor lymphocytes. No such changes were noted for mice that received transplants of malignant melanoma, In contrast, there were no apparent differences in the responses of nude mice, who were given transplants of human tumors, to be T-cell mitogens concanavalin A or phytohemagglutinin or in the number of theta-bearing spleen cells. The success rate for transplantation was significantly improved when explants, rather than single-cell suspensions, were performed. Tumors transplanted to nude mice derived from strictly homozygous matings behaved like tumors transplanted to mice born of heterozygous mothers. Finally, despite the dramatic size of subcutaneous tumor nodules, there were no examples of invasion or distant metastases.

One hundred and seventy-nine patients with
endometrial carcinoma
underwent hysterography as part of their pretreatment evaluation. One hundred and twenty-six women received preoperative irradiation followed by hysterectomy; 30 had surgery alone; while 23 received radical radiotherapy. We have classified hysterograms by the degree of tumor invasion suggested radiographically and have compared these, in the operated cases, with postoperative pathologic examination. Characteristically, lesions confined to the endometrium, or with superficial myometrial penetration, demonstrate either no radiologic abnormality or a small, shallow defect by hysterogram. In those patients with deep myometrial invasion of
cancer
, hysterograms usually demonstrate a large defect in the endometrial cavity. We believe that hysterography helps assure optimum treatment of each individualized case by helping to determine the volume of tumor and extension into the myometirum.

The incidence of
endometrial cancer
has increased rapidly during recent years, paralleling the increased postmenopausal use of estrogens. The relation of
endometrial cancer
to disease conditions in which estrogens are elevated and three case-control epidemiologic studies suggest the likelihood of a cause-and-effect relation between estrogen use and
endometrial cancer
. These studies and New York State
Cancer
Registry data further suggest that the incidence will continue to rise in coming years. A thorough reevaluation of the medical indications for estrogen replacement therapy is urgently needed. Women with long-term exposure to estrogens should have periodic
cancer
screening examinations.

Mixed adenosquamous
carcinoma of the endometrium
have been reported in recent years to have a steady increase in incidence, extreme aggressiveness, poor responses to radiation therapy, and a low five-year survival (less than 20%). In the present report, 87 mixed carcinoma (MC) are compared with 260 pure adenocarcinomas (AC) and 29 adenoacanthomas (AA). There were no basic differences in incidence, clinical history, responses to radiation therapy, and prognosis for any of these three entities. Adenocarcinomas of the endometrium with and without squamous elements should be regarded and approached as any pure AC. There is an overall tendency for endometrial carcinomas to be at an early stage at diagnosis and the five-year survival regardless of pathologic type, stage, grade, myometrial invasion, and therapy is 80%.

Three hundred and seventy-two patients with
endometrial cancer
treated from 1948 through 1969 and having complete follow-up were reviewed. All patients completed definitive therapy with preoperative irradiation and surgery or irradiation alone and have separated into two main groups: 1) those treated with irradiation by radium alone or 2) those treated with external irradiation plus diminished amounts of radium. Pathology material from each patient was reviewed by one pathologist. Treatment by radium alone led to fewer pelvic failures than did external irradiation plus diminished amounts of radium, when evaluated by stage, grade, and specific histology. Both irradiation modalities led to about the same incidence of failures due to distant metastasis only.

A review of all newly diagnosed cases of cervical and
endometrial carcinoma
in Israel during the 5-year period of 1961-1965 yielded mean annual incidence rates of 4.9/100,000 and 7.4/100,000, respectively. Cervical cancer was more prevalent in Moroccan-born women and among divorcees, while the risk of
endometrial cancer
was highest in older age, among the European born, and the single; it also appeared earlier in life. Postmenopausal bleeding constituted the most frequent first symptom in both sites. Fifty percent of the patients of both groups were diagnosed within 1 month, but the delay was somewhat longer in the endometrial group. Median survival was 5 years in patients with cervical cancer and above 12 years in those with
cancer
of the corpus. Five-year survival was 50 and 75%, respectively. Survival tended to be better in younger patients in both groups. It is expected that the gradual disappearance of intraethnic differences in Israel would lead to a decrease in the incidence of invasive cervical cancer, coupled with an increased incidence of the endometrial category.

Androstenedione metabolism was studied at the New York Medical College-Metropolitan Hospital Center in 14 patients with
endometrial cancer
. 5 normal postmenopausal patients served as controls. A priming dose of 15 mcCi of 7 tritiated-androstenedione was given followed by 30 mcCi of the same steroid for 130 minutes at a rate of .38 ml/minute. Blood samples were collected at 130, 145, and 160 minutes. There is a significantly higher conversion of androstenedione to estrone (p less than .05) but not to estradiol (p greater than .7) in
endometrial cancer
patients as compared with the controls. The mean plasma concentration was significantly elevated (p less than .02) in the
cancer
patients compared with the postmenopausal controls. There was no statistical difference in plasma levels of estrone and estradiol (ps greater than .3 and greater than .4, respectively) in the 2 groups of patients. It "appears that estrone may have a significant effect on the estrogen-responsive organs such as the endometrium."

The serum enzymatic activities of cystine aminopeptidase and leucine aminopeptidase were measured in a group of 113 patients of whom 90 had benign uterine or ovarian tumors, and 23 had
cancer of the endometrium
or ovary. Thirty healthy nonpregnant women and 260 women at different stages of normal pregnancy served as control groups. The presence of pregnancy-specific enzymes in women with uterine or ovarian tumors showed once again that similar processes occur during pregnancy and
malignancy
. When ovarian or uterine
malignancy
is suspected on clinical examination, determination of the activities of these enzymes in serum may be of diagnostic value.

In order to prevent vaginal recurrences after hysterectomy for
endometrial cancer
, a operation method is described which permits sampling of endometrial cells and enables cytologic examination, evaluation of sensitation response and radiation response and search for vital
cancer
cells after preoperative irradiation. In 84 endomentrial carcinomas operated upon using this procedure no vaginal reccurences have been met.
