Basic parameters of central and intracardiac hemodynamics were studied in 49 urological patients 24 of which with urolithiasis entered group I, 13 with hypertension-group II and 12 with varicocele-group III. The patients' age averaged 46.4, 41.6 and 28.6 years, respectively. The data were provided by routine clinical and laboratory examinations, ECG, one-passage radionuclide cardiography with 132I-albumin using a radiocirculographer of Hungarian manufacture and radiocardioanalyzer RKAZ-01 made in this country. Neither marked ischemic disturbances of the myocardium nor valvular defects were revealed. Ambiguous group-specific shifts presented in central and intracardiac hemodynamics. Total peripheral vascular resistance exhibited a moderate increase while left ventricular circulation time grew 1.5-2-fold. The greater resistance can be attributed to activation of renin-angiotensin system in prolonged ischemia of renal parenchyma due to nephrolithiasis. Group II patients demonstrated parallel elevation of arterial pressure, peripheral resistance, left ventricular performance and output suggesting myocardial functional stress. In group III there was a rise in blood volume, left ventricular performance and output, cardiac index, stroke volume. This myocardial overloading may result from changes in intravascular volumetric relations characteristic of hypervolemia. These hemodynamic changes reflect adaptation in urological patients and should be accounted for in treatment and operative interventions.

828 patients were operated on the retroperitoneal organs since 1985 using a retroperitoneoscope. A total of 836 surgical interventions were performed. A cut of 3.0-3.5 cm long was made to revise the kidney and ureter, retroperitoneal part of the colon, tail of the pancreas, retroperitoneal vessels and lymph nodes via lumbocostal and inguinal approaches. A new intercostal (ribs X-XI) approach is proposed. Most of the patients had urolithiasis, pyoinflammatory lesions, renal cysts, varicocele, obstructive uropathies. Complications developed in 1.7% of cases. Retroperitoneoscopy is a method of choice in diagnosis and treatment of retroperitoneal diseases.

The article presents the results of the evaluation of in-patient medical care of patients with urological diseases depending of the level of medical care assistance. In the specialized departments of municipal multi-type hospitals and Republic medical establishments, the structure of in-patients with urologic pathology is characterized by larger portion of urolithiasis and lesser portion of varicocele, phimosis, orchitis and epididymitis as compared with the medical care services of municipal district. However, according to the medical insurance organizations expertise, the level of quality of medical care in the Republic medical establishments was higher than in municipal district medical services. The lower level of medical care quality in municipal district formed due to decreasing of treatment quality and volume of diagnostic procedures.

According to the data of preventive medical examinations since the end of 2009 to April 2012, data on the urological incidence in 654 athletes surveyed during this period were collected and analyzed. Among the diseases identified in athletes, the main place is occupied by varicocele (5.35%), urethritis (5.04%), urolithiasis without clinical manifestation of acute inflammation (1.37%).

We sought to describe the incidence rate of the urologic disease in the Korean military by reviewing diagnoses made in active duty soldiers from 2008 to 2013. A total of 72,248 first visits were generated in the Defense Medical Statistics Information System (DMSIS) with its gradually increasing trend over 6 years. A sharp increase of first visit was observed after implementation of the regular health check-up for all conscripted soldiers since 2013. Urolithiasis, prostatitis, epididymoorchitis, urethritis, and varicocele were prevalent. Prostatitis was the highest diagnosis made in the outpatient service, while varicocele was ranked the highest in the inpatient service. The incidence rates of urologic disease varied from 12.3 to 34.2 cases per 1,000 person-years. The urologic disease in conscripted men showed different distribution when we separated the population into conscripted and professional soldiers. Epididymoorchitis was the highest disease followed by urolithiasis, dysuresia, and balanoposthitis in 2013. This study underscores that the urologic disease has spent significant amount of health care resources in the Korean military. This calls for further study to find any significant difference and contributing factors of the urologic disease in the military and the civilian population.
