A retrospective study was carried out on patients with gastrointestinal complaints, who were accidentally found to have varying degrees of
hydronephrosis
between October 1989 and April 1991. The criteria for
hydronephrosis
given by Ellenbogen et al were used for ultrasonographic diagnosis. Grade 0 and 1 were classified as mild degree and grade 2 and 3 as moderate and severe, respectively. The causes of
hydronephrosis
were determined by intravenous pyelography, retrograde pyelography, and/or computed tomography. Of 44 patients with
hydronephrosis
who had complete studies, 20 had a mild degree of
hydronephrosis
, 18 had a moderate degree, and 6 had a severe degree. All 24 patients with moderate and severe degrees of
hydronephrosis
had obstruction as the etiology, but only 50% of the cases with mild
hydronephrosis
were found to have pathological lesions.
Urolithiasis
(79.4%) was the most common cause of
hydronephrosis
in this study. Benign prostate hypertrophy, carcinomas, external compression, and tuberculosis followed.
Hydronephrosis

. The patient was treated successfully with extracorporeal shock-wave lithotripsy (ESWL). Serum creatinine and twenty-four-hour creatinine clearance were unchanged from levels prior to ESWL.

Ureteral motility changes were followed up in 10 patients with
hydronephrosis
and
urolithiasis
which were recorded by multichannel impedance ureterography prior to internal drainage and during stent manipulations. The establishment of the ureteral stents reduces peristaltic amplitude and frequency as well as ureteral tonicity. After the stent removal 20% of the examinees showed peristaltic hyperfunction of the ureter. Urodynamic positive effects of the stents consist in better regulation of the motility, elimination of its local disturbance foci, improved coordination of movements of different parts of the upper urinary tracts. For positive results it is important to practice an individual approach to choice of drainage time and stent size. Urodynamic impedance studies can be used in control of endoscopic treatment adequacy.

In this study we describe 22 cases of retrograde ureteral stent placement in pregnant women with therapy-resistant flank pain due to
hydronephrosis
. Eleven were primiparous and one patient expected twins. Eight of 22 patients presented symptoms of pyelonephritis. In 21 cases the
hydronephrosis
was located on the right and in 4 cases it was bilateral. Maximal lower calix diameter was 12 mm (range 9-22 mm). With the exception of two cases, sonographically controlled stent placement was performed under local anesthesia without sedation. All patients were painfree within 6 days and were given prophylactic low doses of antibiotic until the day of delivery. Renal function remained within the normal limits. Double-J stent displacement occurred in 3 patients - of which one underwent nephrostomy. Postnatal examination demonstrated
urolithiasis
in 4 of 19 patients. This study provides evidence for effectiveness of retrograde ureteral Double-J stent placement as a therapeutic option in cases of severe symptomatic
hydronephrosis
during pregnancy with a low morbidity rate.

Management of urologic disorders in pregnant patients often increases the anxiety of all involved. Based on a thorough understanding of the physiologic changes seen in various organ systems the urologist has to assume the responsibility for the well-being of the mother and the fetus. Apart from the urinary tract infection, which occurs as frequent as in non-pregnant patients but has a significantly higher risk of acute bacterial pyelonephritis, it is mainly the pregnancy-associated symptomatic
hydronephrosis
and the
urolithiasis
which are complicating approximately 1 of every 1000-1500 pregnancies. Urinary tract infections should be treated in any case by antibiotics according to a antibiogram. High risk patients with history of vesicoureteral reflux or recurrent pyelonephritis should be treated prophylactically. Following parturition these patients should be investigated urologically to exclude structural abnormalities of the genitourinary system. In case of symptomatic
hydronephrosis
and calculous disease the first approach should be a watchful conservatism with symptomatic relief. If the symptoms persist insertion of a double-J-stent or in case of live-threatening situations (e.g. urosepsis) when urgent decompression and rapid evacuation is mandatory a percutaneous nephrostomy can be brought in place under sonographic monitoring completely thereby avoiding any radiation exposure.

We report a case of

in a patient with idiopathic hypoparathyroidism treated with vitamin D therapy. A 30-year-old woman with idiopathic hypoparathyroidism, who had been treated with vitamin D therapy with 2-4 micrograms/day of alpha-calcidol for 9 years, was admitted for recurrence of bilateral renal stones and progressing left
hydronephrosis
. Laboratory data revealed normal serum calcium level and remarkable hypercalciuria. The dose of oral administration of alpha-calcidol was reduced to 1 microgram/day and 2 mg/day of trichlormethiazide was started. Now her serum calcium concentration and the total amount of urine calcium was completely under control. Bilateral renal stones are no longer progressive and tetany has not been recognized. We considered it essential to monitor closely not only the serum but also the urine calcium level in the vitamin D therapy for idiopathic hypoparathyroidism.

The kidneys, the bladder and nephrology in general were discussed in the Talmudic literature from their anatomical, pathological and philological aspects. The Sages' deliberations were based on the Biblical texts. The characteristic phraseology of the scriptures uses the kidneys as symbols of the human emotions, contrary to the heart, which is regarded as the location of wisdom. The kidney is considered to be the individual's seat of their deepest desires--perhaps because it is an "internal" organ, well hidden, surrounded by a capsule and by a layer of perinephric fat. The Talmudic corpus considers the kidneys to be the origin of secret counsels--"the kidneys advise". An injury to the kidneys is used symbolically as an example of a cruel and serious injury. The sages were concerned with nephrological problems such as a diseased kidney of small size, fluid and pus in the kidney, injuries, perforations and more. Fascinating advice is given regarding micturition, its timing, characteristics and significance. The Halakhah is concerned with the kidneys during the examination of an animal after slaughter for the fitness for its ritual consumption. The paper presents various nephrological diseases from the Talmudic corpus. Among them are, for example, "Tzemirtha"--
