Fifty eight patients who underwent ileal conduit formation in our hospital for the past 14 years were reviewed. Out of them, 11 patients developed
hydronephrosis
which should be treated. It occurred due to
urolithiasis
, recurrent tumor, contracted ileal loop in one patient respectively, and ureteroileal junction stricture in 8 patients (10 ureteral units). The 8 patients consisted of 6 males and 2 females. All of the males underwent total cystectomy because of bladder tumor (T.C.C.), and the females had anterior pelvic exenteration because of uterine cancer (adenocarcinoma). Out of 10 ureteral units, 7 were corrected by surgical intervention, 1 was dilated with endourological procedure, and 2 had no treatment. Six ureteral units including 1 ureteral unit treated by an endourological method resulted in improvement or disappearance of
hydronephrosis
. Histological examination of the stricture revealed fibrosis in 7 ureteral units treated surgically. According to our clinical studies, infection in the pelvic space seemed to be one of the causes which brought about ureteroileal junction stricture.

A 34 year-old man, who had been under care with diagnosis of Crohn's disease in the department of gastroenterology of our hospital since 1983, was referred to our urological clinic on May 21, 1987, because of right

(2 cases) and ureterocolonic fistula (1 case). The patients were from 10 to 57 years (27.2 years in average) old. Sex distribution was uneven, 39 of patients were men and 2 were women. Urologic surgery was performed in almost all of the cases except for the patient with

. All the patients having vesicoenteric fistula had urological complaints but the patients with

were relatively free from urological complaints. Thus, Crohn's disease has been recognized as an important gastrointestinal disease for urologists, and we will emphasize that periodical abdominal ultrasonography and urography for the patients with this disease should be necessary for checking up other complications such as

Interobserver variations between three radiologists with different backgrounds (duration of practice, and departments) were studied in connection with the diagnosing of
urolithiasis
. Films from 152 consecutive patients (examined with conventional abdominal films, urography/tomography) were analyzed independently. With respect to the number of patients with or without calculi there was agreement between the three observers in 129 of the 152 cases, which was taken to represent very good agreement beyond chance. The total number of stones was interpreted as 832, 460 and 570 respectively, and the median stone size as 6, 9 and 11 mm2. There was a fair interobserver agreement rate regarding whether stones were localized to the renal parenchyma or calyces, and whether stenosis/stricture or
hydronephrosis
was present. Tomography appeared to reduce the interobserver variability significantly. Adequate bowel cleaning, oblique projections, and lack of clinical information apparently did not influence the agreement rate. Two hundred and three patients with urographic appearances suspicious of a space-occupying lesion were examined by sonography. Seventy-nine showed normal results; there were 65 cysts, 20 malignant lesions, 20 pseudo-tumours and 19 with other abnormalities such as

and inflammatory lesions. Pseudotumours are due to varients of the structure of the kidney, which resemble space-occupying lesions in the excretory urogram. The sonogram shows them as low echo, centrally protruding structures arising from the edge of the parenchyma. Angiographically they are islands of parenchyma with normal vessels. Sonography has proved to be a necessary complement to excretion urography and provides a more certain indication for renal arteriography. Angiography can be limited to changes in the parenchyma, since sonography is able to define cystic processes.

ESWL of urinary stones is a well-established treatment in adult patients. The treatment of urinary stones in children has gained increasing importance in recent years. From 1987 to 1993, a series of 27 children with

in all parts of the urinary tract were treated by ESWL. Treatment was performed with general anaesthesia or analgosedation. During the treatment no complications occurred. On average, 34 sessions of ESWL, with 2165 shockwaves and 22.3 kV energy, were performed. Minor early complications, such as fever, pain and

Records of 90
urolithiasis
patients (50 boys and 40 girls) of 5 months to 18 years old (medium 8.7 yr) treated in our department from 1975 to 1993 were reviewed. Family history was found in 25 (27.7%), concomitant uropathy in 9 and predisposing factors to lithogenesis in 35 patients. The most frequent clinical manifestations were hematuria, lumbalgia, dysuria and calculus elimination. Urinary tract infection was found in 26 cases. Treatment modalities used were: medical in 31, extracorporeal shock wave lithotripsy (ESWL) in 12 surgery en 28, endoscopy en 1 and instrumental in 1. Six of 28 surgically treated patients presented residual
urolithiasis
and another 6 relapsed. Three complications were registered. Recovery from
hydronephrosis
and/or vesicoureteral reflux was seen in all patients with these lesions.

. In our institution the patients with ureterosigmodostomy undergo a follow-up protocol in which blood chemistry, ultrasonography, intravenous pyelography and C.T. are periodically performed. The aim of the present paper is to compare the accuracy of kidney sonography after diuretic stimulation with intravenous pyelography in the diagnosis of ureteral stenosis. Out of 91 patient with ureterosigmoidostomy 18 patients (34 kidneys) underwent intravenous pyelography, a basal U.S. and then a dynamic one at 5, 10, 15, 30, 45, 60, 90, 120 minutes after administration of furosemide 20 mg i.v. At basal U.S. 27 kidneys were normal and 7 showed a dilations. After diuretic stimulation we observed 16 normal kidneys, 16 dilated units and 2 intermittent
hydronephrosis
. Out of 16 dilated kidneys 6 became normal in 60 minutes. Out of 10 dilated units 3 were normal in 90 minutes (hipotonic), 2 were normal before 120 minutes (low grade obstruction) and 5 were dilated after 120 minutes (high grade obstruction). With intravenous pyelography we observed 27 normal kidneys and seven dilated units. Dynamic sonography have shown high sensibility (100%), specificity (88.8%) and accuracy (91%) in diagnosis of ureteral obstruction in to I.V.P. in the follow-up of this kind of divesion.
