MHC class I and II molecules serve as restriction elements for the presentation of antigen-specific T cell reactions and may be implicated in the process of cellular transformation. We have analysed the expression of MHC class I and II antigens including the associated invariant gamma-chain in 100 consecutive renal biopsies by a sensitive immunoperoxidase method using monoclonal antibodies against HLA-ABC, HLA-DR, HLA-DQ, HLA-DP and the invariant gamma-chain (Ii). HLA-ABC was expressed by almost all parenchymal cells except for podocytes. Staining for HLA-DR was found consistently in glomerular and interstitial capillary endothelial cells, in "dendritic" (LCA positive) and most infiltrating interstitial cells but not in mesangial and most extracapillary cells. Similar but weaker positivity was observed for HLA-DQ and -DP. HLA-DR positive infiltrating mononuclear cells in glomeruli were noted in acute glomerulonephritis (GN) and Lupus GN associated with enhanced staining of endothelial cells. In addition, HLA-DR was uniformly or focally expressed by predominantly proximal tubules in 80% of biopsies, especially IgA-GN, focal glomerulosclerosis, acute GN but also in 54% of cases with minimal change nephrosis. HLA-DP and -DQ were only positive in a limited number of HLA-DR positive tubules. The expression of the invariant gamma-chain was comparable with the reactivity for HLA-DR. In addition, all HLA-DR-positive tubules expressed the invariant gamma-chain. Subtyping of T cells in biopsies with DR-positive tubules and interstitial infiltrates (n = 20) showed a predominance of CD 4 positive cells in the majority of cases.(ABSTRACT TRUNCATED AT 250 WORDS)

Since the mechanism mediating the beneficial effect of intravenous cyclophosphamide (IVCY) in systemic lupus erythematosus (SLE) is unknown, we investigated lymphocyte subsets and markers of activated lymphocytes in patients received IVCY, and compared the results with the effect of steroid pulse. In 55 patients with SLE, 34 patients receiving IVCY [21 cases (61.8%) were responsive] and 25 patients received steroid pulse [21 cases (84.0%) were responsive] (four patients who were resistant to steroid pulse therapy were transferred to IVCY). When the lymphocyte subsets and markers of activated lymphocytes were compared in the responsive and unresponsive group of IVCY, soluble CD4 levels and the ratio of HLA-DP-positive T cells were significantly higher in the unresponsive group. Further, the changes of these markers and costimulatory molecules [LFA-1 (CD11a), ICAM-1 (CD54), CD40 and CD40-ligand (CD154)] were also examined in the responsive patients. The ratio of HLA-DP-positive T cells did not change in the IVCY-responsive group, while it decreased in the steroid pulse therapy-responsive group. The ratio of CD11a on T cells increased and CD54 on B cells decreased in the IVCY-responsive group. The ratio of CD154 on T cells increased in the steroid pulse-responsive group, while it decreased in the IVCY-responsive group. These results suggest that the effect of IVCY is different to that of steroid pulse therapy and mainly related to B cell activation, and that these markers may contribute to predict the responsiveness of IVCY.

Z39Ig is a recently-identified gene with immunoglobulin-like domains whose function is unknown. We examined expression of Z39Ig in 1432 human cDNA libraries, and found it primarily in synovium of patients with rheumatoid arthritis, in placenta, and in lung. We analyzed its co-expression pattern using the Guilt-by-Association (GBA) algorithm, and found that it is most similar in expression to early genes in the classical complement system (C1qA, C1qB, C1qC, C1r, and C1 inhibitor), MHC class II genes (HLA-DR alpha, HLA-DR beta 1, and HLA-DP alpha 1), Fc receptors (Fc gamma RIIa and Fc epsilon R1), lysosomal protein (LAPTm5), tissue transglutaminase, and macrophage receptors (MARCO and CD163/M130). The sequence and expression data suggest that Z39Ig is a cell surface receptor, expressed in activated macrophages, and linked with the classical complement system, most likely in phagocytosis preceding antigen presentation. Knowledge of this gene may contribute to better understanding of the role of complement and activated macrophages in rheumatoid arthritis and systemic lupus.
