The erythrocyte sedimentation rate (ESR) is a time-honored blood test, which assesses the degree of erythrocyte aggregation by acute phase proteins such as fibrinogen and immunoglobulins. Various intrinsic factors may influence the ESR, among them polycytemia, microcytosis or fibrinogen consumption lead to a decreased ESR, while anemia, macrocytosis or hypoalbuminemia lead to an increased ESR. The ESR still is a very valid test for the diagnosis of certain chronic diseases (polymyalgia, rheumatoid arthritis, multiple myeloma, septic arthritis and ostemyelitis) and the follow-up of certain chronic diseases (polymyalgia rheumatica, systemic lupus erythematodes, chronic infections, prostatic cancer, and Hodgkin's disease). In acute disease states and their monitoring C-reactive protein and eventually procalcitonin are the tests of choice. The ESR should not be used for screening and check-up examinations in asymptomatic patients.

Acute inflammation is the result of a complex signal transduction pathway that protects and heals our body and is necessary for our good health and normal wellbeing. Whereas, chronic inflammation can be correlated well with the onset of a plethora of autoimmune disorders; rheumatoid arthritis, systemic lupus and polymyalgia, rheumatic and other diseases like asthma, inflammatory bowel diseases, cardiovascular disorders, ulcerative colitis and Crohn's disease. Also, it has been reported to be associated with the onset of various cancers. An effective anti-inflammatory drug should be able to inhibit the development of chronic inflammation without interfering in normal homeostasis. A number of herbal drugs have been identified in the past that can target inflammatory cytokines. Among these, Ganoderma lucidum: a powerful medicinal mushroom has been found to possess immune-modulating and immune-potentiating capabilities and has been characterized as a wonder herb. This review mainly focuses on the molecular mechanism of anti-inflammatory and antiallergic action of this mushroom and also sheds light on various patent studies related to its pharmacological action.

The relationship between primary hematologic disease and rheumatologic manifestations is well known, especially acute lymphocytic leukemia, lymphomas, plasma cell dyscrasias and myelodysplastic syndrome (MDS). Currently, more has been described about autoimmune manifestations in chronic myelomonocytic leukemia (CMML). Many different clinical scenarios may lead a patient with MDS/CMML initially to seek a rheumatological unit. Autoimmune features such as polymyalgia rheumatic symptoms, myositis, neutrophilic dermatosis, cutaneous vasculitis and positive antinuclear antibodies (ANA) are some examples of clinical presentation of MDS/CMML. Moreover, peripheral cytopenias are a common initial presentation both for systemic lupus erythematous (SLE) and MDS/CMML. The aim of this study was to describe a case of an elderly woman with thrombocytopenia and positivity of antibodies to anti-extractable nuclear antigens (anti-ENA) as initial manifestation of CMML mimicking SLE, and to present some clues that encourage the clinician to perform a bone marrow study in such a clinical scenario.
