Acceptability of Bacille Calmette-Guerin (BCG) vaccine has tended to reflect many factors unrelated to the real effects of the vaccine itself. In particular, the risk of tuberculosis in a given country at a given time has always been a more significant factor than the incidence of complications associated with the vaccine. A possible protective effect of BCG on the occurrence of
acute leukemia
has also led to the intensification of BCG vaccination among young children entirely on the initiative of physicians. The public is more concerned with anomalous local lesions at the site of intradermal vaccination than by real complications such as fatal dissemination, osteitis, and
lupus
. As a result, many physicians feel pressured by their patients to replace the intradermal route of administration with scarification and to replace strong strains with weaker strains of vaccine. It is of major importance, then, that public health authorities are correctly informed, with a clear understanding of the balance between the use of an active strain with some side effects but providing good immunity and the use of a weaker strain with fewer side-effects but with a lower level of protection. When tuberculosis is still an important public health problem in a country, relatively innocuous side-effects should be accepted. On the other hand, if tuberculosis is no longer a significant threat, efforts should be made to avoid any annoying consequences of BCG vaccination, no matter how minimal they may be.

The significance of cold lymphocytotoxins, observed at 15 degreesC, is not clearly understood at the present time. The frequency of their appearance has been studied in normal subjects (blood donors, aged people, vaccinated subjects, post-traumatic splenectomy) and in patients with a neurologic disease (multiple sclerosis), a neoplasic disease (breast cancer)and hematologic diseases (thrombocytopenia,
acute leukemia
, chronic lymphatic leukemia, Hodgkin disease and systemic lupus erythematosus). There are no antibodies found in the geriatric group; they are found only in 3,9 % of blood donors and in 18 % of the subjects after vaccinations. A range of 17 to 30 % is found in subjects with breast cancer or multiple sclerosis. More than 50 % of the individuals with Hodgkin disease or
lupus erythematosus
produce these antibodies (52 % and 73 % respectively). In acute leukemias and chronic lymphatic leukemias, lymphocytotoxic antibodies sometimes appear at 37 degrees, reacting with autologous cells and having no HL-A specificity.

The one-direction radial diffusion method has been used in studies of the blood sera from 75 subjects and the kappa and lambda immunoglobulin types levels have been determined. The kappa:lambda (K:L) index was also calculated. The index values in sera of 20 healthy subjects remained in the range from 1.58:1 to 2.14:1 (mean: 1.86:1). The normal index values have been stated in 5/5 sera of patients with bacterial pneumonia. In 12/12 sera from patients with plasma cell myeloma the significant abnormalities of the index values have been noted. Abnormal index values have also been stated in 9/10 patients with
acute leukemia
, 4/9 patients with chronic lymphocytic leukemia, 7/8 patients with
lupus erythematosus
and 4/4 patients with autoimmune hemolytic anemia. The quantitative determination of particular immunoglobulin types may be useful in the diagnostics of the immunoglobulin types may be useful in the diagnostics of the immunoglobulin monoclonal synthesis. The abnormalities of the K:L index observed in patients with leukemias and autoimmune disease suggest that these diseases are frequently characterized by the monoclonal synthesis of immunoglobulins.

Bone marrow necrosis (BMN), defined morphologically by destruction of hematopoietic tissue, including the stroma, with preservation of the bone, is a rare syndrome. The conditions in which it is seen include sickle cell disease,
acute leukemia
, metastatic neoplasia, and bacterial infection, particularly when hypovolemia and septic shock are present. BMN is also associated with disseminated intravascular coagulation (DIC) following irradiation and antineoplastic therapy. The antiphospolipid syndrome (APS) is characterized by antibodies directed against the antiphospolipid substrate. Because this substrate is prominently involved in the coagulation cascade and widely distributed on cell walls, patients present with venous or arterial thromboses, recurrent abortion, thrombocytopenia, and Coombs' positive hemolytic anemia, typically with raised anticardiolipin antibodies or a diagnostic
lupus
anticoagulant test. BMN does not appear to have been previously recognized in this context. We report what we believe to be the first such case and suggest that the high titers of antibodies present may have played a central role in its pathogenesis.

Glucosteroids (GS) are widely used drugs for various inflammatory pathologies (Nephrotic syndrome, Proliferative glomerulonephritis, Extramembrane glomerulonephritis, Nephropathy of the Nodous Poliarterita (PAN), Nephropathy from purple Henoch-Schonlein,
lupus
nephropathy (LN), Acute adrenal insufficiency Waterhouse-Friederichsen, Chronic adrenal insufficiency Addison, Systemic Lupus Erythematosus (SLE), Polymyositis and dermatomyositis, Chronic granulomatosis, Crohn's disease, Hemorrhagic rectocolitis, Hemolytic anemias,
Acute leukemias
and chronic lymphocytic leukemia, Hodgkin's lymphoma). Although they are prescribed for their anti-inflammatory and immunosuppressive properties, they also have many side effects, hyperglycemia being one of the most common and representative, which is why these drugs need careful monitoring when administered over the long term. This paper presents the case of a 39 year old patient diagnosed with systemic lupus erythematosus (SLE) with class IV
lupus
nephropathy (LN) who developed numerous complications due to the pathogenic side effects: diabetes, amenorrhea, recurrent infections, and depression.

Background:
The beneficial effects of exercise training on depressive symptoms are well-established. In the past years, more research attention has been drawn to the specific effects of exercise training on depressive symptoms in somatically ill patients. This reviews aims at providing a comprehensive overview of the current findings and evidence of exercise interventions in somatic disorders to improve depressive symptoms.
Methods:
We systematically searched PubMed and Cochrane databases and extracted meta-analyses from somatically ill patients that underwent exercise interventions and provided information about the outcome of depressive symptoms.
Results:
Of the 4123 detected publications, 39 were selected for final analysis. Various diseases were included (breast-cancer, prostate cancer, mixed-cancer, cardiovascular disease, coronary heart disease, hemodialysis, fibromyalgia syndrome,
acute leukemia
, other hematological malignancies, heart failure, HIV, multiple sclerosis, mixed neurological disorders, Parkinson's disease, stroke, ankylosing spondylitis, traumatic brain injury,
lupus
erythematodes). Most meta-analyses (33/39) found beneficial effects on depressive symptoms, but quality of the included studies as well as duration, intensity, frequency, and type of exercise varied widely.
Conclusion:
Exercise training has the potential to improve depressive symptoms in patients with somatic disorders. For specific training recommendations, more high quality studies with structured exercise programs and better comparability are needed.
