Three patients with systemic lupus erythematosus (SLE) associated with anti-phospholipid antibodies are reported. All three had severe vaso-occlusive disease: the first had an unilateral vaso-occlusive retinopathy, the second an unilateral central retinal venous obstruction, and the third a bilateral ischemic choroidopathy. The association of these occlusive ocular vascular diseases with the presence of the
lupus
anticoagulant and other associated factors in SLE is discussed.

Two patients with retinal vascular disease also had antiphospholipid antibodies, defined by the presence of the circulating
lupus
anticoagulant (LAC), anticardiolipin antibodies, or both. One had retinal arterial occlusions and the other had a nonischemic central retinal vein occlusion. The association of thrombotic vascular disease, including both retinal and cerebral vessels, with the presence of these antibodies should be investigated in otherwise normal individuals or in patients presenting with a lupuslike syndrome and retinal vascular occlusive disease. Low-dose aspirin administration could be of therapeutic value in managing thrombotic events in this group of patients.

This report describes the use of pars plana vitrectomy (PPV) in the management of a patient with a serous retinal detachment (SRD) secondary to
lupus
choroidopathy. Surgery was performed after anti-inflammatory treatments and laser photocoagulation failed to resolve the condition. The patient's vision improved, and the subretinal fluid has not reaccumulated at 5 years postoperatively. The high viscosity of the subretinal fluid observed during the procedure may account for the persistence of the SRD despite control of inflammation. PPV surgery can be considered as a treatment option for patients with
lupus
-related SRD when anti-inflammatory medications are not successful and the subretinal fluid does not reabsorb independently.
