Human endothelial cells (EC) cultured from umbilical cord veins were reacted with sera from patients with systemic
lupus
erythematous (SLE), rheumatoid arthritis (RA) and normal persons. Immunofluorescent staining for IgG, IgM and IgA was then performed. Two types of control cells were also used, human foreskin fibroblasts and KB cells (a human
carcinoma
cell line). Sera from 9 of the 18 (50%) SLE patients showed cytoplasmic staining of EC for IgG. KB cells and fibroblasts did not stain. None of the RA or normal sera showed positive staining. In the 9 instances in which there was positive staining, 4 had evidence of cutaneous vasculitis. In contrast, none of the 9 patients without EC cytoplasmic staining had clinical evidence of cutaneous vasculitis. This data suggests that some SLE patients have an IgG antibody that reacts with EC cytoplasm which may be related to cutaneous vasculitis.

A modification of the microcytotoxicity assay of Terasaki & McClelland was used to screen for serum inhibitors of myeloid progenitor cells (CFU-C). Sera from 104 patients with neutropenia or bone marrow failure and from 104 controls, including 30 normal subjects and 74 patients with various disorders or multiple transfusions, were studied. Inhibitors of CFU-C were found in 19 of the 104 patients with neutropenia or marrow failure: three with acquited neutropenia and 16 with aplastic anaemia. In 17 of the 19 patients the inhibitor was either complement dependent or chavincingly associated with clinical remission of disease in one patient. In another patient, the inhibitor was an IgG antibody directed against non-HL1 antigens present on CFU-C of all 50 normal target marrows tested; however, it was not clearly autoreactive. Inhibitors were also found in one of nine patients with
lupus erythematosus
, one with breast
carcinoma
, one patient with renal transplant rejection, and six of 46 multiply transfused patients without marrow failure. Our findings indicate that most serum inhibitors are associated with multiple transfusion: convincing evidence of pathogenetically important autoreactive antibody was present in only one of 19 patients with inhibitors.

The main task of the department was to work out new approaches in estimation of the state of immune and blood system at the tissue, cell and molecular levels, using biochemical, biophysical and molecular biology techniques. Changes were primarily found in malignant skin lesions, but they are identical in cases of keratosis senilis as a premalignant lesion. However, concerning nonmalignant lesions, depending on exterior factors, long time influence of noxa, collagen fibers and their degree of degradation have an important role in the cell interaction, and more attention should be paid to exploring decomposition of collagen fibers as components of the local immunobiologic reaction of the organism to lesion formation.

The association of systemic lupus erythematosus (SLE) and malignancy has been reported previously and suggests an increased risk of cancer in this disease. Lymphomas are the most frequent neoplasias reported in these patients and carcinoma of the cervix and breast are also seen. Several factors probably play a role in the aetiology of malignancies associated with SLE including intrinsic disturbances of immunity and concomitant immunosuppressive therapy. We report five solid tumors (one breast
carcinoma
, one squamous cell carcinoma of the anus, one adenocarcinoma of the rectum, one carcinoma of the cervix and one
carcinoma
of the gallbladder) among 96 patients with SLE. The most striking finding in this study was the absence of haematological malignancies. The incidence of malignancy in the series, the age of diagnosis of SLE and neoplasia and the time delay between SLE and malignancy diagnosis was similar to other series. We did not find any clinical or immunological feature that predicted the development of neoplasia. In conclusion, patients with SLE may have the same malignancies as the general population after adjustment for age and sex. There are no predictive indicators for malignancy and immunosuppressive therapy may be a contributing factor.

Due to the severity of these lesions, a biopsy was performed, which implicated panniculitis associated with pancreatic disease, rather than

panniculitis. Serum amylase was normal, but the serum lipase was markedly elevated. An abdominal computerized tomographic scan demonstrated a pancreatic mass, which upon laparotomy was found to be an acinar cell
carcinoma
. After resection of the mass, her symptoms improved, where they had not responded to prior immunosuppressive therapy. She has subsequently remained well without recurrence of the disease. This case illustrates that an uncharacteristic presentation of panniculitis in a patient with
lupus
does not necessarily imply
lupus
panniculitis, and a biopsy is imperative to distinguish other sometimes life threatening etiologies.
