We report on a case of ureteral injury during laparoscopically radical hysterectomy to treat a patient with a stage
IA2
carcinoma of the uterine cervix. The advantage of laparoscopically radical hysterectomy is the prevention of the identification and dissecting of the
ureter
from a vaginal approach, the main problem in the Schauta technique. However, ureteral injury may still occur while resecting the cardinal ligament without good visualization of the
ureter
during a vaginal procedure. Because the
ureter
was well identified and freed laparoscopically, a vaginal approach after uterine removal may be an option for treatment. Moreover, using the ureteral stent as a marker during a modified Schauta technique is helpful in the prevention of ureteral injury during laparoscopically assisted radical vaginal hysterectomy.

Eleven patients with stage
IA2
-IIA carcinoma of the cervix have been treated by combined laparoscopic-vaginal radical hysterectomy and bilateral pelvic lymphadenectomy (3-Stage
IA2
, 5-stage IB, 3-Stage IIA). The patients were unselected. Three patients had bulky (>e; 5 cms) tumors, one of whom weighed 239 lbs; one had prior anterior-posterior repair, was apareunic and had significant vaginal narrowing; two patients had extensive pelvic adhesions, one of whom also had a 480 gram uterus. Pelvic lymph node metastases were present in one patient and paracervical lymph node metastases in one. The technique used has undergone significant modification. The laparoscopic phase of the procedure contributes much more to the operation than the lymphadenectomy for it allows a symbiotic partitioning of the operation into the laparoscopic and vaginal components. Only those steps of the operation are carried out vaginally that are easier to perform from below (division of the uterosacral and cardinal ligaments, unroofing of the
ureter
), and they are made much easier by the preceding laparoscopic phase of the operation. Laparoscopic development of the para-vesical and para-rectal spaces makes vaginal entry into these spaces very straightforward, and laparoscopic division of the uterine artery facilitates vaginal unroofing of the
ureter
. By allowing the proximal
ureter
to be freed from the medial leaf of the broad ligament, and the proximal attachments and blood supply of the uterus to be divided, the laparoscopic phase of the operation also permits the cervical ligaments to be divided before the ureters are freed from the vesico-cervical ligament, which helps to avoid a Schuchardt incision in most patients.

One part of the operative procedure of radical hysterectomy (RH) is the dissection of the
ureter
from its overlying tissue and this may result in injury to the ureteric adventitia. This might induce ureteric obstruction and consequently produce hydronephrosis. The objective of this prospective study was to evaluate the incidence of hydronephrosis after RH in patients with early stage cervical cancer. From July 2006 through March 2007, 77 patients with
IA2
-IIA cervical cancer who planned to undergo radical hysterectomy and pelvic lymphadenectomy (RHPL) received urinary tract ultrasonography 5 times (one day before surgery and 7 days, 6 weeks, 3 months and 6 months after the operation) from one radiologist. Patients who had hydronephrosis before surgery, suffered intraoperative ureteric injury, or were lost follow-up at 7 days after surgery were excluded from the study. Urinary tract ultrasonography was performed on 77, 55, 52 and 52 patients at each visit. Right hydronephrosis was detected in 16, 7, 5 and 3 patients, and left hydronephrosis in 16, 11, 3 and 1 , at 7 days, 6 weeks, 3 months and 6 months, respectively, after the operation. Hydronephrosis persisted in 8 patients (15%) after 3 months. Two of these had undergone exploratory laparotomy for lysis of ureteral adhesions. One patient who developed hydronephrosis had local recurrence and received further treatment with concurrent chemoradiation therapy. In conclusion, the incidence of persistent hydronephrosis over 3 months after RHPL was 15%, even without intra-operative ureteric injury. However, only a few cases required surgical intervention.

The aim of this study was to evaluate the clinical efficacy of a temporary ureteral catheter in preventing iatrogenic ureteral damage in cervical cancer patients undergoing laparoscopic radical hysterectomy. All cases had confirmed diagnoses of cervical cancer preoperatively between December 2008 and December 2012 in our hospital and were in clinical stages
IA2
to IIA. In total, 176 laparoscopic radical hysterectomy and lymphadenectomy procedures were performed. The 176 cases were divided into two groups: ureteral catheters were installed using cystoscopy before the operation in 86 patients (group A), and ureteral catheters were not placed in 90 patients (group B). These cases were retrospectively analyzed based on postoperative hospitalization time and intraoperative and postoperative complications. A total of 6 cases (3.41%) had ureteral injuries, and 4 of the cases (4.65%) of ureteral injuries occurred in group A. In two of these cases, urinary leaking appeared at the post-operative 8th and 9th days and at the 10th and 25th days, respectively. There were 2 cases (2.22%) of ureteral injuries in group B: 1 case of intraoperative direct injury and the other of urinary leaking, which appeared at post-operative day 21. Statistically significant differences between the two groups were observed in operating time and the incidence of hemorrhage, hematuria (including microscopic hematuria), post-operative urinary tract infection, and pain (P < 0.05). A ureteral catheter that is placed preoperatively can help to identify the
ureter
in laparoscopic radical hysterectomy, but does not decrease the incidence of ureteral injury.

Radical hysterectomy (RH) is an effective treatment for early-stage cervical cancer
IA2
to IIA1 but RH is often associated with several significant complications such as urinary, anorectal and sexual dysfunction due to pelvic nerve injuries. Pelvic autonomic nerves including the superior hypogastric plexus (SHP), hypogastric nerves (HN), pelvic splanchnic nerves (PSN), sacral splanchnic nerves (SSN), inferior hypogastric plexus (IHP) and efferent branches of the IHP. We aimed to precise the neuroanatomy of the female pelvis in order to provide key-points of surgical anatomy to improve NSRH for cervical cancer. The SHP could be injured during periaortic lymph node dissection and its preservation necessitates an approach on the right side of the aorta and a blunt dissection of the promontory before lomboaortic lymphadenectomy. Injuries to HN can occur during the resection of USL at the posterior pelvic wall and of rectovaginal ligaments and to preserve HN only the medial fibrous part of the uterosacral ligament should be resected. The middle rectal artery, the deep uterine vein and the
ureter
should be identified to preserve PSN and IHP during resection of paracervix. Vesical branches can be preserved by blunt dissection of the posterior layer of the vesicouterine ligament after identifying the inferior vesical vein.
