A 45-year-old Italian woman, who was admitted to the Royal Free Hospital in London, England, with a 14-day history of general malaise, lethargy, epigastric pain after meals, and night sweats, had had an IUD inserted 13 years earlier which had not been changed. The patient was pyrexial on examination; she had a temperature of 38 degrees Centigrade but no jaundice or enlarged lymph nodes. There was mild epigastric tenderness, and a tender indurated rectal stricture involving the posterior fornix of the vagina was palpable on pelvic examination. The rectal stricture was confirmed on sigmoidoscopy. The biopsy revealed a chronic inflammatory cell infiltrate with lymphocytes, extending from the submucosa through to the muscularis mucosae. A preoperative barium enema showed a long irregular rectal stricture. A large mass of inflammatory tissue was found adherent to the uterus, rectum, fallopian tubes, and ovaries at laparotomy. Bilateral retrograde ureterograms showed complete obstructions of the left ureter at 5 cm and a long irregular stricture was seen at the same level on the right. The histological examination revealed actinomycosis of the uterus, fallopian tubes, and ovaries. 12 weeks postoperatively the patient was well; sigmoidoscopy to 25 cm showed complete resolution of the rectal stricture. The antibiotic treatment was stopped. The most likely source of this patient's actinomycosis was the IUD for the relationship between the two is well established. In this patient a prolonged course of antibiotics proved effective in treating the infection.

60 patients with primary retroperitoneal tumor were reported. Among them, 26 cases were benign and 34 malignant. Abdominal mass was the most frequent presenting symptom (58.3%). The next was abdominal pain. The clinical course varied from 1 day to 22 years. Malignancy was predominant in patients with history less than one year. The size of tumor varied from 3 X 3 X 3 cm to 34 X 24 X 11 cm. Malignancy was more frequent in tumors larger than 10 cm in diameter. The location is decided mainly by displacement and compression of the gastrointestinal tract in barium meal examination and displacement or compression of the kidney or ureter and the presence of hydronephrosis in intravenous pyelography. The final diagnosis can only be established by pathological examination on celiotomy. All 60 patients were operated and the lesion of 43 was resectable. In the 20 with malignant tumor resected, only 8 patients are alive, while all the 26 patients with benign tumor are surviving, though 3 of them had unremovable lesions. The treatment is to resect the tumor as thorough as possible, including the adjacent invaded organs. Recurrence is likely for which resection should be reconsidered if possible. Radiotherapy or chemotherapy should be given to patients with lymphosarcoma, unremovable tumors and residual lesions after resection.

Experiments have been carried out with isolated ring preparations of human ureter. The tissue displayed spontaneous activity and contracted when exposed to barium chloride (0.5-4 mM) but no responses were obtained with carbachol (0.1 micromolar-0.1 mM). This raises questions about the value of treating ureteric colic with anti-muscarinic drugs.

The effect of tetracyclines on the isolated dog ureter is dependent on: (a) the tetracyclines used-mepicycline and doxycycline antagonizing, and tetracycline and rolitetracycline increasing the contractor action of barium chloride; (b) the percentage of 4-epiderivatives in the tetracyclines used-the higher the epiderivative concentration, the smaller the effect of mepicycline or doxycycline, and the greater the action of tetracycline or rolitetracycline.2. In vivo the addition of the antibiotics into the renal pelvis shows no significant differences between the various tetracyclines or different 4-epiderivative concentrations on the intra-ureteral flow of the dog or guinea-pig.3. Intravenous injection of mepicycline or doxycycline does not induce a significant change in the intra-ureteral flow, while intravenous administration of tetracycline or rolitetracycline produces a triphasic response: (a) a marked decrease of the intra-ureteral flow for a few minutes; (b) a return to the control condition for 30-60 min; and (c) a lesser but persistent decrease in flow for 60-120 minutes. In the first phase the ureteral smooth muscle is directly affected by the antibiotics circulating in the blood, while in the third phase the tetracyclines act via the intra-ureteral mucosa.4. Neurogenic effects on the ureter-bladder junction in vivo are not affected by the tetracyclines tested.

To determine the role of the autonomic nervous system and of cyclic AMP in the control of ureteral peristalsis, isolated guinea pig ureters were placed in a 20-ml organ bath containing modified Krebs solution. Theophylline induced significant dose-dependent reduction in frequency and amplitude of contractions of the ureter hypertonified with barium chloride. No change in frequency or amplitude of contractions was observed with isoproterenol. Besides, our data explain why beta-agonist drugs are of very little value in the treatment of renal colic due to spasms of the pelviureteral tract.

French research on Latimeria published before 1972 is first briefly recalled. After the Anglo-Franco-American Expedition of 1972, the material obtained enabled us to focus attention on the histology of the kidney and ureter and on ultrastructural aspects of the postnatal gland. Ovarian eggs of unusual volume have been collected, measured and weighed. Kinematic analysis of the intracranial articulation was carried out in 1973 on a frozen coelacanth that reached Paris. Studies in 1974 of a 41 cm specimen, the smallest known at that time and estimated to be 3 1/2 years old, showed that the brain still filled the cranial cavity: thus, it is later that allometric growth of the cranial cavity relegates the brain to the posterior part of the skull. In the same specimen, radiographed after injection with colloidal barium oxide, we were able to specify the likely role of the contractile organs fused to the branchial arteries: they may serve to regulate arterial pressure in the head during sudden movements of the jaws, and thus of the anterior cranium, as prey is captured. Lastly, the structural characteristics of the teeth, scale denticles, and spines on the lepidotrichia have been revealed, as also has the lamellar structure of the scales, in which there is a crossed orientation of the fibres in the different layers.

The development of colon carcinoma after ureterosigmoidostomy has not received adequate attention in the radiologic literature. Two patients who had ureterosigmoidostomy and subsequently developed tumors are described. The third case is a patient with a ureterosigmoidostomy and a ureterocele that simulated the appearance of carcinoma in the sigmoid colon. This is the first report of this entity. Ureterosigmoidostomy patients need frequent follow-up studies since their incidence of carcinoma is significant. The use of barium to study the colon in this group of patients is controversial. Barium can potentially reflux into the kidney with fecal material, which can lead to fibrosis and impaired renal function. However, barium reflux up the ureter is usually of no significance except on those rare occasions when intrarenal reflux occurs and inflammatory changes may develop. The diagnosis of tumors in these patients, diagnostic pitfalls, and controversy regarding use of barium enemas in following these patients are discussed.

This report presents three cases of colon tumors which developed after an ureterosigmoidostomy. In a checkup, four years after the operation, an inflamed polyp was found in one patient. The second patient developed an adenoma in the section of the colon where the ureter had been implanted. The last patient died of adenocarcinoma of the colon 26 years after her operation. Possible reasons for the development of the tumors, specifically the cancerous ones, are discussed. The risk of developing colon carcinoma is 500 times higher in those who have had an ureterosigmoidostomy than in healthy people. In the case that the ureters are rediverted the section of the colon where they were previously attached must be excised; since there is a strong possibility of cancerous development. To detect the early development of tumors in the colon, we suggest that patients have their stool tested for blood at regular intervals starting 3 years post operatively. If no problems arise barium enema and coloscopy are recommended every five years.

A 36-year-old woman presented with a palpable tender mass at the left lower quadrant of the abdomen. She had suffered from constipation for five years and had a previous history of intrauterine device-use for one year. Preoperative barium enema and abdominopelvic CT showed a compatible finding of rectosigmoid colon cancer or left ovary cancer. She underwent segmental resection of the sigmoid colon along with the removal of left distal ureter, left ovary and salpinx. Pathologic examination revealed actinomycotic abscesses containing sulfur granules. Thereafter, she took parenteral ampicillin (50mg/kg/day) for one month and oral amoxicillin (250mg, tid) for 2 months consecutively. The patient has no specific problems for 6 months after surgical resection and long-term antibiotic therapy. This report may be the first of intrauterine device-associated pelvic actinomycosis involving both sigmoid colon and rectum extensively.

Spontaneous ureterocolic fistula is rare and usually caused by urinary calculi. We present the fifth reported case of spontaneous ureterocolic fistula caused by diverticular disease of the colon. Review of these cases revealed a preponderance of women (3:1 ratio), with mean age of 77 years. These patients may have a protracted course before an accurate diagnosis is made (up to 10 years) because of the typical vague presentation. Urologic symptoms predominate, especially urinary tract infection (100%), fecaluria (75%), and abdominal (75%) or flank pain (50%). Barium enema is the most reliable diagnostic test in demonstrating the fistula (75%) compared with intravenous pyelogram (33%) or retrograde pyelogram (25%). The left ureter is usually involved (75%). Surgical intervention is generally directed towards resection of the diseased bowel with primary anastomosis when feasible. Surgical manipulation of the urinary system is unnecessary except for removing a non-functioning, infected kidney. Results of surgery were excellent, with 100 per cent cure and one unrelated mortality on long term follow-up. We recognize the potential for increase in this type of internal fistula, given the increasing lifespan and the established increase in incidence of colonic diverticular disease with advancing age. The correct diagnosis can often be determined preoperatively (75%), and surgical intervention is routinely successful.
