The function of 20 human autologous kidney grafts was studied for 4 to 8 years (mean 5 1/2) after autotransplantation with a direct pyelocystostomy. The indications for the operation were low-grade, low-stage tumour of the renal pelvis and/or ureter, frequently recurrent renal stones and remaining outflow obstruction after pyeloplasty for hydronephrosis. The wide pyelocystostomy implies reflux of urine from the urinary bladder to the renal pelvis but no significant changes were found in glomerular or tubular function, studied with 51Cr-EDTA clearance, 131I-Hippuran renography, analysis of beta 2-microglobulin and total protein excretion in urine, and determination of concentration ability after administration of desmopressin. No increase in the severity or frequency of urinary tract infections was observed.

Fifteen patients with acute unilateral obstruction of ureter and 30 control patients were studied with simultaneous 131I-Hippuran and 99mTc-DTPA gamma camera renography. In severe outflow obstruction the relative function of the obstructed kidney was higher when calculated with Hippuran than with DTPA. No difference was found in the control subjects. This means that the glomerular filtration rate of an obstructed kidney may be relatively more reduced than the renal blood flow. Therefore, measurement of the single kidney fraction of total glomerular filtration rate has to be calculated with a glomerular filtered tracer, e.g., 99mTc-DTPA.

The function of the ileo-ureter and the operated kidney was investigated by means of the camera functional scintigraphy in 23 dogs operated under different techniques. This non-invasive examination was carried out with two different radio-remedies (131I-Hippuran and 99mTc-DTPA) by a gamma camera microprocessor. The direct single-layered ileo-ureteric anastomosis showed the best urine transportation. This method has a great practical importance in the opinions of the authors.

The distribution and transport of Hippuran is discussed. The activity leaving both kidneys has been calculated and compared with the bladder activity. It is shown that the Impulse Response Function of the ureter and bladder may be obtained by deconvolution, and data are presented relating to studies on one patient.

Alterations on the upper urinary-tract and on the kidney have been investigated by histology and scanning-electron-microscopy (SEM) in an animal study on the rat. Examinations have been performed 7, 14, 28 and 56 days after double-ligating the ureter. The documentation shows the developed alterations which are remarkable in the area of the papilla and medulla of the kidney, whereas the glomerula are nearly unchanged in comparison with the controls. At the same time function-tests with radioactively labelled Jodium-Hippuran and technetium-DTPA have been performed. The nephrograms in the region of interest technique showed after 14 days isosthenuria and after 4 weeks no signs of any function on the operated side.

Renal, ureteral, and vesicle functions were evaluated in 209 patients with obstructive lesions of the ureter due to schistosomiasis haematobia (bilharziasis), using Hippuran I131 extended renography. High values for Tmax. and T 1/2 were noted for both kidneys irrespective of radiological diagnosis of a left- or right-sided lesion. A high percentage of residual vesicle urine was demonstrated in a number of patients with bilharzial uro-obstructive lesions. The incidence of vesicoureteral reflux in relation to different lesions was also correlated. The results of this study support the concept that urinary schistosomiasis is a progressive disease which affects mainly the urinary bladder and lower ureter. The data obtained demonstrate the efficacy of extended renography in assessing renal, ureteral, and vesical function in patients with bilharziasis.
