-year-old man presented with left hydronephrosis 1 year after aortofemoral bypass. Hydronephrosis was due to extrinsic compression of the

between the graft anteriorly and the native iliac artery. Treatment by endoscopic transluminal balloon dilation resulted in complete resolution of the hydronephrosis.

A routine chest roentgenograms showed a sharply outlined mass in the postero-medial portion of the left lung field. Computed tomography demonstrated left intrathoracic kidney with anomalies of rotation. Intravenous pyelography demonstrated a left high ectopic kidney with complete duplication of the renal pelvis and incomplete duplication of a
ureter
. The patient was diagnosed as left intrathoracic kidney with renal anomalies. The 79 cases of intrathoracic kidney reported in Japan were analyzed concerning their etiology.

-year-old woman who had an abdominal mass was admitted to our hospital in May, 1983. Physical examination revealed the presence of a soft mass without tenderness in the right upper abdomen. The results of laboratory tests were within the normal range. An excretory urogram showed a slight lateral displacement of the upper position of the right
ureter
and a slight dilatation of the right pyelocalyceal system. The urogram for the left kidney and
ureter
was normal. Abdominal CT scan revealed a cystic homogeneous mass anterior to the right kidney, 12 X 11 cm in size. Ultrasonography showed a cystic mass corresponding to the CT scan finding. Under ultrasonographic guidance, percutaneous puncture of the cyst was performed, and 800 ml clear straw-yellow fluid containing 457 ng/ml alpha-fetoprotein was drained. A repeat CT scan 9 months later showed no evidence of fluid reaccumulation and the patient remained asymptomatic.

-year-old female presented with left back pain at our hospital. She had had the same symptom about 1 year previously, but she had been presumed to have undergone stone passage because her symptom had disappeared. At this time a urogram, either excretory or retrograde, showed narrowing of each caliceal infundibulum and dilatation of each calyx in the left kidney, but otherwise normal findings. A cytology of left ureteral urine was class V, and cystoscopy revealed no abnormality. Under the diagnosis of left renal pelvic tumor she underwent nephroureterectomy with resection of a bladder cuff and retroperitoneal lymphadenectomy. The resected specimen had no gross tumor throughout the renal pelvis and
ureter
, but histological examination revealed transitional cell carcinoma in situ (grade 2) in most of the renal pelvis and infiltration of inflammatory cells in the submucosa. The

did not have any cancerous lesion, and no lymph node metastases was found. Four months postoperatively she is thought to have no evidence of disease with negative urinary cytology.

. Computerized tomographic (CT) scan demonstrated a tiny lesion in the right
ureter
. Under a tentative diagnosis of right ureteral tumor, flexible fiberoptic ureteroscopy was performed. As a pedunculated tumor with a smooth surface was found, biopsy was performed. Since pathological examination showed malignancy, right nephroureterectomy was performed. Histological diagnosis was transitional cell carcinoma, grade 1. Cancer was covered by normal transitional epithelium, and developed inverted growth. This is a rare case of inverted growth of the transitional cell carcinoma in the

-year-old female visited us for asymptomatic macroscopic hematuria. Cystoscopy revealed a papillary pedunculated tumor in the bladder protruding from the right ureteral orifice. Under a diagnosis of superficial tumor at the right ureteral end, transurethral resection was performed using a rigid ureteral resectoscope. Pathological examination revealed transitional cell carcinoma, grade 2 without muscular invasion. There is no evidence of recurrence after 25 months of follow up postoperatively. Case 2: On a routine cystoscopy after transurethral resection of bladder tumor in a 68-year-old male, papillary pedunculated tumors protruding from the right ureteral orifice were found. Under a diagnosis of superficial tumor at the right lower
ureter
, transurethral resection was performed using cold cup forceps and loop electrocoagulation. The tumor was transitional cell carcinoma, grade 1 without invasion. No evidence of recurrence was detected 15 months postoperatively. Transurethral resection of the ureteral tumor at the lower end appeared to be useful for the selected cases. The indications, complications and follow up for this procedure were discussed.

-year-old man who had received radical nephrectomy for left renal cell carcinoma, G1 and about two years earlier presented with asymptomatic macrohematuria. He had received interferon therapy and surgical treatments for bone metastasis two times after the operation. Cystoscopic examination revealed bleeding from the left residual
ureter
but CT scan showed no abnormal findings. Left ureterectomy and partial cystectomy was performed and a small finger tip-sized tumor was found at 13.5 cm above the ureteral orifice. Pathological examination showed metastatic renal cell carcinoma, G1 > G2. Histologically and clinically, this tumor seemed to have metastasized by a hematological pathway. Seventeen cases of ureteral metastasis of renal cell carcinoma have been reported previously in the Japanese literature.

A case of pyonephrosis with high levels of serum CA19-9 antigen is reported.

-year-old woman was admitted with right flank pain. Computed tomography and ultrasonography showed severe hydronephrosis and hydroureter due to a right
ureter
stone. Laboratory data revealed a high level of serum CA19-9. However, no tumor was found in the pancreas, gallbladder, liver, gastrointenstinal tract or genitourinary tract. Drip infusion pyelography showed a non-functioning pattern of right kidney. Therefore, right nephroureterectomy was performed for right pyonephrosis. Histological examination revealed chronic inflammation. Malignant cells were not seen in the resected specimen. The serum CA19-9 levels before and after operation were 102.9 U/ml and 24 U/ml, respectively, being normal after the operation. Immunohistochemical examination revealed the presence of CA19-9 antigen in the urethelium, indicating its expression in the specimen. To our knowledge this might be the first case of pyonephrosis associated with high levels of serum CA19-9 antigen.

-year-old man was admitted to our hospital with the chief complaint of macroscopic hematuria. IVP showed the dilation of left

and renal pelvis. Retrograde pyelography showed a 2cm filling defect in the right middle
ureter
. From the data given, a right ureteral tumor was suspected, and we performed tumor biopsy under an ureteral scope. Since it was diagnosed not as transitional cell carcinoma (TCC) of
ureter
but as ML with strong inflammation, partial ureterectomy and uretero-uretero anastomosis were done. Histological diagnosis was B cell follicular center lymphoma. Because Ga-scintigraphy revealed no evidence of other involement, it was diagnosed as ML originating from
ureter
. ML secondarily invading the urinary tract organs is commonly found, but ML originating from the urinary tract is rare. Especially, in regards to ML originating from
ureter
, only 16 cases have been reported in Japan.
