Changes in the visualization of non-ionic contrast medium (
Omnipaque
300) in the urography by DIP were monitored for the normal sided urinary tract in patients with urolithiasis using a densitometer. In addition, changes of the visualization in relation to the dose of contrast medium used was examined and compared with that of ionic contrast medium (60% Urografin). The optimum photographing time was 15 minutes in the upper urinary tract (nephrogram, calyx, pelvis, upper

) and 20 minutes in the lower urinary tract (lower
ureter
, urinary bladder). Visualization of high usefulness appeared to be obtainable for the imaging diagnosis by DIP in subjects with normal renal function even when the photographing was completed 20 minutes after infusion of contrast medium. In relation to the doses of contrast medium used, no difference was observed in the variation pattern but a better imaging was obtained in the 100 ml group than 50 ml group with a significant difference (P less than 0.01) in the 15-20 minute images of the calyx and pelvis of the kidney and urinary bladder in particular, this suggested the high usefulness of 100 ml dosing for the imaging diagnosis. In the visualization of ionic contrast medium, some difference was observed in the variation pattern and the visualization was better in Ominpaque 300 groups than in 60% Urografin group with a significant difference (P less than 0.01) in the 15-20 minute images of the calyx and pelvis of the kidney and urinary bladder. This suggested the high usefulness of non-ionic contrast medium.

Iohexol
and 99mTc-DTPA were used in 43 patients to determine the relative glomerular filtration rate (GFR), i.e., the GFR of each kidney in percent of total GFR. The amount of any GFR marker accumulating in Bowman's space, tubuli and renal pelvis within a few minutes after i.v. injection, before any marker had left the kidney via the
ureter
, was defined as proportional to the GFR of that kidney. The renal accumulation of iohexol was determined by CT using 10 slices of 8-mm thickness 1 to 4 minutes after injection. The renal accumulation of 99mTc-DTPA was determined with a gamma camera within 2 minutes after injection. The correlation coefficient between the two methods was 0.98. Due to the higher radiation dose from CT than from 99mTc-DTPA injection, relative GFR determination with CT should be performed when there is also a diagnostic need to reveal morphology.

This study was performed to validate the procedure of transarterial embolization of the renal artery (TAE-RA) using iohexol-ethanol solution in rabbits with unilateral experimental hydronephrosis and to evaluate the embolized kidney and contralateral normal kidney using B-mode ultrasonography and color Doppler ultrasonography. Experimental hydronephrosis was induced at 17 days after ligation of unilateral

in 13 rabbits. Renal artery embolization was performed using selective catheterization in the hydronephrotic kidney of eight rabbits and electrocardiography, oxygen saturation, body temperature, pulse, and respiratory rate were within normal ranges during procedures.
Iohexol
-ethanol solution was used as embolic material. Average ethanol dose for renal artery embolization was 1.4 +/- 0.7 ml/kg. There were no rabbits expired after TAE-RA and no side effects associated with regurgitation of iohexol-ethanol solution. In color Doppler ultrasonographic findings, there was no blood flow into the embolized kidneys treated by TAE-RA, however, blood flow signal was found in hydronephrotic kidney not treated by TAE-RA. Ultrasonographically, the mean longitudinal length of the embolized kidney significantly decreased at 2 and 3 months after TAE-RA. No significant difference of resistive index values was found between contralateral normal kidney of rabbits treated by TAE-RA and contralateral normal kidneys of rabbits treated with nephrectomy. We may conclude that TAE-RA with iohexol-ethanol solution is a viable alternative to nephrectomy in rabbits with unilateral hydronephrosis.
