Loin pain may be a major presenting symptom in patients with glomerulonephritis. Most of these patients show an underlying focal and segmental proliferative glomeruloneyphritis and there may be associated deposits of IgA and Igg in the mesangium. In this group of patients, vascular lesions are often prominent in the absence of hypertension. Episodes of recurrent macroscopic hematuria also occur, but the pain cannot be attributed to
colic
due to blood clots in the
ureter
.

Few urologic problems require the consideration of so many factors as does an obstructing calculus in the
ureter
. The questions that arise in each case are whether an operation should be done for immediate relief of the obstruction or whether manipulation should be done, possibly subjecting the patient to a period of disability, painful attacks of
colic
and febrile reactions.

In a 23-year-old man attacks of nephritic
colic
led to the discovery of an obstruction on the left lumbar
ureter
. Segmental resection of the
ureter
was performed, removing 10 mm of malformed, obstructed
ureter
. This was an incomplete duplication, the two ureteral segments lying side-by-side, each with its own musculature, for a distance of 7mm. Above and below the anomaly, the
ureter
was normal. This exceptional malformation is compared with other internal obstructions of the
ureter
.

Endoluminal sonography using the IVUS is a minimally invasive procedure performed using a flexible 6.2F catheter with a 20 megahertz transducer that can easily be passed endoscopically to image the urethra, bladder,
ureter
and renal pelvis. In the current study, a woman patient in her third trimester of pregnancy with a renal pelvic stone and persistent
colic
was safely treated with placement of an indwelling ureteral stent. Because it is minimally invasive and does not require the use of roentgenograms, we believe the IVUS will be a useful tool to assist with the management of ureteral
colic
in patients who are pregnant in whom intervention is required.

Medical records of 68 horses with urolithiasis were examined. Calculi were in the bladder in 47 horses, urethra in 11 horses, kidneys in 15 horses, and
ureter
in two horses. They occurred at several sites in six horses. Common clinical signs included hematuria, altered micturition (pollakiuria, dysuria, urinary incontinence), and tenesmus. Weight loss, possibly attributable to chronic renal failure and
colic
, was associated more commonly with renal and ureteral calculi. Weight loss also occurred in 13% of horses with cystic calculi only. In male horses, most cystic calculi were removed by perineal (ischial) urethrotomy under epidural anesthesia. Although there were few surgical complications with urethrotomy, seven of 15 horses with follow-up suffered recurrent urolithiasis.

Blind ending bifid
ureter
is a rare anomaly in the urinary tract. This anomaly may result from failure of a premature branch of the ureteral bud to join with the metanephric blastema. A 21-year-old man was admitted with macroscopic hematuria and
colic
pain in the left flank region. Urinalysis demonstrated hematopyuria and excretory urography suggested bifid ending accessory
ureter
with a stone on the left side. Surgical exploration showed that the accessory
ureter
was bifurcated from the left
ureter
at about 5 mm from the bladder wall and ran parallel with the left
ureter
. Although dense adhesions to the surrounding tissue existed, the accessory
ureter
was resected at the site of the junction. It measured 3 cm in length and 1 cm in greatest diameter. The stone found at the tip of the accessory
ureter
was composed of calcium oxalate (24%) and calcium phosphate (76%). Histological examination revealed that the
ureter
had all layers of normal ureteral structure and no renal tissue was identified in the specimen resected. During a follow-up period of 22 months after the operation, he was free of urinary tract infection and abdominal pain. Of 77 cases with blind ending bifid
ureter
reported in the Japanese literature, a ureteral stone was found in the blind branch in only 5 cases.

We describe the urological manifestations arising from hematoma of the left iliac psoas that developed in the course of anticoagulant therapy in a 54-year-old patient with a mitral prosthesis. The onset of the clinical picture was acute and mimicked left reno-ureteral
colic
. Ultrasound and CT were decisive in making the diagnosis, revealing a left retroperitoneal mass in the anterolateral aspect of the left psoas muscle. Intravenous urography evidenced the urological repercussions of the disease process consisting of medialization of the left
ureter
, ureterohydronephrosis, and anterior and lateral displacement of the kidney. Although this condition is considered by most to be a surgical emergency, we opted for conservative treatment because the patient had no hemodynamic instability, severe obstructive uropathy or signs or crural paralysis. The literature on the urological repercussions of psoas hematomas due to anticoagulant therapy is briefly reviewed.

Prostaglandin synthetase inhibitors are used for the treatment of ureteric
colic
. However, there is controversy regarding the mechanism of action of these drugs. In this study, differential prostaglandin synthesis in the human renal pelvis,
ureter
and bladder was measured using specific radioimmunoassays and gas chromatography/mass spectrometry. There was a significant quantitative predominance of the smooth muscle constrictor eicosanoids, PGF2 alpha and TXA2 over the dilatory PGE2 in tissue from all sites--renal pelvis,
ureter
and bladder. The results indicate that prostaglandins play a direct role in smooth muscle activity of the upper urinary tract and the inhibition of this activity with indomethacin indicates a further mode of its action in pain relief in ureteric
colic
.

We treated 38 consecutive outpatients with
ureter
colic
with a new anticholinergic spasmolytic agent, cimetropium bromide, in order to evaluate its efficacy in this condition. After assessing pain intensity by means of a visual analogic scale, we administered 5 mg i.v. Already after 30 minutes, 4 patients (11%) reported complete subsidence of pain. In 21 patients (55%) pain subsided almost completely within one hour. Eighteen patients required a second 5 mg i.v. injection after one hour; of these, 13 (72%) had complete regression or marked reduction of pain. Apart from dryness of the mouth which appeared in two cases after the second injection and was of moderate intensity, no side effects were observed.

Two cases of unilateral ureteral diverticulum are reported, one of which was associated with a bifid
ureter
. In the first case, this anomaly was incidentally discovered during a urographic work up of a patient with
colic
nephritis. In the second case, the presenting symptom was hematuria which was finally attributed to ureteral diverticulosis. A structural classification of these anomalous lesions is described, and the commonly associated conditions (obstruction, infection) are reviewed. The authors do not advocate treating these anomalies.
