We report here two new cases of ureteral
diverticulosis
. This condition is found usually in the man of the fifties who has most frequently an urinary tract infection or a prostatic adenoma. The roentgen findings are multiple out-poutchings of the lumbar

. Several factors are combined (constitutionnal, urinary tract infection, disordered ureteral-motility) to explain the creation of diverticules in the areas of weakness of ureteral wall, where penetrate the vessels.

The urogenital tract has been examined in 35 patients who required 3 or more years of therapy. A range of studies revealed a short urethral stricture in 11, long urethral stricture in 26, megalocystis in 6, domicile urinary bladder in 2 and bladder
diverticulosis
in 6 patients. Simple and diuretic excretory urography showed normal bilateral renal function in 5, moderately impaired function in 14 and severe loss of function in 6 patients. Single-kidney functional impairment was seen in 10 patients. Over 504 of patients showed prolonged urographic retention of urine in the pelvis and

. Bilateral ureterohydronephrosis was found in 3 and unilateral one in 3 patients. Descending, ascending and micturitional urethrocystography revealed urinary reflux into the prostate (n = 13), seminiferous ducts (n = 3), seminal vesicles (n = 4) and ureters (n = 4). Vasovesiculographic sizes and shapes of the seminal vesicles were normal in 2 patients while the vesicles were uni- or bilaterally dilated or constricted in other patients. The treatment was operative in 34 patients. Histological examination of scars and resected tissues showed fibrous sclerotic lesions in the prostate and suppurative inflammatory and fibrous lesions in seminal vesicular walls. With long treatment of urethral strictures, micturition disorders were superimposed by reflux of infected urine into the prostate, seminal vesicles and ureters, inducing inflammation and functional abnormalities; these caused shrinkage and compression of the posterior urethra, bladder cervix, intramural and prevesical ureteral segments, resulting in chronic renal failure. An early and radical plastic operation on the urethra may prevent the mentioned disorders.

. In the first case, this anomaly was incidentally discovered during a urographic work up of a patient with colic nephritis. In the second case, the presenting symptom was hematuria which was finally attributed to ureteral

. A structural classification of these anomalous lesions is described, and the commonly associated conditions (obstruction, infection) are reviewed. The authors do not advocate treating these anomalies.

Intrauterine operations were carried out in fetal sheep varying from 50-122 days of gestation. Silk, catgut and polyglycolicacid sutures were used to produce varying degrees of stenosis. The urachus was ligated with a silk suture in all cases. Urachal ligation alone produces no ill effects on the upper urinary tract. The addition of urethral stenosis results in dilatation of the upper tract depending on the degree of infravesical obstruction. Partial ligation of the urethra produces a pyelocalyectasis which may disappear later. More severe degrees of obstruction are followed by hydronephrosis, hydroureter and megacystis. Dilatation begins in the region of the renal pelvis and proceeds in the antegrade manner towards the bladder. Histologically the parenchyma was reduced in thickness and showed dilated tubules. In the
ureter
and the bladder muscular hypertrophy was noticed. Depending on the time of the experiment, there was an increase of connective tissue within the
ureter
and bladder. In cases of extreme obstruction the
ureter
showed muscular atrophy. No infection, ureteral reflux, bladder
diverticulosis
, extravesicalisation of the
ureter
or narrowing of the
ureter
by hypertrophied musculature was seen. These experiments may throw some light on the pathogenesis of the megaureter syndrome occurring in childhood.

Spontaneous ureterocolic fistula is rare and usually caused by urinary calculi. We present the fifth reported case of spontaneous ureterocolic fistula caused by
diverticular disease
of the colon. Review of these cases revealed a preponderance of women (3:1 ratio), with mean age of 77 years. These patients may have a protracted course before an accurate diagnosis is made (up to 10 years) because of the typical vague presentation. Urologic symptoms predominate, especially urinary tract infection (100%), fecaluria (75%), and abdominal (75%) or flank pain (50%). Barium enema is the most reliable diagnostic test in demonstrating the fistula (75%) compared with intravenous pyelogram (33%) or retrograde pyelogram (25%). The left
ureter
is usually involved (75%). Surgical intervention is generally directed towards resection of the diseased bowel with primary anastomosis when feasible. Surgical manipulation of the urinary system is unnecessary except for removing a non-functioning, infected kidney. Results of surgery were excellent, with 100 per cent cure and one unrelated mortality on long term follow-up. We recognize the potential for increase in this type of internal fistula, given the increasing lifespan and the established increase in incidence of colonic
diverticular disease
with advancing age. The correct diagnosis can often be determined preoperatively (75%), and surgical intervention is routinely successful.

Different reconstructive operations were performed in 20 patients for intraoperative traumas of the urinary tract. 4 patients had injured
ureter
and urinary bladder. The damage was done in the course of obstetric operations (cesarean section, uterine extirpation). In 12 cases the
ureter
was injured in uterine extirpation for cancer (n = 4), myoma (n = 4), prolapse of the uterus, (n = 1), extirpation of uterine cervix stump (n = 1), ureteral electrocoagulation (n = 1) and adnexectomy (n = 1). In 4 cases ligation of the
ureter
complicated surgical interventions for cancer of the sigmoid colon (n = 1) and rectum (n = 1),
diverticulosis
of the colon (n = 1) and portal cirrhosis of the liver with evident cirrhosis (n = 1). Surgical policy in the treatment of intraoperative urinary tract injuries was organ-saving. Only in 3 patients with severe acute pyelonephritis surgery was two-staged with prior nephrostomy. In the rest cases primary reconstructive operations were made. Two patients with bilateral injury of the ureters after uterine extirpation have undergone transabdominal bilateral reimplantation of the ureters by Boari in Gregoir's modification. ureter
was often made by using a urinary bladder graft (Boari's technique). In 1 female patient with extensive vesicovaginal fistula resultant in detruzor corrugation sigmocystoplasty was made with a good result. Serious complications after the reconstruction were absent. Urinary fistulas formed in 4 cases. In 3 of them they closed without surgical intervention. In 1 patient, to close urinary fistula complicating ureterocystoanastomosis Boari's operation was conducted with a favourable outcome. Reconstructive operations saved the kidney function.

Various reconstructive surgeries were performed in 21 patients because of intraoperative injuries of the urinary tracts. In 4 patients trauma of the

and urinary bladder was associated with obstetric operations (Cesarean section, hysterectomy). In 13 cases injury of the

was the consequence of hysterectomy due to cancer (4), hysteromyoma (4), prolapses of the uterus (1), extirpation of the stump of the uterine cervix (1), electrocoagulation of the

(2) and adnexectomy (1). In 4 patients ligation of the

complicated surgery for cancer of the sigmoid colon (1) and rectum (1),
diverticular disease
of the colon (1) and portal hepatic cirrhosis with severe ascites. Surgical policy was organ-saving. Only in 3 patients with severe acute pyelonephritis surgical treatment was performed in two stages with preliminary nephrostomy. In the rest cases primary reconstructive surgeries were used. Reconstructive surgeries saved the kidney as a functioning organ.

During the past 5000 years, ancient nomenclature and dogmas regarding the etiology of protrusions have accumulated. Whereas, in the abdomen, the Latin "hernia" supplanted Greek, it, based on content, persists in the pelvis as cystocele, rectocele, etc. Russell (Lancet 1:1519-1523, 1902) championed the congenital saccular theory of herniae, denying they could ever be acquired pathologically. and colon are rare. Most spontaneous ureterocolic fistulae are caused by urinary calculi. We report a case of a spontaneous ureterocolic fistula which occurred as a consequence of
diverticular disease
. This rare presentation was further complicated as it occurred in the presence of a solitary kidney. The patient underwent a laparoscopic defunctioning loop ileostomy and after 6 weeks underwent definitive surgical treatment of the ureterocolic fistula. We describe the presentation and management of this fistula and review the current literature.
