Upon the exposure to remote or combined radiotherapy, the patients with uterine cervical cancer often develop ureterostenosis due to recurrence or postradiation alterations in the walls and adjacent tissues of the
ureter
. Etiology of the urodynamic defect is essential for the treatment policy choice. Therefore, in attempt to find out effective means of differential diagnosis between ureteral obstruction due to cervical cancer relapse and due to postradiation lesion, the authors tried transperitoneal aspiration biopsy with thin needle as a tool of such diagnosis. The biopsy was obtained from 19 females. In 14 of them the obstruction was attributed to
recurrent cancer
, in 5 to radiotherapy. No errors in the diagnosis were made. Thus, the accuracy of the method reached 100%.

Two cases (spouses) of carcinoma of the ureter possibly induced by long-term use of analgesic were presented. Case 1: A 66-year-old female with hematuria was diagnosed as a carcinoma of the ureter in January, 1985 and underwent resection of the left kidney and the
ureter
with partial resection of the urinary bladder. In December, 1988 she died due to
recurrent cancer
. Case 2: A 70-year-old male (a husband of case 1 patient) with hematuria was diagnosed as a carcinoma of the ureter in August, 1987 and underwent resection of the kidney and the
ureter
of the left side. In October, 1988 he died due to
recurrent cancer
. These two patients had used routinely phenacetin because of persistent headache for about 20 and 15 years, respectively and the presumed total dose was amounted to 4 and 2.5 kg, respectively.

We report a case of renal hypertension 6 months after a panhysterectomy for cervical cancer. Clinical investigation revealed that
recurrent cancer
obstructed the left
ureter
, resulting in the formation of a gigantic perirenal pseudocyst and, subsequently, hypertension. Constriction of the renal parenchyma was responsible for the overactivity of the renin-angiotensin system (the Page phenomenon). Ultrasound-guided percutaneous drainage relieved the symptomatology completely.

During the period from June 1973 to August 1982, thirteen patients with gastric cancer and seven patients with colorectal cancer received surgical treatment due to the obstruction of the rectum or
ureter
. Of thirteen gastric cancer patients, 11 underwent the formation of the artificial anus for rectal obstruction. Nephrostomy was performed in 4 gastric cancer patients with ureteral obstruction. Seven patients with colorectal cancer underwent the formation of the artificial anus for rectal obstruction. In the
recurrent cancer
of the gastrointestinal tract, obstruction of the rectum or
ureter
mainly occurred due to peritoneal invasion or tumor formation. During laparotomy, OK-432, a Streptococcal preparation, was administered with a large dose of 100 K. E. to the patients with advanced or
recurrent cancer
foci. The prognosis for those patients was poor, and the main reason for poor survival was the advancement of disease in the cases of rectal or ureteral obstruction.

Recurrence of the disease represents the major problem in patients who undergo "curative" resection for rectal cancer, with published rate ranging from 3 to 50%. Most relapses occur within first two years of follow-up. Depending on the site of the recurrence, it can be local or distant. It also can be solitary or diffuse. In terms of potential surgical cure the best results are achieved with solitary, localized metastases. The most common sites of the solitary metastases are pelvis, liver and lung, with a fairly even distribution among these three sites. Other sites of the localized metastases can be peritoneum, lymph nodes, brain, bone, abdominal wall,
ureter
and kidney. These sites are less common, but not so amenable to resection. Local recurrence varies depending on the original type of surgery. It can be stated that surgical technique directly influences local recurrence rate in patients with rectal cancer. According to the results from a number of different authors 5-year survival rate after reresection is 2-13% of all patients with locally
recurrent cancer
, both alone and associated with distant metastases. The most important moment in this problem is to decide when not to operate. The absolute contraindications for salvage surgery are: "frozen pelvis", aneuploid tumors and those with mucinous component, clinical or CT evidence of invasion of the pelvic nerves, lymphatics or veins, or
ureter
bilaterally. Also, evidence of involvement of the lateral pelvic sidewalls and/or upper sacral marrow, and/or S2 is an absolute contraindication for surgery. Thus, main goals of this type of surgery are respectively: palliation of symptoms, a good quality of life and, if possible, cure with low treatment-related complication rates.
