Injuries to the
ureter
were found to occur primarily in cases of left-side
Dukes
C tumour, along with dissection of regional lymph nodes to cope with peritumorous infiltration. The area of an injury must be very carefully dissected. A splint should be inserted, when the ureteral lumen has been opened. A few approximation sutures should be made and the retroperitoneum drained. Anti-refluxive re-implantation and Psoas Hitch plasty are recommended to cope with situations in which distal parts of the
ureter
are missing (due to direct or indirect damage or late sequels). Percutaneous nephrostomy is considered to be the optional therapeutic approach to extravasation and engorgement of the kidney.

Villous adenoma is a common lesion of the gastrointestinal tract, but it is rare in the
ureter
. Thus, as far as we know, only one case limited to this location has been described. Intestinal metaplasia of the urothelium is not rare. However, only one case of gastric metaplasia with pseudopyloric glands has been described in the literature. We here report in detail on a tubulovillous adenoma of the
ureter
associated with diffuse gastric and intestinal metaplasia and a concurrent primary, solid, high grade transitional cell carcinoma, with extensive clear cell change, in a 56-year-old male patient. He had undergone a left nephrectomy for renal tuberculosis twenty years earlier, and the lesions developed in the ureteric stump. To the best of our knowledge, such a combination of lesions has not been reported previously either in the
ureter
or in the rest of the urinary tract. The coexistence of diverse lesions in our case might represent the pluripotentiality of the urothelium in association with chronic inflammation and neoplastic induction. The present report also emphasizes the metaplastic and malignant potential of a defunctioned urothelial structure. This case is of particular interest, because these coexistent lesions arose simultaneously with an anatomically separate adenocarcinoma of the rectum (
Dukes
' B). The patient died 76 days after admission. The dismal prognosis of our case was determined by the advanced anatomical stage and the histological high grade of the transitional cell carcinoma of the
ureter
.

Malakoplakia, characterized by histiocytes with Michaelis-Gutmann bodies, is a rare entity. It is particularly so in the gastrointestinal tract, where it has been described in association with colon cancer, with about 20 cases described worldwide. The significance of this condition lies in its potential effect upon the preoperative staging and treatment of associated colorectal cancer. Its presence may lead to preoperative clinical and radiological over staging and more extensive resection, as well as the use of neoadjuvant therapy or a decision to undertake palliative care. This condition is more common in males. We present the case of the oldest reported patient with this association. The patient was a 90-year-old female who was treated with a sigmoid resection for an obstructing sigmoid lesion. At operation, the left
ureter
was embedded within an inflammatory pericolic mass but was not grossly involved with tumor. The tumor was a
Dukes
' stage B adenocarcinoma and occurred in association with malakoplakia.
