The patient was a female aged 18. Hematuria was discovered at an examination two years previously at which time the patient was complaining of right lumbar pain. The patient visited our hospital with a chief complaint of right abdominal tumor in June, 1988. A retroperitoneal tumor, 77 X 45 mm in size, was observed by CT and ultrasonic examination, and the right kidney was in hydronephrosis. An operation was performed in September, 1988, based on a diagnosis of retroperitoneal tumor and hydronephrosis. The tumor was seen to have involved the right ureter. Subsequently the tumor was surgically removed; right total nephroureterectomy and partial cystectomy were performed at that time. The removed tumor was 90 X 70 mm in size and 120 g in weight; it was histopathologically diagnosed as extraskeletal Ewing sarcoma. A systemic examination was carried out postoperatively, but no abnormalities were observed. As of September, 1990, the patient had had no recurrence and was being kept observation on an outpatient basis. There have been a total of 210 case reports regarding extraskeletal Ewing sarcoma published worldwide, including our case and 26 cases in Japan, so far as we know. However those of a lesion of retroperitoneal origin are extremely rare, numbering only 11 cases worldwide and 2 Japan. Herein we report our case of extraskeletal Ewing sarcoma with reference to the other 209 cases reported in the literature to date.

A 25-year-old man presented with acute pain in the right lower quadrant of the abdomen and a locally palpable mass. The tumour was extirpated together with the right seminal vesicle, adjacent ureter and bladder wall. The histologic diagnosis was extraskeletal Ewing's sarcoma. No tumour tissue was seen within the seminal vesicle. Intensive postoperative chemotherapy was given and at 18-months follow-up the patient was apparently tumour-free.

We report a case of extraskeletal Ewing's sarcoma arising in the retroperitoneal cavity. The tumor was excised en bloc with the left kidney and infiltrated portion of the left ureter. Histological diagnosis was confirmed by both electron micrography and immunohistochemical staining using a newly developed mouse monoclonal antibody 5C11, which specifically reacts with Ewing's sarcoma. An adjuvant chemotherapy T-11 regimen has been successfully preventing the recurrence.

Extraosseous Ewing's sarcoma/primitive neuroectodermal tumor (ES/PNET) is a rare soft tissue tumor of childhood usually found in the extremities. The authors present the case of a 17-year-old girl who presented with right flank pain and hematuria and during operation was found to have a right ureteral mass. The histopathologic, immunohistochemical, ultrastructural, and cytogenetic characteristics of the excised mass were consistent with extraosseous ES/PNET. This is the first known reported case of extraosseous ES/ PNET of the ureter. The pathologic features and clinical management of this case, as well as a review of the literature, are presented.

An 18-year-old male patient with a history of Ewing sarcoma originally involving the right ilium was evaluated with an FDG PET/CT scan to evaluate the effect of salvage therapy after standard treatment failed and disease progressed to involve the right T12 pedicle. Autologous stem cell transplantation and cyberknife therapy resulted in mixed tumor response, with incidental note made of prominent areas of cortical FDG avidity in the right kidney. These regions demonstrated focal hypoenhancement on the corresponding diagnostic contrast-enhanced CT, which additionally demonstrated peripheral enhancement spanning the length of the right ureter. Clinical workup produced a diagnosis of acute pyelonephritis.
