Involvement of the ureter from metastatic renal cell carcinoma is doubtlessly uncommon. Five cases of ureteric secondaries are studied herein. All were ipsilateral and synchronous with the appearance of the primary tumor except for one that was diagnosed 5 years following nephrectomy. The most consistent clinical feature was that of hematuria which presented in all cases. Urography (IVP) and retrograde ureteropyelography (RUP) proved to be fundamental in the morphologic diagnosis of these lesions. Treatment was always by surgery. The foregoing was combined with immunotherapy (BCG) in one case and palliative external radiotherapy in another case. In all cases, tumor stage and grade were T3-T4 and G2-G3, respectively, and all but one patient presented extrarenal venous spread (3 cases V2 and 1 case V1). The regional lymph nodes were positive in all 5 cases. In 2 cases, the histopathologic examination revealed concomitant metastasis to the ipsilateral adrenal. Because prognosis is poor, the therapeutic alternatives and the possible indication of prophylactic nephroureterectomy in certain cases are discussed.

A case of ureteral metastasis from renal cell carcinoma is reported. A 71-year-old man who had received radical nephrectomy for left renal cell carcinoma, G1 and about two years earlier presented with asymptomatic macrohematuria. He had received interferon therapy and surgical treatments for bone metastasis two times after the operation. Cystoscopic examination revealed bleeding from the left residual ureter but CT scan showed no abnormal findings. Left ureterectomy and partial cystectomy was performed and a small finger tip-sized tumor was found at 13.5 cm above the ureteral orifice. Pathological examination showed metastatic renal cell carcinoma, G1 > G2. Histologically and clinically, this tumor seemed to have metastasized by a hematological pathway. Seventeen cases of ureteral metastasis of renal cell carcinoma have been reported previously in the Japanese literature.

Metastatic renal cell carcinoma commonly involves the lungs, bone, liver, adrenal glands, and brain. Less commonly affected organs include the gallbladder, thyroid, and pancreas. Even metastatic spread to the contralateral kidney and the bladder has been reported. Computed tomography is the standard imaging technique to evaluate for contralateral involvement. One of the disadvantages of computed tomography as a screening modality is its difficulty in identifying small ureteral lesions. We report a rare case of metastatic renal cell carcinoma in the contralateral ureter presenting as acute obstructive renal failure after radical nephrectomy.

Metachronous presentation of metastatic renal cell carcinoma (RCC) to the ureter is extremely rare. We report a solitary metachronous metastatic RCC in the contralateral ureter 14 months after right radical nephrectomy for Fuhrman grade 2 pT3a clear cell disease after the patient re-presented with gross hematuria. The proximal left ureteral lesion was excised followed by ileal-ureteral interposition. Pathologic examination confirmed metastatic RCC. To date, only 51 cases of metastatic RCC to the ureter have been reported, with only 6 occurring metachronously in the contralateral ureter. Also, we report the presence of focal extramedullary hematopoiesis occurring within this metastatic lesion.

Metastatic renal cell carcinoma (RCC) to the contralateral ureter is a rare phenomenon. We report a metastatic RCC to the contralateral ureter 5 months after right radical nephrectomy for Fuhrman grade 3/4 clear cell adenocarcinoma with pathologic T3 staging. The distal ureter was excised followed by partial ileal ureteral substitution. Pathology confirmed metastatic clear cell RCC Fuhrman grade 2/4. Ileal ureteral substitution has been shown to provide good long-term functional outcomes and should be considered as a possible option for surgical treatment of ureteral metastasis.
