A retrospective study of urogenital tract injuries treated in the Urology Unit of Port Harcourt University Teaching Hospital from January 1985 to December 1994 was done. There were 67 injuries in 62 patients. Fifty-five (82%) of these were blunt injuries. Males were more frequently injured (85%) than females. Patients younger than 60 years sustained 61 of the 67 injuries (91%) with peak incidence in the age groups 0-10 years (22%) and 21-30 years (28%). The urethra was the most frequently injured structure (49%) followed by the bladder (24%). The anatomically secured kidney and
ureter
as well as the fairly mobile penis and
scrotum
were much less frequently injured. The commonest aetiological factor was road traffic accident (57%). There were ten injuries (15%) resulting from industrial activities. Eight injuries (12%) were iatrogenic. Treatment was successful in 52 injuries (78%). It was satisfactory in the few urethral injuries that required repeated dilations. Four injuries (6%) resulted in permanent disability. There were two deaths (2%) both from iatrogenic causes involving an ureteric injury at hysterectomy and haemorrhage from cultural circumcision in the village. Prevention is the most effective way to avoid these injuries.

We reviewed the surgical statistics of the Department of Urology, Isesaki Municipal Hospital between June 1998 and May 2003. A total of 1940 surgeries were performed. These consisted of 399 surgeries on the kidney, adrenal, and pelvis, 212 on the

, 433 on the bladder, 256 on the prostate, 149 on the urethra and the penis, 192 on the

, 255 on the arteriovenous fistula and CAPD. In September 2003, a screening program for prostate cancer was started, and the number of prostate biopsies and total prostatectomies increased. As the adaptation of many procedures to laparoscopy expanded, the number of laparoscopic surgeries increased.

We report two cases of rare cystic dilatations of the ejaculatory system. In case 1, a 6-month-old boy was referred to us for the management of recurrent epididymo-orchitis (E-O) complicating open drainage and a colostomy was performed elsewhere for a purulent rectal discharge thought to be rectal duplication. Diagnostic imaging showed a retrovesical cyst. Urethrocystoscopy showed a swelling of the verumontanum. No fistula was seen between the cyst and rectum on colonoscopy. At laparotomy, both ejaculatory ducts and seminal vesicles were found to be fused into a mass with cystic dilatation of the ejaculatory duct. Intraoperative histopathology of the cyst identified a metaplastic epithelial lesion. The cyst was excised with bilateral vasoligation. Since surgery, 8 years ago, urination and defecation have been normal. In case 2, a 4-month-old boy presented with fever and a swollen right
scrotum
. Ultrasonography showed a retrovesical cyst. Right grade IV vesicoureteral reflux diagnosed on voiding cystourethrography was treated by
ureter
reimplantation (Cohen) but complicated by recurrent E-O. Urethrocystoscopy with retrograde contrast via the utriculus showed that the cyst opened on the verumontanum, that both ejaculatory ducts opened into the cyst, and there was reflux into the right vas deferens. Right vasoligation alone was performed through a scrotal approach. Although the cyst was not excised, there has been no recurrence of E-O nor enlargement of the cyst for 6 years. Cysts of the ejaculatory system should be considered in the etiology of recurrent E-O in prepubertal children and a high index of awareness is recommended.

Vater/caudal regression anomalies, Mullerian duct/aplasia, unilateral renal agenesis, and cervicothoracic somite anomalies association, and Coloboma, heart anomaly, choanal atresia, retardation, genital and ear anomalies syndrome has been considered in differential diagnosis.

We report 2 cases of inguinoscrotal hernias involving urologic organs. The first case involved an elderly gentleman with a history of micturition by squeezing his
scrotum
. He was diagnosed as having a right-sided indirect inguinal hernia involving the right
ureter
and bladder. Treatment was surgical. The second case involved an achondroplastic male who presented with acute kidney injury. He had bilateral hydronephrosis and ureteric obstruction secondary to an ureteroinguinal herniation bilaterally. The presentation, diagnosis, and treatment of inguinoscrotal hernias involving the bladder and ureters are discussed.
