Operations on inpatients during the 15-year period from 1975 to 1989 were analyzed statistically. A total of 3,791 operations were performed at our Department of Urology, including 717 operations on the kidney, 316 operations on the ureter, 583 operations on the bladder, 811 operations on the prostate, 136 operations on the urethra, 1,097 operations on the scrotum, 43 operations on the adrenal gland, 19 operations on the retroperitoneal space and 69 operations on other organs.

The statistics of urological operations performed at our department between Jan., 1, 1982 and Dec., 31, 1986 were reviewed. During the 5 year period, 2331 patients were admitted to our hospital and 2192 urological operations were performed on 1834 patients. These statistics were compared with earlier statistics for two periods (1972-1976 and 1977-1981) and the recent trend of urological operations was investigated. With regard to operated organ, operations on the kidney were the most frequent followed by those on the urinary bladder. Operations on the ureter decreased during the recent 5 year period and they were the fourth most frequent following those on the scrotum, scrotal contents and penis. Operations on the adrenal gland, retroperitoneal space and parathyroid gland have gradually increased. As in the former report, transurethral resection of bladder tumor (TUR-Bt) was the most frequent and nephrectomy, TUR of prostate (TUR-P) and renal allotransplantation have followed. TUR-Bt and TUR-P have increased more and more and due to the introduction of percutaneous nephrolithotripsy and transureteral lithotripsy, open surgery for urolithiasis has greatly decreased. Fetal diagnosis has vastly improved over the last decade. Ultrasound has become the imaging modality of choice. As real-time equipment has improved technologically, the ability to deduce subtle abnormalities has greatly increased. The fetal genitourinary tract may be evaluated for renal dysplasias, anomalies, or obstruction. Points of obstruction and, at times, the exact cause of obstruction may be deduced. Abnormalities of the ureter, bladder, urethra, scrotum, or reproductive system can be detected. Early diagnosis allows for proper individual or family counseling for hereditable disorders and chromosomal abnormalities. The perinatology team can be alerted to an abnormality that may require early neonatal surgical or medical intervention. Early treatment can prevent significant deterioration of renal function. The possibilities of prenatal intervention may be considered.

This study was performed to assess the changes occurring in the field of urology with developments in outpatient surgery during a 5 year period. Using ICD-9-CM code, data for all urology procedures performed in the main operating rooms, day surgery and laser centre operating rooms of a large, general hospital were collected for the years 1987-92. A substitution index (SI) was determined as the ratio of the number of outpatients to the total number of procedures and expressed as a percentage. Changes in the SI reflects the degree to which emphasis has shifted from conventional inpatient to outpatient surgery. It was found that 26% of urology procedures were being performed as outpatients in 1987, and this increased to 42% by 1992. When broken down according to organ, the greatest increase in SI from 1987 to 1992 was for the kidney (57%) and the least, the penis (2%), with ureter, urethra, testes and scrotum all revealing intermediate, but significant, increases (27, 28 and 24% respectively). This paper thus demonstrates that, as in other fields, urology has experienced a marked increase in outpatient surgery. With developments in surgical and anaesthetic techniques, financial pressures, changing physician and patient attitudes and technological advances, further increases in urology outpatient care can be expected.

A rare complication following a difficult unilateral 1-stage Fowler-Stephens orchiopexy resulting in ipsilateral distal ureteral obstruction is reported. To our knowledge excessive vasal traction during orchiopexy causing partial obstruction at the ureterovesical junction has not been previously described. The complication arose when the transected spermatic vessels allowed tension to be placed solely on the vas when the testis was placed in the scrotum. The vas in turn compressed the ureter during bladder filling.

Massive herniation of the bladder into the scrotum is uncommon and can have a deleterious effect on the upper urinary tract. We report a case where complete scrotal hernia of the bladder lead to obstructive renal failure due to torsion of the trigone. Most simple bladder hernias can be explained by a prostate or uretral obstacle. The mechanism in our patient with no known prostate or uretral disease was the passage of the bladder via a defective inguinal canal caused by excessive obesity. This type of massive hernia has been reported in Sumo boxers. The effect on the upper urinary tract may be related to a fold in the terminal ureter or more exceptionally in the trigone. Immediate treatment requires urine drainage above the obstacle and intensive care. After stabilization, the anatomic position of the excluded bladder is re-established with reconstruction of the inguinal area, usually with a prosthesis. Any obstacle below the bladder must be removed.

This article analysis the usage of B'-Scan in some special cases in clinical forensic medicine, such as the injuries of eyeball, urinosexual organs (include kidney, ureter, urinary bladder, urethra penis, scrotum, testis, uterus et al) liver-gallbladder-spleen, thyreoid and parathyreoid, and the position and course of the scars in the deep of the body. The result indicates that the B'-Scan can be used to display not only the morphological changes, such as the positions, characteristics and degrees of the injuries, but also the function when to be used combining with the Doppler.

A 12-year follow up result of hemicorporectomy in a patient is represented. The patient was operated several times since 1983 for massive perianal condylomas. In 1985 the ulcer with hard edges was revealed in the perianal region, spreading to the perineum and root of the scrotum. Biopsy data evidenced for epidermoid carcinoma Abdominoperineal extirpation of the rectum was carried out with broad dissection of the skin of the perineum and with resection of the seminal follicle. Postoperative period was complicated by prolonged pyogenous infection of the perineal wound which prevented from radiation treatment. 9 months later the relapse of the tumor was detected in the perineum with deep pyogenic fistulas formation. 6 courses of chemotherapy by 5-fluorouracyl were carried out. During the process of examination in September 1987 in the perineal area a large massive tumor occupying the whole pelvic cavity and growing into the posterior wall of the urine bladder and left ishial bone, spreading to the scrotal root and surrounded by the net of fistulous tracts was revealed. Hemicorporectomy was carried out with previously layed one-stem sygmostomy keeping intact, and retroperitoneal Y-shaped uretero-ureter anastomosis being formed and right ureter being fixed at the skin of the right abdominal wall. A special prosthesis--"a glass"--was made for the patient, in which he could move from the bed to the chair or the wheeled chair, to move at home or in the street and to drive his own car. Later, evacuation of the uroliths through the uretherocutaneous stoma was observed. Gradually urolithiasis progressed, mainly in the right kidney, and in 1995 the development of purulent rightsided paranephritis was detected which demanded right-sided nephrectomy. Thus, in spite of a number of complications we can state, that hemicorporectomy has cured the patient of advanced, cancer and he feels satisfied with this treatment and saving 12 years of life.

Urethral reconstruction following failed hypospadias repair or post-traumatic chronic stricture requires adequate amounts of tissue. Many surgical techniques utilizing different types of biological tissues have been attempted: (a) vascularized skin flaps from the prepuce, scrotum or penile shaft; (b) full-thickness free skin grafts; (c) vesical or buccal mucosa grafts; (d) ureter; artery; vein and appendix tissue. More recently, biodegradable polymers have also been used as delivery vehicles of urothelial cells in animals. It has been demonstrated that the implant of an acellular tissue matrix in the bladder can guide the regeneration of urothelium, blood vessels, smooth muscle and nerves. The aim of this study was to create an experimental model of urethral defect, and then repair it by implanting homologous acellular aortic grafts as urethral substitutes. An acellular matrix was obtained by detergent enzymatic treatment of rabbit thoracic aorta. The growth of urethral epithelium was verified in vitro, and homologous acellular vessels were then implanted in rabbits, bridging a previous surgical urethral defect. The outcome of reconstructive surgery was evaluated histologically at 10 days, 3 weeks, 3 and 12 months. As the time after surgery increased, the neourothelium became less thick, signs of inflammatory response disappeared, and the orientation of collagen fibrils and smooth muscle fascicles resembled that of a normal urethra. The implants displayed abundant vascularization, and the luminal surface started to become irregular. Acellular blood vessels may represent a promising approach to urethral defect therapy for different reasons: (a) unlimited availability, (b) readily obtainable in different lengths and gauges, (c) the potential for being organized as tissue bank, and (d) that just one simple surgical procedure is needed. Nevertheless, before this technique can be applied in humans, it must be tested in more species and animals.
