A case of emphysematous pyelonephritis with septic shock was present in a 58-year-old diabetic woman. The spontaneous production of gas was present within the right renal pelvis in kidney-ureter-bladder X-ray. The patient's condition deteriorated rapidly after admission, became complicated with acute renal failure, disseminated intravascular coagulation and acute respiratory failure. Transurethral drainage of the pelvis using a 6 Fr. UPJ occlusion balloon catheter and endotracheal intubation with respiratory assistance were performed as a life-saving procedure. The optimal therapy with surgical or conservative approach for such a severe condition is discussed. The use of transurethral drainage of pelvis as a non-invasive treatment is suggested.

Hepatocytes isolated from livers of rats with various models of acute uremia (binephrectomy, ureter ligation, uranyl nitrate-induced, or ischemic ARF) were incubated with glucagon, adrenalin, or cyclic AMP using serine as a substrate. A marked increase in glucose production was observed in the hepatocytes of uranyl nitrate-treated, binephrectomized, and ureter-ligated rats compared to starved controls or sham-operated animals. This effect was strengthened in the presence of glucagon, adrenaline, or cyclic AMP. In liver cells of binephrectomized and ureter-ligated animals, the production of acetoacetate and beta-hydroxybutyrate was significantly higher than in controls and sham-operated rats. Oxoglutarate and ATP production was only enhanced after ureter ligation. The correlation between glucose concentration and the cytosolic redox state was different in control and sham-operated rats than in either uremic group. This study confirms earlier investigations of a key role of serine in carbohydrate metabolism in acutely uremic rats.

Nine patients were operated on with the use of robotic complex. In 3 cases the nonorganic retroperitoneal pelvic tumor was removed, the 4 full-layer urinary bladder and ureteral orifice resections and simultaneous ureteral plasty together with ileal-obturatory lymphadenectomy (2 - cancer of the urinary bladder cervix, 1 - pheochromocytoma of the urinary bladder) and 2 ureterocele with complete ureter duplication and megaureter were performed. Technical aspects of the robotic layerwise resection of the neck of the urinary bladder were worked out. The operation time varied from 1 hour 45 minutes to 4 hours 45 minutes. The maximal blood loss was 100 ml. One patient developed the acute respiratory failure. The were no other postoperative complications. The postoperative hospital stay was 7-12 days.
