A series of 46 children treated by the author since January 1972 for congenital posterior urethral valves is presented: 22% were diagnosed at birth, 28% as neonates and 52% in the first 3 months of life. Ninety-three per cent had unilateral or bilateral dilatation of the upper urinary tract at the time the valves were diagnosed and 72% had
ureteric reflux
. Unilateral reflux occurred into the left
ureter
twice as often as the right. Renal failure was present at the time of diagnosis in 72% of all of the children but in 83% of those aged less than 3 months. Surface urinary diversion was used minimally during post-operative management and contributed little to the recovery of renal function. Reflux disappeared spontaneously in one-third of the refluxing ureters. Ureteric dilatation subsided spontaneously in 57% of dilated ureters. Surgery was performed mostly for reflux. Non-refluxing ureteric dilatation was made worse by surgery in a few instances and in others the dilatation improved with time rather than as a result of surgery. Renal function returned to normal in over 60% of the children who were in renal failure at diagnosis. Measurement of glomerular filtration rate was the most accurate method of predicting recovery of renal function: a value of less than 50% of normal for age at the time of diagnosis forecast persistent chronic renal failure with all its attendant complications.

Vesico-ureteral reflux
was the major associated anomaly (39 cases) involving most often the lower pole
ureter
; the surgical correction of the reflux was performed according to the technique of Cohen with a favorable outcome in all cases. Because of poorly functioning and dysplastic upper renal segment, partial upper pole nephrectomy was performed in 8 among 12 cases of associated ectopic ureteral orifice and in 9 among 15 cases of associated ureterocele. A reimplantation of the
ureter
was made possible in 3 cases of ectopic ureteral orifice and in 5 cases of ureterocele with little corresponding renal lesions. With the experience of one neonate who died from septic shock following partial nephrectomy there were no other post-operative complications in this series.

produces in a previously patent pyelo-
ureter
. Before bypassing, the median ureteric baseline activity was 1.6-2.0 A.P./min (action potentials/min). During bladder filling the median activity increased by 15-44% to 2.3 A.P./min, together with a slight increase in pelvic pressure. In the refluxing ureters the median baseline ureteric activity was 1.6 A.P./min. During bladder filling the pelvic pressure was the same as the bladder pressure and the median ureteric activity increased by 88% to 3.0 A.P./min. This increase in activity was seen as soon as the bladder filling started. The study adds information to suggest a neurogenic mediated reaction to sudden increases in bladder volume and pressure.

The dynamic function of the urinary tract was studied in 6 infravesically obstructed pigs with vesico-
ureteric reflux
to a total of 8 ureters. After 10 weeks of obstruction a bladder hyperreflexia was found. The baseline ureteric activity in the refluxing ureters was reduced to a median of 0.3 A.P./min (action potentials/min). During bladder filling, the activity increased only slightly to 0.6 A.P./min. In the start the pelvic pressure was more or less independent of the bladder pressure, but when the reflux producing bladder pressure was reached then an abrupt increase in pelvic pressure was seen and thereafter there was pressure equilibration. Ten weeks after release of the obstruction, a minor restitution had taken place. The baseline activity during low urine output was 1.0 A.P./min and during bladder filling it was unaltered, 1.0 A.P./min. Even during high urine output there was no sign of activity increase. 30% of the ureteric contractions stopped in the mid-third of the
ureter
. Almost all peristalsis was antegrade. Thus, ureters subjected to obstruction and reflux showed deterioration of the peristaltic function and the changes were irreversible.

Vesicoureteric reflux
is a common lower-tract abnormality. The seemingly paradoxical coexistence of these entities was found to occur nine times over the 10-year period, 1973 to 1983 (in about 2800 children with reflux and about 75 children with primary megaureter). This coexistence should be suspected on voiding cystourethrography when the juxtavesical segment of the megaureter is normal in caliber, the refluxed contrast agent is diluted by urine that is trapped in the
ureter
, and drainage of refluxed contrast agent into the bladder is impeded. The diagnosis can be confirmed by excretory urography with a catheter draining the bladder to temporarily prevent the reflux. The surgical treatment consists of resection of the distal aperistaltic segment and reimplantation of the
ureter

Vesicoureteric reflux
was produced in eight piglets by opening their bladders and slitting the anterior intravesical wall of the
ureter
. Cystography confirmed the presence of bilateral reflux in six piglets and unilateral reflux in two. Six to eight weeks later the bladder was again opened and Teflon paste injected in the space behind the intravesical
ureter
, thereby creating a support for the submucosal
ureter
. Cystography four to six weeks after injection of Teflon paste showed absence of reflux in all animals. Intravenous pyelography showed obstruction at the vesicoureteric junction in only one of the 14 treated ureters and this was later confirmed at necropsy. Animals were followed up from one to six months and then were killed. Gross examination of the vesicoureteric region showed a well circumscribed subureteric Teflon mass of firm consistency, retaining its shape and position at the site of the injection. Histological examination showed encapsulation of the implant by a thin layer of fibrous tissue and a foreign body granulomatous reaction with histiocytes and giant cells within the implant. Hence it is technically feasible to correct experimentally produced vesicoureteric reflux in the piglet by intravesical injection of Teflon paste--a relatively inert material. It may now be possible to treat vesicoureteric reflux in man by endoscopic injection of Teflon behind the intravesical

Of 49 children with renal ectopy, 25 (51%) had hydronephrosis on excretory urography. Dilatation of the renal pelvis was the result of primary ureteropelvic junction obstruction (17 children), significant vesicoureteral reflux (6 children), or extrarenal pelves and calices with renal malrotation producing apparent ureteropelvic junction obstruction (2 children).
Vesicoureteral reflux
was a common finding in these patients, occurring in 52% of those with crossed renal ectopy and in 70% of those with pelvic kidney. The need for surgical intervention results from either (1) ureteropelvic junction obstruction secondary to high insertion of the
ureter
on the renal pelvis or (2) vesicoureteral reflux secondary to aberrant migration of the lower
ureter
. Most children with renal ectopy and ureteropelvic dilatation require surgical reconstruction at the renal level. In selected patients, ureterocalicostomy is an attractive alternative to conventional pyeloplasty and appears to afford improved drainage with superior results.
