Four cases are presented with clinical diagnosis of scrotal varicocele on the left side, and one case with ureter varices and left-sided haematuria as a result of compression of the left renal vein between the aorta and superior mesenteric artery (SMA), also known as "nutcracker phenomenon". The clinical signs and the radiological diagnostic methods of the condition are discussed.

We examined 11 patients with acquired obstructive azoospermia resulting from irreparable obstruction of 1 vas deferens and severe damage to the contralateral testis. All of the patients underwent transseptal crossed vasovasostomy with no morbidity. Of 8 patients evaluated with postoperative semen analyses 4 (50 per cent) demonstrated total sperm counts of 29 to 205 million and 2 pregnancies (25 per cent) have been reported, with followup ranging from 5 months to 2 years. The etiologies of the vasal obstruction included previous inguinal surgery in 7 patients, vasectomy in 1, ejaculatory duct obstruction in 1, ectopic ureter in 1 and vasal agenesis in 1. Factors leading to loss of the contralateral testis were torsion in 5 patients, mumps orchitis in 2, varicocele in 1, pediatric inguinal herniorrhaphy in 1, epididymal blow out in 1 and unknown in 1. A representative case involving a unilateral ectopic ureter emptying into the seminal vesicle and subsequent contralateral testicular torsion is presented. The results indicate that a transseptal crossed vasovasostomy should be done in patients satisfying the criteria presented.

The years following the first laparoscopic nephrectomy in man performed in 1990 were evidencing wide spread of transabdominal laparoscopic approach in operative interventions on the kidney, adrenals, ureter, urinary bladder, in iliac lymphadenectomy for prostatic and bladder cancer, in the treatment of varicocele, etc. Since 1993 we have made 4 nephrectomies using laparoscopic approach. The first operation in a 18-year-old male with parenchymatous hypertension lasted 7 hours, the following operations took from 2 hours 47 min to 1 hour 30 min. No complications occurred. Postoperative stay in hospital was, on the average, 7.2 days. The laparoscopic approach in urology and surgical nephrology of adults and children needs detailed studies, specification of indications and contraindications prior to its introduction into wide practice.

828 patients were operated on the retroperitoneal organs since 1985 using a retroperitoneoscope. A total of 836 surgical interventions were performed. A cut of 3.0-3.5 cm long was made to revise the kidney and ureter, retroperitoneal part of the colon, tail of the pancreas, retroperitoneal vessels and lymph nodes via lumbocostal and inguinal approaches. A new intercostal (ribs X-XI) approach is proposed. Most of the patients had urolithiasis, pyoinflammatory lesions, renal cysts, varicocele, obstructive uropathies. Complications developed in 1.7% of cases. Retroperitoneoscopy is a method of choice in diagnosis and treatment of retroperitoneal diseases.

We report the case of a young woman admitted because of several and recurring episodes of macroscopic hematuria beginned after her first pregnancy. Contrast-enhanced multidetector computed tomography images showed dilated ovarian veins due to a typical pelvic varicocele. We supposed to be a right ovarian vein syndrome, a rare clinical situation characterized by an anomalous compression of the lumbar ureter by the ectasic ovarian vein; this condition may cause a chronic inflammatory stimulus above the urothelial mucosa with a following hematuria. All symptoms were solved by an endovascular treatment through the sclero-embolisation of the pelvic varicocele. After eighteen months the patient didn't present hematuria anymore and she no longer complained about her right side lumbar pain.

A previously healthy 30-year-old man with a symptomatic varicocele underwent gonadal vein embolization using nickel vascular plugs. He developed a painful hypersensitivity to his nickel plugs and elected to pursue laparoscopic excision and proximal gonadal vein ligation. In the operating room, the gonadal vein was isolated from the ureter, and ligated proximal to the cephalad plug and distal to the caudal coil. His pain is completely resolved 5 months after surgery. Metal allergies are well documented in orthopedics and cardiology implants, but there are a limited number of case reports of metal allergies after varicocele embolization. Interestingly, nickel is the most common type of metal hypersensitivity.

Intrathoracic kidney is a partial or complete displacement of the kidney above the hemidiaphragm into the mediastinal compartment of the thorax. It is usually seen as an incidental finding discovered on chest radiograph or abdominal ultrasound. However computed tomography consents the correct detection of intrathoracic masses and defines their shape, size, and extent. We here report a case of ectopic thoracic kidney in a 22-year-old man who had a long history of scrotal discomfort associated with right varicocele. Frequently, this ectopia does not affect renal function and the stretched ureter provides good drainage. In literature, a small number of cases shows that varicocele is a possible mode of presentation of kidney tumors, but this is the first case of varicocele secondary to intrathoracic kidney ectopia.
