The incidence of clinical maternal infections was determined in a prospective study of 44,016 pregnant women at 12 university hospitals. Viral and bacterial infections were the most frequently reported. Overall, the highest incidence of infections was for
vaginitis
, influenza/"flu"-like disease, and infections of the kidney,
ureter
, bladder; parasitic and fungal infections occurred at low rates. Most women experienced only one infection during pregnancy (89.7%); however, some had two different infections (8.5%), and a few had three or more (1.7%). The analysis of the effects of some of the maternal infections on the pregnant women and their children have been published, and further studies are in progress.

This article presents a case in which a diaphragm, in place in the vagina of a 61-year old woman for 37 years, caused vesicovaginal fistula and genitourinary tract infections. The patient presented with a history of foul smelling watery vaginal discharge for the preceding 10-15 years. The pathology specimen revealed
vaginitis
, cervicitis, endometritis, salpingitis with tubo-ovarian adhesions, and chronic cystitis in conjunction with a bladder calculus. The vertically positioned diaphragm was inspissated and calcified. The position of the diaphragm had not interfered with menstruation. A hysterectomy, bilateral salpingo-oophorectomy, cystectomy and anterior vaginectomy, right nephroureterectomy, and appendectomy were performed, and the left
ureter
had to be anastomosed to a right lower quadrant ileal loop. The postoperative course was uneventful. This case illustrates the serious complications that can accompany prolonged diaphragm neglect; this patient had no contact with a physician following diaphragm insertion.

Childhood gynecologic problems are different from adult problems in physiology, disease, psychology, evaluation and management. The most common is vulvovaginitis. The child is susceptible to vulvovaginitis because of a relatively exposed vulva, a thin vaginal wall and poor hygiene. Additional problems are heat and moisture, clothing, coverings and possible sexual abuse. Most vulvovaginitis is primary, nonspecific vulvitis with secondary
vaginitis
. It is set off by poor hygiene and responds to keeping the vulva clean, cool and dry. Persistent or recurrent vulvovaginitis may be due to a variety of causes, including vulvar skin disease, a foreign body in the vagina, primary
vaginitis
, ectopic
ureter
and rhabdomyosarcoma. Vaginoscopy is indicated for recurrent vulvovaginitis or bleeding and for suspicion of a foreign body, neoplasm or congenital anomaly. The extent of evaluation depends on the anxiety of the child versus the extent of the problem, and therefore individualization is required.
