Six cases of uretero-
vaginal fistula
are presented. Five patients developed this complication after gynaecological surgery and one after Caesarian section. Constant urinary incontinence combined with normal voiding is typical of this lesion. Excretory urography revealed a stricture in the distal part of the
ureter
in 5 cases, while in one patient the urographic findings were normal. Accumulation of contrast medium in the vagina was seen on urography in 2 patients. Surgical correction is the treatment of choice, and all patients were successfully treated by resection of the distal part of the
ureter
with reimplantation of the
ureter
into the bladder.

In 908 radical hysterectomies including dissection of pelvic lymph nodes, 13 (= 1.43%) urogenital fistulas were seen. They occurred postoperatively between the 7th and 15th day. In 12 cases it was a uretero-vaginal and once a vesico-

vaginal fistula
reveals difficulties in separating the
ureter
in its prevesical part proceeding from tumour spread, endometriosis, scar or bleeding. Since 1985 a percutaneous nephrostomy has been performed (5 cases). 4 fistulas were cured spontaneously, one of them had a percutaneous nephrostomy. In all cases, urography was normal. In 7 cases, ureterocystoneostomy had to be performed. This was done once with fixation of the psoas muscle and 6 times according to Boari. All patients were cured without any complications and with an undisturbed drainage assessed 1 year later by urography. 1 patient, who refused any follow-up, died subsequently. The only vesico-
vaginal fistula
has been closed by Latzko's procedure. In recent years the problems concerning urogenital fistulas resulting from surgery of cervical cancer have receded. However, if patients adopt an offensive attitude against surgical therapy of stage T2b, the rate of incidence of this complication can be expected to increase.

The authors studied the complications and other current problems of surgery of 212 cases subjected to Wertheim-Okabayashi operation. Of them 118 were cervix cancer in stage I and 90 in stage II and 4 were cancer of the uterus body in stage II. Two cases of operation death, 1 case of

Cervical carcinomas of stage Ib and II have been treated for extended radical therapy by radioisotope radical surgery at the First Department of Obstetrics and Gynecology, University of Vienna, Austria. This extension of a more radical surgery requires to take measures in order to prevent urological and other postoperative complications. Chromocystoscopy, intravenous urography and functional scintigraphy of the kidney are carried out as routine preoperative investigations. Preservation of the adureter and an exact drainage of the field of operation are taken as intraoperative measures. Main emphasis of postoperative prophylaxis is the stimulation of
ureter
activity by distigminbromide and hexoprenaline. Distigminbromide stimulates the prevesical part of the
ureter
and increases the number of urinary excretions into the bladder. Hexoprenaline is a betamimetic substance which increases both local blood flow and number of
ureter
contractions. In addition, treatment by antibiotics, thrombosis prophylaxis with heparin and marcoumar and urine drainage by catheter are performed during surgery. The results of 187 radioisotope radical operations show that an uretero
vaginal fistula
was observed ruly in one patient (0.53%). Hydronephrosis was registered in 10 females (5.3%) and 42 subjects (22.5%) lack of bladder feeling was noted and incontinence was observed in 22 patients (11.8%) in the postoperative period.

Ureterocutaneostomy, the anastomosis of the
ureter
to the skin, represents the most simple supravesical urinary derivation. It can be performed as unilateral, bilateral, bilateral with one orifice, or rarely as transuretero-ureterocutaneostomy. In period 1993-1998 on the Clinic of Urology, Clinical Center of Serbia, ureterocutaneostomy was performed in 155 patients as definitive type of urinary derivation. The most frequent indication for this type of derivation was the bladder tumor (130 out of 155 pts.). The rest of 25 pts. had carcinoma of vaginal portion of uterus, vesico-
vaginal fistula
(most frequently after irradiation), high ureteric lesions bladder neurogenic disfunction or rare bladder diseases. Having in mind short period of follow up two major complications were noticed: stenosis of uretero-cutaneostomy and retraction of anastomosis with consecutive formation of subcutaneous urinoma and abscesses formation. Although rarely used as definitive type of urinary anastomosis, ureterocutaneostomy has its place in wide spectrum of urinary derivations, since it is the only way to help those patients that have no other option of urinary derivation that might provide better way of social and professional rehabilitation.

Between November 1994 to May 1996, there was a total of eight ureteric injuries out of 2495 major gynaecologic operations and one ureteric injury out of 4146 caesarean sections at the Kandang Kerbau Hospital, giving the incidence to be 0.3% and 0.02% respectively. Antecedent operations were four abdominal hysterectomies, two Werthiem's hysterectomies, one laparoscopic-assisted vaginal hysterectomy and one caesarean section. All operations were performed by qualified specialists. Three patients had previous abdominal operations and five patients had procedures complicated by dense adhesions. Only one injury was detected intraoperatively and the rest presented with uretero-
vaginal fistula
. One presented with immediate postoperative anuria. The average time interval for diagnosis ranged between one day and twenty-three days (average 10.7 days). The commonest damage was transection of the
ureter
. Double J stents were used in all repairs with two cases requiring reimplantation with psoas hitch, and two cases of ureteroneocystostomies and one case of primary reanastomosis. There were no mortality in our series.
