Medial displacement of the lower end of the ureter is usually indicative of a mass lesion in the pelvis. We present two women in whom medial deviation of a distal ureter was shown by computed tomography to be caused by a full bladder itself indented by the posteriorly situated uterus.

For this study 52 formaldehyde-fixed pelvis from both sexes dogs (Canis familiaris), unknown races and different ages, injected with latex through the aorta, were used. The results were classified in common (A) and uncommon (B, B1, B2 and B3) modalities: A-44 dogs (84.62%; 74.82-94.42%), with emergence of the urogenital artery from the visceral branch of the hypogastric artery; B-5 dogs (9.62%; 2.70-22.10%), where the vessel arises from the umbilical artery. In this manner, the intrapelvic organs (ureter, deferent duct, bladder, prostate and urethra in male, and ureter, bladder, urethra, uterus and vagina in female) are supplied with blood through the urogenital and umbilical arteries, and urethral branch, as well as the cranial and ventral vesical arteries to the bladder.

A case of malformation of the urogenital system with a persistent urogenital sinus and a cloaca is reported. There was unilateral agenesis of the kidney, megaureter and a double ureter with an ureteral orifice into the vagina and the clitoris. There was a double uterus and vagina with an orifice into the terminal gut. The patient had chronic recurrent urinary tract infections which required a colostomy during childhood. The operation largely restored a normal urogenital area and a functional anus.

Contractions were chemically induced in vitro in pig uterus segments by a high potassium concentration in a metabolic solution. Slow contractions were found mainly in the transverse direction. The near absence of contractions in the longitudinal direction is ascribed to a nearly circular orientation of the muscle fibers. (In terms of the helical muscle-fiber model of the ureter, our findings would correspond to a pitch of 3 to 5 degrees for the helix.) It is further postulated that the coupling structure of fibers in the transverse direction, determining an annular contraction mode, differs from that in the longitudinal direction, determining the longitudinal propagation of the annular contraction mode. The over-all conclusions on the basis of our results is that the helical muscle-fiber model now generally assumed for the ureter must be greatly modified to correlate with the experimental facts that its continued use is misleading and undesirable.

Reported in this paper is a rare urogenital malformation in a patient, 17 years of age. Extensive presurgical diagnosis of cele and cysts formation in the minor pelvis, with the right parts of vagina and ureter of a doubled urine-excreting system being involved, was confirmed by surgical therapy in two sessions. This hydronephrotic ureter, with its normal ostium located in the urinary bladder, was found to communicate with the right cervix of a double-bicorn uterus. Excess pressure in the cavities caused fluid to empty splashingly from the right vagina into the left which was in normal condition. The effects of such combination of malformations on individual life are discussed.

The authors report a new case of uretero-uterine fistula and on this basis review the 9 other publications devoted to this subject since 1971. In their own case, the diagnosis was based upon the clinical picture only : permanent flow of urine through the uterine cervix with persistence of normal micturation and left lumbar pain following a caesarian section. Uretero-pyelo-calyceal distension proximal to a low pelvic ureteric stenosis visible by IVU was confirmed by RUP which succeeded in opacifying only the final 3 cm of the ureter. Only peroperative opacification of the ureter showed the fistula tract with the uterus. This was successfully treated by uretero-vesical reimplantation into a psoas bladder. Review of the literature revealed the following points : --The ureter is damaged by blind haemostasis of bleeding at the lateral angle of the hysterotomy, most often on the left, as a result of dextrorotation. --Clinical features consist of the urinary fistula, pain and infectious complications with upper urinary excretory tract obstruction. --The ureteric lesion is situated very low down, between 2 and 4 cm above the ureteric meatus. --Uretero-vesical reimplantation into a tubular vesical flap or a psoas bladder ensures treatment when the kidney remains functional. --The prevention of such lesions may be summarised very briefly : beware of the ureter at the lateral angles of hysterotomy for caesarian section.

The long term result after the operative treatment of 29 ureteral strictures by various methods in 19 patients treated radically for a malignant tumor of the uterus was evaluated an average of 50 months after the urologic reconstructive surgery. Four patients died of repeat carcinoma and one patient died of a pulmonary embolism before the follow-up examination. In the other patients, a clinical examination, isotopic nephrography, intravenous pyelography, midstream culture and sediment as well as serum creatinine assay demonstrated that transureteroureterostomy resulted in a good, or satisfactory, outcome in patients with a unilateral stricture. A similar outcome was obtained by ileal replacement of the ureter, provided renal function had not been impaired preoperatively. The results obtained with the bladder flap operation, ureterolysis and cutaneous ureterostomy were poor. The significance of early detection and active therapy for strictures is emphasized. Nonradiated tissue should be used for the reconstructive procedure.

A very rare case of cloacal exstrophy associated with other multiple congenital anomalies is presented.Other congenital anomalies included exomphalos; myelomeningocele; microcolon with blind end and rectal agenesis; absence of external genitalia, appendix, pubic arch with separation of rami, left kidney, and ureter; duplication of the uterus and vagina; lacunar defect of cranial bones; Arnold-Chiari malformations; and hydrocephalus.The literature on this subject is reviewed.
